Machinery spaces of category A shall be adequately ventilated so as to ensure that when machinery or boilers therein are operating at full power in all weather conditions including heavy weather, an adequate supply of air is maintained to the spaces for the safety and comfort of personnel and the operation of the machinery. PSCO observed danger tag on "STBD-Aft Engine Room Supply Ventilating Fan". Vessel confirmed fan is inoperable.

The condition of the ship and it's equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea wihtout danger to the ship or persons on board. PSCO's obeserved the door to the electrical room on A-Deck, identified as an A-60 fire boundary on the fire control plan, propped open and server racks/distribution panels within the space propped open due to overheating. The crew indicated that some of the equipment was recently installed and the ventilation is no longer adequate for the space. Additionally, PSCO's observed missing bolts to the fresh air intake vent with fire damper on both port and starboard sides of the F-Deck. SOLAS20/I-11

The master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the waters of the United States must follow these practices: (g) Maintain a Ballast Water Management (BWM) Plan that has been developed specifically for the vessel and that will allow those responsible for the plans implementation to understand and follow the vessel's BWM strategy and comply with the requirements of this subpart. The plan must include -(1) Detailed Safety Procedures PSCO observed vessel stowage sewage in #8 P/STBD WBT. The vessel's ballast water management plan indicates that this tank is used for ballast water and does not address it's use as a sewage holding tank isolation procedures from other WBTs, bypassing of the installed BWTS, or cross contamination of the ballast water piping. 33 CFR 151.2050(g)(1)

The Company should establish procedures to ensure that they ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The operational nature of the deficiencies noted below provide objective evidence of a serious failure of the implementation of the ISM Code. An external audit is required. 74 SOLAS (20) IX/3.1 ISM CODE 10.1

The arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. Excessive fuel oil leaks were observed on the main engine fuel delivery piping system, HFO service tank, F.O. purifier #1, D.O. purifier #1, aux generators #2 and #4, and G/E F.O. heat exchanger piping. Modified catch basins fitted on the aux. generators and M/E were filled with oil despite being required to be cleaned out per ship’s SMS procedures prior to operating under unmanned machinery space. 74 SOLAS (97) II-2/15.2

...Oil fuel pipe lines shall be screened or otherwise suitably protected to avoid as far as practicable oil spray or oil leakages on to hot surfaces... Leakages from high-pressure oil fuel pipes shall be collected. Multiple high pressure fuel line jacketing on the main engine cylinders were found deteriorated which caused the fuel lines to be exposed and leak at the connection points on to the M/E casing. The oil delivery piping between fuel injectors #5 and #6 was actively leaking and sealed with welding epoxy. 74 SOLAS (97) II-2/15.5.1

Oil fuel pipes and their valves and fittings shall be of steel or other approved material... Flexible hoses were found to be used throughout the engine room being used to collect and transport oil. A modified connection on the HFO settling tank piping was observed connecting a flexible hose to another oil storage tank. One flexible hose ran from the auxiliary generator to underneath the deck plates where a modified catch basin was placed. Additionally, a manufactured catch basin was found under a main engine cylinder that contained approx. 1 liter of oil and was fitted with a flex hose to drain oil to another collection point. Lastly, approx. 10-foot metal pipe labeled “TRANSFER PIPE FWD SETT TO 7 STBD” was found with a rag stuffed in it with dried oil. 74 SOLAS (97) II-2/15.2.8

Every ship... shall carry a garbage management plan which the crew shall follow. While in the Engine Room, an excess of 50 oil/chemical containers and catch basins were found stored in a single void space near the HFO tank. Additionally, oily rags, garbage, and discarded engineering equipment were scattered throughout the Engine Room. Bags of oily rags were being stored in a clean rag receptacle and one oily rag receptacle contained mixed plastics, oil-soaked rags, and metals. MARPOL V/10.2

… Boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Steam piping from boiler leading to accommodation spaces was leaking on the deck near the boiler at a rate of 3-5 drops per second. 74 SOLAS (04) II-I/26.1

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed multiple disconnected grounding wires on pieces of machinery as well as missing stuffing tubes exposing wiring insulation on 440V equipment throughout the engine room creating an electrical hazard to the crew. 74 SOLAS (04) II-I/54.2.8

Bulkheads forming boundaries between cargo spaces and machinery spaces of category A shall be insulated to ‘A-60’ standard. Insulation on the main engine machinery space bulkhead was missing. 74 SOLAS (97) II-2/54.2.8

After any survey of the ship… no changes shall be made in the structural arrangements, machinery, equipment, and other items covered by the survey, without the sanction of the Administration. Lube oil purifier #1 was partially disassembled and taken out of operation by the crew without utilizing proper lock out/tag out procedures or approval. 74 SOLAS (20) I/11b

Machinery and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Hydraulic piping for mooring line auto-tension system was sheared off. Cite: 74 SOLAS (04) II-1/26.1

MAIN AND AUXILLARY MACHINERY ESSENTIAL FOR THE PROPULSION AND SAFETY OF THE SHIP SHALL BE PROVIDED WITH EFFECTIVE MEANS FOR ITS OPERATION AND CONTROL. DURING DEPATURE VESSEL LOSS PROPULSION DUE TO AN ISSUE WITH THE BACKUP ENGINE CONTROL ROOM SYSTEM.
