The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the space being ventilated. E/R ventilation closing devices outside of exhaust stack found to be broken off hinges and inoperable. 74SOLAS(04) II-2/5.2.2.1

Where air pipes to ballast and other tanks extend above the free board or superstructure decks, the exposed parts of the pipes shall be of substantial construction. MI found Bow thruster, port and stbd fwd ballast water vents to be corroded compromising water tight integrity. ICLL(96) Reg 20(1)

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. MI found substantial wastage on remote hand brake wire guide for port lifeboat winch.

All life-saving appliances shall be kept in working order and available for immediate use before the ship leaves port and at all times during the voyage. Port lifeboat was found to have taken on water during operational test, and is incapable of ensuring watertight integrity due to inoperable port sliding door. 74SOLAS III/4(c)

Starboard lifeboat was found to be incapable of ensuring watertight integrity due to inoperable overhead hatch, and has sustained damage to fender system. 74SOLAS III/4(c)

The construction of all doors in "A" class divisions shall provide resistance to fire as well as to the passage of smoke and flame. Found A-15 door on main deck to dry stores with 3"-4" gap on bottom sill. 74SOLAS II-2/37(d)

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards shall be ensured. Conduit below shaft had several cut electrical conductors with bare wire. 74SOLAS(97)II-1/40.3

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service they are intended… Safety relief valves for the start air receivers, and service air receiver had no readable tag indicating setting or servicing. 74SOLAS(97)II-1/26.1

The Company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. The revision of the PSTP instructions in use on board were a newer revision than what was approved by ABS. 74SOLAS (20)IX/2.1.3 ISM Code 7

The Company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. Crew’s annual completion logs of PSTPs shows a significant number left unfinished within the one-year time frame. 74SOLAS (20)IX/2.1.3 ISM Code 7

The Company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. PSTP instruction for the automatic bilge pump is not a feature supported by the installation and did not function. 74SOLAS (20)IX/2.1.3 ISM Code 7

Wiring shall be supported in such a manner as to avoid chafing or other injury. Conduit housing electrical wiring to both port and starboard davit systems found to be wasted and broken. 74SOLAS II-1/27(a)(vi)

Except as specified in paragraph 3 of this regulation, any ship of 10,000 gross tonnage and above shall be fitted with oil filtering equipment complying with paragraph 7 of this regulation. Received report from Class (ABS) of a failed Oily Water Separator. Vessel shall enact permanent repairs to the satisfaction of class. MARPOL I/14.2

Engine room fan space deck repaired with temporary concrete patch which is failing in multiple areas. Determine condition of deck plating condition and repair as original. The condition of the ship and its equipment shall be maintained to conform with the provision of the present regulations to ensure that the ship in all respects will remain fit to sea.

Galley electrical space broken door to electrical enclosure with temp repairs. Repair as original. Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards shall be ensured.

Incinerator space pooled oil in fuel tank containment, pooled oil beneath motor, oil soaked insulation around peripheral of space. Remove pooled oil, determine source and remove/replace soaked insulation. Means shall be provided to control leaks of flammable liquids.

Terminations and joints in all conductors shall be made so as to retain the original electrical, mechanical, flame-retarding and where necessary, fire resisting properties of the cable. MI found emergency control air for the main engine to have a exposed electrical wire and severely degraded sheathing. Vessel to enact permanent repairs to the satisfaction of class.

External high-pressure fuel delivery lines between high pressure fuel pumps and fuel injectors shall be protected with a jacketed piping system capable of containing fuel from a high pressure line failure. MI observed all double jacketed fuel lines on all three service generators to be disconnected. Vessel to enact permanent repairs to the satisfaction of class.

Ships shall be provided with a main steering gear and an auxiliary steering gear to the satisfaction of the Administration. The vessel reported a failure of both steering pumps during transit from Puerto Rico to Houston, TX.

A survey, either general or partial, according to the circumstances shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Vessel master reported the M/E #2 cylinder high pressure fuel line suffered a failure. Ships crew repaired the fuel line with parts on board. A damage survey shall be completed by the RO (ABS) and the results provided to the Coast Guard marine inspector for acceptance. Vessel must contact Sector Houston-Galveston OCMI prior to entry into port. 46 CFR 91.30-1

In general, compliance with the standards established by the American Bureau of Shipping (ABS), see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. Structural repairs to shell plating shall be completed in accordance with ABS approved repair plan reference DWG 2241-S11-1-2 dated 07APR2025. Vessel shall not operate past 01JUN2025 until repairs are completed to the satisfaction of ABS attending surveyor.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. According to ships SMS SMM-ENG-03 5.5 & 5.6, vessel maintenance shall be carried out in accordance with company and manufacturers' instructions; vessel shall maintain spare parts stock levels adequate for safe and effective operation. Vessel found to have a substantial overdue maintenance list(217 items), vessel unable to keep up with maintenance due to lack of spare parts and resources. An external SMS audit focused on maintenance and critical spare parts accessibility is required within 30 days.

However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. The vessel has significant wastage in the engine room sideshell plating, rendering the plate thickness below the ABS acceptable standards. Submit a detailed permanent repair proposal through ABS to the USCG, to bring the affected areas into compliance with the ABS minimum thickness rules. This shall include material lists, drawings, and engineering analysis. 46CFR92.01-10(a) 16ac 21MAR2025 - extended to 01APR2025 - MHM 31MAR2025 - extended to 15APR2025 - MHM 14APR2025 - extended to 01MAY2025; final repairs due 01JUN2025

The hull, machinery and equipment of a cargo ship shall be surveyed on completion and thereafter in such a manner and at such intervals as the Administration may consider necessary in order to ensure that their condition in all respects satisfactory. Temporary repairs to side shell were completed on 17FEB25. Vessel required to notify USCG upon next port of call in Houston, TX to schedule attendance to assess condition of repairs.

Evaporator pump inoperable requiring vessel to utilize general service fire pump for evaporator operations which has resulted in increased failures of fire pumps.

General arrangement plans shall be permenantly exhibited for the guidance of the ships officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions together with the particulars of fire detection and fire alarms systems... Plans and booklets shall be kept up to date; any alterations thereto shall be recorded as soon as practicible. Ships current revision of fire control plan has incorrectly labeled spaces (Gymnasium, Electrical workshop, etc) and doors that have been removed from the vessel. Additionally, fire boundaries on the main deck and "B" deck have been modified and not noted on the fire control plan. Vessel fire control plan does not match current arrangement. 74 SOLAS (20) II-2/ Reg 15.2.4.1

Except where otherwise stated, the normal equipment of every lifeboat shall consist of: (.12) a food ration totaling not less than 10,000kj for each person the lifeboat is permitted to accommodate. 05 food rations in the port lifeboat and 03 rations in the starboard boat were found unsealed. 74 SOLAS (97)III/ Reg 41.8.12

Fire extinguishing appliances shall be kept in good order and available for use at all times. Multiple fire extinguishers on the main deck and one on the bridge were observed with dry rotted and cracked hoses. 74 SOLAS (97) II-2/ Reg 21

The aggregate capacity of the electric ship's service generating sources required in § 111.10-3 must be sufficient for the ship's service loads. Vessel reported casualty to #1 SSDG resulting in loss of ability to utilize vessel's bow thruster without risk of excessive load fluctuations. Vessel shall provide an additional assist tug in maneuvering/docking situations in which the bow thruster would normally be utilized. 46 CFR 111.40-4

Except as specified in paragraph 3 of this regulation, any ship of 10,000 gross tonnage and above shall be fitted with oil filtering equipment complying with paragraph 7 of this regulation. Vessels OWS found to be leaking water from 3 different temporary repairs. Repair to satisfaction of class.

Vessels salt water cooling line for the ECR air conditioning system found to be ruptured.

Termination and joints in all conductors shall be made as to retain the original electrical, mechanical, flamretarding and, where necessary, fire-resisting properties of the cable. During the inspection of the electrical equipment room on the poop deck, MI found two electrical NEMA enclosures fed by an extension cord as well as several loose connections.

Electrical installations shall be such that the safety of crew and the ship from from electrical hazards will be ensured. Inspectors identified unauthorized modifications had been made to the reefer receptacles distribution box to accommodate the installation of welding machine power supplies.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. During lifeboat drill, lifeboats were unable to be recovered due to limit switch and fast/slow switch failures. Deficiency corrected and proper operation demonstrated. 74 SOLAS(20) III/20.1 10 C

Fire doors shall be self closing. Fire doors found not non-latching and/or not self closing. Repair and demonstrate proper operation. 74 SOLAS(97)II-2/30.4.1 16 A

The machinery shall be adequate for the service intended Excessive water ingress in the following engine room pump assemblies due to shaft seal failures. Repair or replace and prove proper operation. 1. Ballast pump. 2. Main fire pump. 3. Main salt water cooling pump 74 SOLAS(97) II-1/26.1 16 A

The means of escape, including stairways and exits, shall be marked by lighting. Inoperable emergency light in engine room emergency escape. Repair and demonstrate proper operation. 74 SOLAS(97)II-2/28.1.10 16 A

Exposed electrical components in following locations, install protective colors. 1. Emergency generator control assembly 2. Lower engine room heat detector overhead MSD. 3. Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum stresses to which it may be subjected to. Steam leak on auxiliary boiler safety relief valve flange. Repair as required. 74 SOLAS (97)II-1/33.1 16 A

Plans and booklets shall be kept up to update; any alterations thereto shall be recorded as soon as practicable. Required forward/focsle life-jackets not found on fire control/safety plan. Verify accuracy of fire control plan, revise fire control plan as needed, and submit to class for review and approval.

A ship of 300 gross tons or more constructed after June 30, 1974 must have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe Holed/wasted pollution containment for ventilation openings on aft deck below Incinerator room door. Draft limit control. The draft loss interlock switch must be tested to ensure proper operation. The draft limit control must cause burner shutdown and prevent startup when an inadequate air volume is supplied to the burner(s). Auxiliary boiler does not have required loss of combustion air alarm or means of testing. Provide alarm and demonstrate proper operation. 46 CFR 61.35-3 (a)(7) 16 A

Escape hatch from engine control room does not properly close or seal. Repair and demonstrated proper operation. 74 SOLAS(20) I/11(a) 16 A

On all ships, within machinery spaces, emergency escape breathing devices shall be situated ready for use at easily visible places, which can be reached quickly and easily at any time in the event of fire. EEBD missing in AFT corner of CO2 room. Replace as required by fire control plan. 74 SOLAS(20) II-2/13.4.3.1 16 A

All hatchways in position 1 and 2 shall be fitted with hatch covers of steel or other equivalent material. Such hatch covers shall be weather tight and fitted with gaskets and clamping devices. The arrangements shall ensure that the tightness can be maintained in any sea conditions. MI observed forward hatch main deck going into the Bos'n Locker with severely corroded/wasted knife edge not sealing. ICLL(88)16.1

MI observed wastage on bulkheads between scullery and crews mess and bulkhead between crews mess and officers mess. Wastage is located at the deck level up approximately 10" next to sideshell. 74 SOLAS (04)I/11(a)

Ventilator openings shall be provided with weather tight closing appliances of steel or other equivalent material. MI observed the natural ventilation mushroom head sealing surfaces for the Bosun Store, Paint Locker, Bow Thruster & Electrical Room were severely corroded and wasted. ICLL(88)19.4

Ships shall be provided with a main steering gear to the satisfaction of the Administration... The vessel reported a failure of one steering pump while in transit to Houston.

"A" class divisions shall comply with the following: they shall be so constructed as to be capable of preventing the passage of smoke and flame. Found 02 cable transits cut in "A" class boundary forming the exterior of the AC space on the A deck. Space is considered to have "A" class boundaries as per Class approved fire control plan.

All Bulkheads required to be "B" class divisions shall extend from deck to deck and to the shell or other boundaries. Found gym space(voids on FCP) bulkhead to be wasted and holed in numerous places to include 3 doors leading to the space from the accommodation hallway. Space is considered to have "B" class boundaries per class approved fire control plan.

The forward ballast had a small pin hole, which leaked into the bow thruster room.

Vessel's oil water separator was inoperable. Chief Engineer was not able to prove proper operation.

Remote lowing cables for vessel's lifeboats do not operate as designed.

Several class A fire doors were found to inoperable and /or prevented quick escape.

Vessel experienced a loss of propulsion during outbound transit. Vessel to provide Class report attesting to proper repairs and operation prior to departure.

Remote lowwering cables for vssel's lifeboats do not operate as designed or specified in the manufacturer's instructions. The remote release cable pays out at a faster rate than the falls, preventing the continual smooth lowering of the lifeboat and requiring the lifeboat operator to repeatedly remove slack from the line into the lifeboat. Make appropriate repairs.

Due to the lifeboat release mechanism deficiency, an internal ISM audit of the vessel's Safety Management System is required. The audit shall be equivalent in scope to the annual internal ISM audit required by the ISM code. Upon completion of the audit, provide a copy of the audit report to this office.

The current fire boundary doors in the engine room are not closing tightly and are deteriorating, specifically noted were the lower level escape and control room doors. Provide and demonstrate the proper operation of fire boundary doors throughout the engine room.

Electrical junction boxes on the stern winches are corroded. Provide watertight connections.

Both port and starboard gangways have areas of the kick plate that are fractured or wasted. Renew kick plates to good metal.

Portable stanchion mounting sockets between Bays 2 and 3, were found corroded and fractured. Renew socket mounts on the port side.
