PSCO observed a crew member assigned to life boat #10, the second in command of life boat #6, MES, and member of muster station C were not familiar with their emergency duties. 74SOLAS20, III/19.2.1, 17c

Stairways and ladders shall be arranged to provide ready means of escape to the lifeboat and life raft embarkation deck from all passengers and crew spaces and from spaces in which the crew is normally employed, other than machinery spaces. PSCO observed security equipment including x-ray machines and metal detectors stored in the stairway outside of muster station D port deck side 7 blocking access evacuation routes from muster station D to the embarkation deck. 74SOLAS01, II-2/28.1, 16c

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCO observed a portable fire extinguisher at 14.v.ii tied to bulkhead and missing bracket. 74SOLAS20, II-2/14.2.1.2, 16c

The construction of all doors and door frames in A class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and oor frames shall be constructed of steel or other equivalent material. PSCO observed fire door FSD01-6.3 not maintaining its A-60 boundary and FSD 04.4.27 not fully latching. 74 SOLAS 01 II-2/30.2 10c

Fire hoses shall be of non-perishable material approved by the Admiistration and be sufficient in length to protect a jet of water to any of the spaces in which they may be required to be used. Each hose shall be provided with a nozzle and the necessary couplings. In interior locations in passenger ships carrying more than 36 passengers, fire hoses shall be connected to the hydrants at all times. PSCO observed fire stations 10.ii.3, 11.iv.4, 11.iii.4 and 12.v.1 with hose not connected to hydrant. 74 SOLAS 01 II-2/4.7.1 10c

For deterring the appropriate fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are classified according to their fire risk as shown in the categories (1) to (14) below. PSCO observed combustible materials incorrectly stowed in technical spaces: 1) Under stairwell near FSD 15.3.7 and 2) technical space near cabin A320. 74 SOLAS 20 II-2/9.2.2.3.2.2 10c

All waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed a plastic trash can labeled scrap metal with combustible materials in the Bosun store, deck 6, fire zone 1 SOLAS 01 II-2/18.5 10c

Before a ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Port rescue boat failed to start when initially tested. 74 SOLAS 20 III/20.2

THE STBD PROPULSION ELECTRICAL MOTOR EXCITOR CABLE LUG CONNECTION WAS DAMAGED DUE TO OVERHEATING AND CAN ONLY BE OPERATED AT 40% OF RATED TORQUE CAPACITY DURING NAVIGATION.

Crewmembers shall be trained to be familiar with the arrangements with the ship as well as the location and operation of any firefighting systems and appliances that they may be called upon to use. During the abandon ship drill observed deck 6 midship stairway guide who was unfamiliar with IMO signage for firefighting equipment. During the abandon ship drill deck 9 midship stairway guide, deck 9 forward stairway guides and deck 14 forward stairway guide were unfamiliar with the use of basic firefighting appliances including fire doors and fire extinguishers. 74 SOLAS 20 II-2/14.2.2.1; 07199; 17c

On-board training in the use of the ship's life-saving appliances, including survival craft equipment, and in the use of the ship's fire-extinguishing appliances shall be given as soon as possible but not later than two weeks after a crew member joins the ship. Lifeboat commander for lifeboat #11 was unfamiliar with basic lifeboat operation. Second in command for lifeboat #17 was unfamiliar with basic lifeboat starting procedures. 74 SOLAS 20 III/19.4.1; 11131; 17c

Safe escape routes shall be provided. In the main laundry, a storage room was converted to a normally employed tailor shop. Caging surrounding the workshop was being blocked by plastic film and textiles impeding visibility. No audible alarm was provided for the space. 74 SOLAS 20 II-2/13.1.1; 07199; 16ac

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. In the ship's medical center, LLL was being blocked by seating. On other side, LLL was missing surrounding the reception desk. 74 SOLAS 01 II-2/28.1.5.10; 07120; 16ac

Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: Remote-controlled sliding or power-ooperated doors shall be equipped with an alarm that sounds as least 5s but no more than 10s before the door begins to move and continues sounding until the door is completely closed. Sliding FSD 11.6.16 did not alarm upon activation. 74 SOLAS 01 II-2/30.4.10; 07105; 40ac

The activation of any detector or manually operated call point shall initiate a visual and audible fire signal at the control panel and indicating units. Manual call points 71.092, 71.091, 71.043, 59.054 did not activate when testing. Smoke detector 47.064 did not activate when testing. 74 SOLAS 01 II-2/13.1.4; 07106; 40ac

NFPA Code 30 Flammable Liquid Locker was used to stow flammable products for the spa in a category (13) space called "Scuba Storage". PSCO observed stowage of non-fixed seating, carts, carpets, signage, engineering spare parts, and combustible materials being stowed in Category (2) stairways. Below the bulkhead deck two means of escape, at least one of which shall be independent of watertight doors, shall be provided from each watertight compartment or similarly restricted space or group of spaces. In the sewage treatment room, IVO watertight door 1.03, PSCO observed metal drums blocking the means of escape, leaving only the watertight doors as a means of escape. 74 SOLAS 01 II-2/28.1.1; 07120; 10c

Every crew member shall be given instructions which shall include but not necessarily be limited to operation and use of the ship's inflatable rafts, problems of hypothermia, first-aid treatment for hypothermia and other appropriate first-aid procedures, special instructions necessary for use of the ship's life-saving appliances in severe weather and severe sea conditions, operation and use of fire-extinguising appliances, and risks associated with and closed spaces and on-board procedures for safe entry. For lifeboats #3, #5, and #7 PSCO observed crew members erroneously identifying seasickness medication for other loose medication being stored in the same watertight container. 74 SOLAS 20 III/19.4.2; 11131; 10c

Flammable liquid storage locker located in the carpenter storeroom (cat 13) was approved to NFPA 30 and only provides protection for 10 minutes. These lockers should only be sued in a cateogry 11,12, or 14 space unless approved to be EN14470-1, Type 60, or Type 90.

In the main laundry, PSCOs observed a room-in room situation. Specifically, the upholstery shop had wire mesh windows that were completely obstructed by fabric curtains. There is only one means of escape provided from the space.

Within the perimeter of such stairway enclosures, only public toilets, lockers of noncombustible materials providing for the storage of safety equipment and open information counters are permitted. The stairway located immediately outside deck 5, glass store and deck 6 carpenter store was filled with various boxes, furniture, oil drums and pallets throughout the space obstructing the escape route.

Any required automatic sprinler, fire detection and fire alarm shall be capable of imediate operation at all times and no action needed by the crew. Combustible materials were stored above the installed fixed fire-fighting sprinklers in the forward carptneters store, deck 6.

Openings in bulkheads and decks in passenger ships. PSCO discovered multiple fire screen doors in the main vertical zone bulkheads, galley boundaries and stairways enclosures are not closing in the correct time and/or are not latching. 74SOLAS 20 II-2/9.4, 50ac

An International Load Line Certificate (1966) shall be issued to every ship which has been surveyed and marked in accordance with the present convention. PSCO discovered load line marking, indicates "CW". ILLC indicates hull marking should read "C1". LLC (Article 16), 50ac.

The machinery and associated piping systems shall be of a construction adequate for services for which intended and shall be so installed as to reduce to a minimum and danger to persons. Potable water pipe was leaking on steam lines. 74SOLAS 20 II-1/26, 50ac.
