The inspection for certification is made by a marine inspector to determine whether the vessel is in a safe and seaworthy condition. At each inspection for certification and periodic inspection the marine inspector must conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as they feel necessary to check safe operation. Each vessel must have a permanently exhibited, for the guidance of the master and crew members, general arrangement plans showing, for each deck, the various fire-retardant bulkheads together with particulars of the—(a) Fire-detection systems;(b) Manual-alarm systems;(c) Fire-extinguishing systems;(d) Fire doors;(e) Means of ingress to the different compartments; and(f) (f) Ventilating-systems, including the— (1) Positions of he dampers;(2) Site of the emote means of stopping the fans; and(3) Identification of the fans serving each section. Fire safety plan was not signed or stamped for coast guard approval IAW new construction requirements outlined in 127.110.

Shapes shall be black and of the following sizes:(i) A ball shall have a diameter of not less than 0.6 meter. The black circle dayshape was deformed and not useful for signalling.

Each operation described in paragraphs (d), (e) and (f) of this section shall be fully recorded without delay in the Oil Record Book so that all the entries in the book appropriate to that operation are completed. Oil record book entries were not complete or signed by vessel master. Crew was coached on proper proceedures for oil record book by marine inspector, and deficiency was cleared on the spot.

46 CFR 128.130(a)(7): Vital systems are those systems that are vital to a vessel's survivability and safety. Propulsion systems and their necessary auxiliaries and control systems are vital systems. STBD Main engine is out of service, due to a failure while sailing into Lambert's Point, Norfolk VA.

Auxiliary steering must be controlled from a location that has communication with the bridge. Sound powered phone at the auxiliary steering controls in the engine room was inoperable.

Fuel oil samples are to be retained under the ship's control until the fuel oil is substantially consumed and for not less than 12 months; vessel was not maintaining fuel samples. Deficiency to remain open until next annual inspection to confirm maintaining of samples. MARPOL 73/78 (11) VI/18.8.1

Transfer procedures must contain a line diagram of the vessel's transfer piping; transfer diagram was missing.

Discharge of oil or oily mixtures into the sea from a ship is prohibited except after meeting specified conditions; vessel's OWS was inoperable due to "RTC malfunction" and could not meet requirements, rectify prior to using OWS.

Inspector not limited in making inspections as he or she deems necessary to be assured of the safety of the vessel; washer, dryer, and two (02) refrigerators were unsecured, making vessel unsafe and not seaworthy.

Standby vessels must have one blanket for each person on the most populated facility that the standby vessel is designated to assist; vessel was 50 blankets short of requirement.

Each vessel must minimize fire hazards, as far as reasonable and practicable; various lines on MDEs and generators had significant chaffing.

A rudder-angle indicator independent of the control of the main steering gear must be installed at the steering-control station in the pilothouse; stbd forward RAI was incapable of operating through the entire range of required movement.

Each ring life buoy must be marked with Type I or II retro-reflective material; retro-reflective tape on multiple ring buoys was worn down.

Each lifebuoy must be marked in block capital letters with the name of the OSV and the name of the port; multiple ring buoys have stenciling needing redone.

Each vessel must carry adequate and up-to-date U.S. Coast Pilot; vessel had outdated Coast Pilot 4 and 5.

Each cable installation must meet IEEE 45-2002. Metallic cable supports must be placed not more than 24 inches apart; cable in engine room in back starboard portion was supported with plastic brackets.

Vessel drydocked for emergency repairs and failed to notify Coast Guard. 46 CFR 126.140(b)

Before any vessel leaves protected waters, the master shall ensure that the vessel's exposed cargo hatches & other openings in the hull are closed; made properly watertight by the use of gaskets; & properly secured for sea. Watertight door & escape hatch leading from engine room to the main deck need to be adjusted for watertight integrity. The gasket was for the escape hatch needs replaced due to a gash in material. 46 CFR 131.520; 17c.

Non-metallic flexible hose must be in accordance with SAE J-1942. Hoses for the fuel oil return on both PORT & STBD main engines were cracked & distorted; hose on STBD steering gear unit was cracked; & hoses for inlet & outlet for bow thruster oil tank were cracked. 46 CFR 56.60-25(b); 50c.

Each portable fire extinguisher must be inspected, maintained, & hydrostatically tested as required by NFPA 10, Chapter 4. Hoses on the portable CO2 extinguisher protecting the paint locker and the portable CO2 extinguisher located next to the STBD main engine were cracked. 46 CFR 132.350; 50 c.

Each battery used in an item of survival equipment & not clearly marked with an expiration date must be replaced during the annual inspection & repair. Batteries for floating waterlights had an expiration date of 2016. 46 CFR 131.575(e); 50c.

Lifebuoys fitted with bouyant line must have line that is twice the height of where it is stowed above the waterline or 30 meters (100 feet), whichever is greater. Line on both lifebuoys located on the 01 deck was 60 feet. 46 CFR 133.70(a)(4).
