Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. The OCMI may also decrease the drydock examination intervals to monitor the vessel's structural condition. Vessel was involved in a collision with another vessel. Vessel is required to complete a drydock / internal structural exam to the satisfaction of the Marine Inspector prior to operating. 46 CFR 176.600(d)

A vessel must comply with the structural design requirements of the standards listed. Port bow nose craqcked and pieces missing.

Vessel >26 ft must have a placard of at lease 5 by 8 inches stating Discharge of Oil Prohibited. Placard was not present. 33CFR155.450

A durable placard must be posted to all radiotelephone installations with the emergency broadcast instructions and information specific to the individual vessel. Placard was not present. 46CFR184.506

Vessel must be fitted with a suitable magnetic compass. Except on a vessel limited to daytime operations the compass must be illuminated. Compass was not onboard. 46CFR184.402

IAW ALCOAST 100/20, Vessel is given a 90 day deferment to the DDX/ISE date. Previous max DDX/ISE date was 31MAR20.New max DDX/ISE date is 30JUN20.

Shutoff Valves, Installed as to close against the fuel flow, must be fitted in the fuel supply lines. Emergency Shut-off valves for STBD engine has rusted off.

A vessel on a limited coastwise route must carry 6 hand red flare distress signals and 6 hand orange smoke distress signals. Vessel only has 7 flares.

An adult lifejacket must be provided for each person carried on board a vessel. 06 lifejackets had tears.

Submit repair plan for hull damage on the STBD quarter of hull, above the water line.

Found cracks along the collision bulkhead in way of the centerline.

Replace expired EPIRB battery.

Make available for inspection cap for Main Diesel Engine jacket water cooling system, STBD side. Prior to the carrying of passengers.

Prove proper operation of the stbd fwd bilge light indicator prior to operating.

Provide proper amount of flares prior to operating on limited coastwise route.

Prove proper operation of the anchor light prior to operating at night or in limited visibility.

Replace stbd engine exhaust hose as original.

Replace ring life buoy and ensure proper retro-reflective tape.

Renew expired FCC radio Telephony cert.

Provide documentation for EAP training for crew and supervisor.

Provide repair proposal for hole/damage to port hull bow.

Make proper repairs to port bow IAW approved plans using listed lay-up schedule and fire retardant resin.

Contact USCG as fiberglass work progresses (sanding/cutting back to good fiberglass and lay-up phases).

Provide certificate of documentation onboard.

Provide retroreflective tape for light buoy.

Repair or replace damaged life buoy.

Show proper operation of both engine room port/stbd bilge indicator lights.

Replace electrical distribution panel cover.

Double clamp raw water hoses for P/S MDE.

Replace or crop to good hose port engine raw water hose from engine to exhaust (exhaust cooling). Hose IWO heat exchanger is chaffed and soft.

Provide 60 ft of buoyant line around body of ring buoy.

Prove proper operation of high water level alarms in both port and starboard engine spaces.

Make both sea water valves available for inspection.

Make permanent repairs to area of hull port side by way of loading area. Fire retardant resin shall be utilized in all repairs.

Make permanent repairs to area of starboard transom by way of access hatch. Fire retardant resin shall be utilized in all repairs.

Prove operation of both MDE and reduction gears after being placed back in vessel,

Provide proof of crew training as per NAVIC 05-01 Change 1 for all operators.

Prove enrollment in drug program under marine employer for all employees.

Provide 7.8 inch bell.

Provide magnetic compass.

Provide 5 orange and 3 red hand flares.
