Vessel must be manned in accordance with the COI. No crew lifeboatman endorsement on MMC.

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date... Vessels annual inspection was due by 22DEC2024.

"Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: (1) Main engine low lubricating oil pressure; (2) Main engine high cooling water temperature; (3) Auxiliary generator engine low lubricating oil pressure; (4) Auxiliary generator engine high cooling water temperature" No high jacket water temp sensor on the aux gen. Fix for next dry dock.

"Alarms must: (1) Be visible and audible at each operating station. The alarm located at the operating station may be a summary alarm; if the alarm at the operating station is a summary alarm, the specific alarm condition must be indicated at the machinery or bilge location" alarm at the wheelhouse is a summary alarm, but there is not panel in engine room to indicate specific conditions. Fix prior to next credit dry dock.

46 CFR 140.910 & 140.915 No towing vessel record on board

A COI may be suspended and withdrawn or revoked by the cognizant Officer in Charge, Marine Inspection at any time for noncompliance with the requirements of this subchapter. Vessel overdue for DDX and ISE exams by 03 months. Vessel is restricted from movement except for transit to drydock until satisfying drydock and internal structural exams are completed. 46 CFR 136.215(c)

Functional requirements. A towing vessel's survival craft must meet the functional requirements of paragraphs (b)(1) through (5) of this section. Functional requirements describe the objectives of the regulation. Survival craft must: (1) Be readily accessible. Vessel did not have a survival craft present on board the ship. Furthermore the stowage of the IBA consisted of an ice cooler in a life raft bracket with a hydrostatic release. 46 CFR 141.305(b)

Station documents must be readily available to the licensed operator(s) on duty during the hours of service of the station and to authorized Commission employees upon request. Vessel did not have Bridge to Bridge Certificate on board. 47 CFR 80.403

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. Vessel did not have required entries listed in their TVR and could not account for crew orientation training in any documents on the ship. 46 CFR 140.915(a)

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. Vessel's pressure vessel was not stamped or marked with max allowable working pressure. 46 CFR 143.300(c)

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Vessel could not provide records or proof that required tests were completed. 46 CFR 143.245(b)

Charts or maps must be current editions or currently corrected editions. Operator was unable to provide proof that charts were updated. 33 CFR 164.72(b)(1)(ii)

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. Operator was unable to provide any towing gear documentation. 46 CFR 140.820(b)

Each towing vessel must be equipped with an illuminated card-type magnetic steering compass. Vessel's magnetic compass could not be illuminated. 33 CFR 164.72(a)(4)

Each towing vessel must be equipped with an industrial type first aid cabinet or kit. Contents of vessel's first aid cabinet were expired. 46 CFR 140.435

A remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Reach rod was not secured to the valve and could become inoperable. Permanently affix to the satisfaction of the attending Marine Inspector. 46 CFR 143.260(b)

Each liferaft must be arranged for float-free launching.IBA found stored in vessel house, not allowing for float-free operation. 46 CFR 199.130(c)(7)

Each immersion suit or anti-exposure suit must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the immersion suit. No lights were fitted on the vessel's immersion suits. 46 CFR 199.70(c)(4)(i)

Each immersion suit must be fitted with Type 1 retro reflective material that meets subpart 164.018 of this chapter. Installed retro reflective material was substantially wasted and required replacing. 46 CFR 160.171-9(n)

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck. Semi-portable was found tied to a rail using a fabric strap. 46 CFR 142.230(c)

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. No navigation assessments were recorded in the TVR or official log book. 46 CFR 140.635
