Resolution: STBD half drive No. 02 had the PCB 101C card replaced in addition to the exciter bridge unit by the vessel crew, and the trip circuit verified. Upon completion the vessel completed ahead to astern movements at anchor with no alarms/failures. All sat.

Where pipes, scuppers, electric cables, etc are carried through watertight bulkheads, arrangements shall be made to ensure the watertight integrity of the bulkheads. Both port and starboard shaft seals failed allowing an excessive amount of sea water through both shaft tubes into the machinery space making the vessel not seaworthy. Ensure shaft seals are replaced with proper seals and sea water entry is minimal amount of water necessary for proper lubrication of shafts prior to departure from Big Stone Anchorage.

Each cable entrance into explosion proof or flameproof equipment must be made with approved seal fittings, termination fittings or glands that meet the requirements of 111.105-9. All thermal couple fittings on cargo pumps, stripping pumps and COW pumps and vibration monitoring devices need to be inspected and where needed renewed packing glands and wiring connections shall be repaired replaced.
