Operational readiness: The manufacturer's instructions for on board maintenance must be on board the vessel, including checklists for use when carrying out the inspections, maintenance and repair instructions, and a schedule of periodic maintenance. The local OCMI may accept a shipboard planned maintenance program that includes the items listed in that paragraph. During periodic inspection, one immersion suit is dated 2007 and the remainder 2013 with no evidence of testing other than monthly visual inspections.

For each generator rated 50v or more, additional measuring instruments, if needed, must be provided. Distribution panel has two banks of 3 phase fault lights, one light bulb was burned out on each bank.

The following plans must be submitted: Arrangement of electrical equipment, electrical one-line diagram that includes wire types, sizes, overcurrent-device rating and setting, and type of electrical equipment enclosure. Switchboard wiring diagram and switchboard material and nameplate list. If the vessel is altered, or repaired, the plans, information, and calculations required by this part must be submitted—(a) To the OCMI in the zone where the vessel is to be constructed, altered, or repaired; or (b) By visitors to the Commanding Officer, Marine Safety Center. Vessel installed HPU with electric motor, connected by cabling and breakers to the vessel electrical distribution panel. Please provide these details.

A Class-1 door may be installed at any place if the vessel's pilothouse contains a visual indicator showing whether the door is open or closed. Visual indicators in the pilot house were not functioning. Until repaired, ensure hourly rounds are conducted and logged to verify door closed condition.

Vent outlets from all tanks which may emit flammable or combustible vapors, such as bilge slop tanks and contaminated drain tanks, must be fitted with a single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two screens of at least 20 by 20 mesh spaced not less than one-half inch (13mm) nor more than 11/2 inches (38mm) apart. The clear area through the mesh must not be less than the internal unobstructed area of the required pipe. The retaining rings on a 2 1/2 inch vent on the starboard day tank and a 3 inch vent on a port fuel storage tank were found broken, allowing air flow around the vent screens.

Fire pump self priming and capable of delivering a stream of water. Firemain found aligned to monitor unable to provide any stream.

Drills: Firefighting training and drills must be held before the vessel leaves port or within 24 hours of departing. Vessel mate has been onboard recently, and not participated in a fire drill.

Tests and inspections of fire fighting equipment. Galley smoke detector found inoperable.

Operational readiness. Each item of lifesaving equipment must be in good working order. Deck lifering found with detoriorated shell and grab line.

Test procedure and operations. Vessel technician no longer services John Deere and needed to perform safety and shutdowns on both service generators.

Vessel reported loss of both ship's service generators. Make necessary repairs and prove satisfactory operation of both generators prior to continued operation/service. Provide the local OCMI office a written report of the repairs/work completed.

Demonstrate proper operation of steering gear to include diagnosis and rectification of steering loss sufferred 07AUG2019.

Make tank adjacent to damage starboard midship available for inspection.

Prove proper operation of both SSDG safety shutdowns; low lube oil pressure, hi jacket water temp.

Prove proper operation of Bow thruster safety shutdowns; low lube oil pressure, hi jacket water temp.

Replace relief valve on #1 air receiver.

Replace wing nuts on E/R batteries with standard nuts.

Replace indicator lights for both steering motors.

Vessel installed a large sonar gondola below the bow without plan approval. Title 46 CFR 127.120 requires alteration plans to be submitted to the USCG for review and approval. You must restore the vessel hull to approved design or receive approval from Marine Safety Center for hull modification, prior to the vessel departing the partially protected waters of Puget Sound.

Provide 500' of synthetic auxiliary line for line throwing appliance.

Fire pump gauge on bridge is not required but installed and is inop. Replace or repair fire pump gauge on bridge.

LIFEBUOY LOCATED ON 02 DECK PORT SIDE IS MISSING 100FT OF LIFELINE. APPROPRIATELY ATTACH LIFELINE.

LIFEBUOY GRAB LINE FOR LIFEBUOY LOCATED ON 02 DECK STBD SIDE IS DETERIORATED. REPLACE GRAB LINE WITH APPROPRIATE LINE.

DRYER VENT DUCT HAS EXCESSIVE LINT BUILD UP. CLEAN DRYER VENT PRIOR TO NEXT USAGE BUT NLT 19JAN12.

Methanol system not operable and crew had never used or tested any of the equipment. Vsl not to use this system until it has been inspected/tested.

Fuel transfer procedures not posted within the engine room.

All watertight doors improperly adjusted.

Test kit not onboard as required.

Sporadic log entries and not IAW requirements as set forth in 46 CFR 131.620. Vessel's master and company representative were shown the requirements within the regulation. Deficiency should be cleared at the next annual exam.
