For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The Company should ensure that each ship is: appropriately manned in order to encompass all aspects of maintaining safe operations on board. The company shall conduct an internal audit focused on their implementation of this provision in light of the allision between the tug REVOLUTION and the San Bruno Shoal Light 5 in the early morning hours of 10NOV2025 in which exceeded work hour limitations and mariner fatigue were principal causal factors. The audit report and any developed corrective actions are to be submitted within 3 months.
