Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Vessel reported approximately 1.5" hole above waterline in 2P void, 2.5' above waterline. The vessel is permitted to conduct temporary repairs followed by transit to Port Arthur, TX for a one time discharge of cargo followed by permanent repairs in the MSU Port Arthur OCMI zone. 46 CFR 32.60-1(a)

Each cargo tank of a tank barge with a Vapor Control System must be equipped with an intrinsically safe high-level alarm that activates an audible and visible alarm so that it can be heard and seen on the vessel where cargo is controlled. Marine inspectors are not limited from conducting tests or inspections deemed necessary to be assured of the proper operation of vessel equipment. Audible and visible alarms were unable to be tested in the presence of USCG Marine Inspectors during the annual inspection. Prior to next loading operation using the Vapor Control System, test alarms to the satisfaction of the attending USCG Marine Inspector.

CCM-3140 was due to have annual inspection completed by 03 Sep 2013.

Vessel prohibited from carrying cargoes that require use of the VCS until annual testing is completed.

Prove proper setting on cargo safety relief valve.

Make permanent/proper repairs to cargo pump emergency shutdown cable prior to conducting cargo operations.

Replace all torn cargo tank flame screens

Remove rope preventing emergency stop capabilities on cargo pump

Remove cargo hose from vsl and/or test as required and properly label

Ensure obstructions are removed from top of P/V valve to allow proper operation.

Vessel was given an extentsion for their ISE. Vessel was not gas freed and was /loaded during COI.Inspection of VCS was not conducted.

Cargo piping test was not conducted annually as required.

Complete damage survey and make necessary repairs to the satisfaction of an attending Coast Guard Marine Inspector prior to commencing cargo operations.
