A pilot ladder shall be certified by the manufacturer as complying with this regulation or with an international standard acceptable to the Organization 5 . Ladders shall be inspected in accordance with regulations I/6, 7 and 8. PSCO observed that the ship’s crew had replaced the failed pilot ladder with a pilot ladder manufactured in 2016 which class approval certificate expired in 2019. PSCO questions the condition of this pilot ladder.

The SMS should provide for specific measures aimed at promoting the reliability of such equipment or systems. These measures should include the regular testing of stand-by arrangements and equipment or technical systems that are not in continuous use. The vessel’s SMS does not provide specific instructions on the inspection and maintenance of the pilot ladder which failed. Instead it depends on the experience of the vessel’s crew. Safety Management audit is required to be carried out by the RO within three months.
