Owner could not provide proof of alarm set point test required in 46 CFR 143.245. Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. Main engine low lubricating oil pressure, Main engine high cooling water temperature, Auxiliary generator engine low lubricating oil pressure, Auxiliary generator engine high cooling water temperature. Perform test in presence of Coast Guard marine inspector or provide qualified technician report to the satisfaction of the attending Coast Guard marine inspector

Vessel has dime size hole in bow rake. Owner shall provide repair proposal or transit plan to repair facility prior to departure. Vessel shall not operate with passengers onboard.

Vsl Rep reported vessel was taking on water in the port side bow rake. The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be question by the OCMI. Vessel will receive temp repairs tested to the satisfaction of the marine surveyor for the affected area. Inspect all compartment daily, inspected affected compartment four times each day, have electric pump onboard the vessel for de-watering. If there are any changes to the damage area contact the local OCMI.

VHF–FM radio installed and capable of monitoring VHF–FM Channels 13 and 16, except when transmitting or receiving traffic on other VHF–FM channels. VHF Radio used to Monitor Channel 16 will not tranmit. Make repairs and or replace.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Generator Battery not secured. Secure battery to prevent tipping.

Each towing vessel must be equipped with at least one fire axe that is readily accessible for use from the exterior of the vessel. Fire axe bracket degraded and falling apart. Properly secure fire axe.

Each lifebuoy on a towing vessel must: (1) Be approved under approval series 160.050 or 160.150. Replace retroflective material

Replace main engine fuel hose in between primary and secondary fuel filters. (hose chafed revealing steel mesh inner sheathing).

Drill unsat, poor communication.

Primary generator failed. Temporary generator installed by electrician. A new generator has been sourced and ordered. The new generator is slightly different from the previous approved generator. Installation will be overseen and approved by the attending Coast Guard Marine Inspector. Owner must notify Marine Inspector prior to installation.

Clutch failed on the main engine while underway making engine non operable. Machinery must be designed and maintained to provide safe operations of the towing vessel and safety of persons onboard. Conduct repairs to the satisfaction of the attending Coast Guard marine inspector.

Deck hand did not have communication equipment to communicate with the pilothouse Each towing vessel must be fitted with a communication system between pilothouse and the engine room that consist of either fixed or portable equipment. Provide deck hand with communications equipment at all times to the satisfaction of the attending Coast Guard Marine Inspector.

Life buoy did not have 60 feet of line for life line. Each life line must have 60 feet of line. Replace to the satisfaction of the attending Coast Guard Marine inspector.

Port nematic air line to the connection system from tug to barge was worn so inner core was visible. Machinery and electrical systems must maintained to provide safe operation of the towing vessel. Replace nematic air line to the satisfaction of the attending Coast Guard Marine Inspector.

Hydraulic hoses on the bow that are used for the vessel locking and pivoting system from the tug to barge are not protected by chaffing gear. Machinery and electrical systems must be designed and maintained to provide safe operations of the towing vessel. Provide chaffing gear for hydraulic hoses to the satisfaction of the attending Coast Guard Marin inspector.

Crew of the ITV JENNY ANN located a one inch in diameter hole in the number two void on 03/16/2021. Vessel representative determined to have vessel transit to drydock without notifying the local OCMI until 03/17/21 putting the vessel, crew and port at risk of an actively flooding vessel. If the master or officer in charge of a navigational watch believes it is unsafe for the vessel to proceed, that an operation endangers the vessel or crew, or that an unsafe condition exists, he or she must ensure that adequate corrective action is taken and must not proceed until it is safe to do so. Vessel shall maintain its mooring at Aqua Marina Boat Ramp MM 464 Ohio River until permitted to proceed with a towing assist vessel approved by Coast Guard MSD Cincinnati. Once approved to proceed to C&B shipyard Hebron KY vessel representative shall make repairs to the satisfaction of the attending Coast Guard Marine Inspector.

Crew observed list of vessel and located active flooding coming from a one inch hole in the number two void. Crew decided to depart port of Augusta KY and transit to Hebron KY knowing watertight integrity was compromised. A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Vessel shall maintain its mooring at Aqua Marina Boat Ramp MM 464 Ohio River until permitted to proceed with a towing assist vessel approved by Coast Guard MSD Cincinnati. Once approved to proceed to C&B shipyard Hebron KY vessel representative shall make repairs to the satisfaction of the attending Coast Guard Marine Inspector.

Vessel does not have installed alarms for monitoring emergency conditions. Each towing vessel must have a reliable means to provide notification when and emergency conditions exist or essential system develops a problem that requires attention. The following alarms must be provided. Main engine and auxiliary generator low lubricating oil pressure. Main engine and auxiliary generator high cooling water temperature. Alarms must be visible and audible at the operating station and may be a summary alarm. Install all alarms to the satisfaction of the attending Coast Guard marine inspector.

Owner had not conducted general health and safety plan annual refresher training. Refresher training must be repeated annually and may be conducted over time in modules covering specific topics. Conduct annual refresher training on general health and safety plan and provide documenation attesting to the satisfactory completed by all master and crew to the satisfaction of the attending Coast Guard Marine inspector.
