Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated cabinet when not in use. If a dedicated storage cabinet is provided it must be secured to the vessel so that it does not move and must be either: A flammable liquid storage cabinet that satisfies UL 1275 or a flammable liquid storage cabinet that satisfies FM Approvals Standard 6050. A 40-B portable fire extinguisher must be located near the storage room or cabinet. Provide a designated cabinet and fire extinguisher for the flammable products.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. The smoke detector in the Captain's berthing and stbd berthing space were missing.

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. The battery for the EPIRB and the hydrostatic release for the EPIRB are expired. Renew the EPIRB, HRU, and NOAA registration.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in an SMI. Provide approved alcohol testing devices on board.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Each towing vessel operating on oceans, coastwise, or Great Lakes routes must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. Provide a means to take blood pressure readings, analgesic medication, and bandages.

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Renew all expired lights on the supplied lifejackets and immersion suits.

The owner, managing operator, or master of a towing vessel must ensure that the vessel's fire detection equipment complies with the applicable requirements of part 142 of this subchapter and is in good working order. The fire detection system gave a continuous "general trouble" alarm when energized. Provide a service report.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Renew the gaskets for the port and stbd ER doors and fwd storeroom hatch.

A crew work space must be of adequate construction to provide for the safe operation of the vessel and the protection and accommodation of the crew. Crop and renew the wasted handrail stanchions by next drydock.

Electrical systems and equipment must minimize fire and shock hazards. Remove the 2 permanently installed extension cords in the stbd berthing.

The owner, managing operator, or master of a towing vessel must ensure that the vessel's fire suppression equipment complies with the applicable requirements of part 142 of this subchapter and is in good working order. The gasket for the fixed fire pump was found to be leaking.

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessel is overdue for annual inspection.

Essential systems or equipment must be regularly tested and examined. Provide a means to test the high cooling water temperature alarms on the main engines and generators in the presence of the attending marine inspector.

All crewmembers must be provided with health and safety information and training. Refresher training must be repeated annually and may be conducted over time in modules covering specific topics. Refresher training may be less comprehensive, provided that the information presented is sufficient to provide employees with continued understanding of workplace hazards. The refresher training of persons subject to this subpart must include the information and training prescribed in this section. All training required in this section must be documented in owner or managing operator's records. Provide proof of annual refresher training on the health and safety plan.

Each towing vessel must be maintained and operated so as to minimize fire hazards. No flame screen was located at the starboard forward fuel tank vent.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. The smoke detector above the dining table was inoperable.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Crop and renew the knife edging around the starboard engine room door by next drydock.

A crew work space must be of adequate construction to provide for the safe operation of the vessel and the protection and accommodation of the crew. Crop the wastage located along the bulwark cap rail by next drydock.

Each EPIRB must be tested monthly. A report of the inspection, including a statement as to the condition of the equipment, must be recorded in the vessel's official logbook. No testing for the EPIRB has been conducted.

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessel is overdue for annual inspection.

Emergency lighting must be provided for all internal crew working and living area. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and each towing vessel must be equipped with at least two portable battery-powered lights, of which on must be located in the pilothouse and the other in the engine room. Emergency lighting in pilothouse is inoperable. Properly replace battery or replace light.

Each towing vessel operator must ensure each detector and its system is installed, tested and maintained in accordance with the manufacturer's design manual. Properly test heat detector in fidley space and proper operation of fire detection system.

Towing vessels must be equipped with an illuminated magnetic compass and be readable from each operating station. Vessel's current compass deviation card is from 2015, and vessel has had significant metal repair which could change compass deviation. Properly swing compass & update deviation card.

Sufficient lighting suitable for the marine environment must be provided within crew working and living areas. Repair light in crew berthing.

Vessel's MMSI number not properly entered in VHF Radio. Enter MMSI into VHF Radio as per manufacturer instructions. 47 CFR 80

The sidelights of a vessel shall be fitted with matt black inboard screens. Sidelights have glossy black paint.

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessel is overdue for annual inspection. A one time trip has been authorized for vessel to return to Fish Island in New Bedford, MA.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted. The valve must be operable from a safe place outside the space where the valve is installed. Remote shutoff INOP.

The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vessel. Temporary repairs were approved for the laz. Crop and renew all wasted structural support members and hull plating IWO laz to include all wasted supports embedded in the concrete to good metal. All repairs are due by 30APR2022 to the satisfaction of attending Marine Inspector.

A vessel that is exposed to salt water more than 6 months in any 12-month period since the last examination or initial certification must undergo a drydock and internal structural examination at least twice every 5 years, with not more than 36 months between examinations. Last drydock completed in 2017. Complete drydock prior to Initial Certification.

Each towing vessel must have a fire-detection system installed to detect engine room fires. The owner must ensure the following: The system is maintained in accordance with the manufacturer's design manual. Fire detection panel has system and power trouble lights on. Repair and conduct test to satisfaction of attending MI.

Each towing vessel must be fitted with a general alarm. General alarm is inoperable. Repair/replace prior to Initial Certification.

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. Vessel missing two appliances. Provide prior to Initial Certification.

There must be an installed or portable bilge pump for emergency dewatering. Installed bilge system inoperable. Repair and test to satisfaction of attending MI.

Pressure vessel must be equipped with one or more relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP as established by the manufacturer. Provide manufacturer's spec sheet showing MAWP for air receivers. Conduct test of relief valves to ensure proper capacity, to satisfaction of attending MI. Prior to Initial Certification.

Towing vessels must be equipped with a means to detect smoke. This may be accomplished by using individual battery-operated detectors meeting UL 217. Smoke detectors do not meet UL 217. Install proper detectors. Ensure detector are not more than 10 years old.

The owner of a towing vessel must ensure emergency procedures related to the tow have been developed. No procedures on board. Provided.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists and be visible and audible at each operating station. Aft operating station not fitted with alarm. Install prior to Initial Certification.

The operator of a vessel with a capacity of 250 or more barrels of oil shall provide transfer procedures that meet the requirements of this part. Transfer procedures not updated. Provide updated procedures.

The alcohol testing of each individual must be conducted within 2 hours of when the SMI. Alcohol swabs expired. Provided. 46 CFR 4.06-3

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Back side of the E/R SWBD and E/R power panel outboard of port MDE have back open allowing access to personnel. Install protective panels.

The following electrical items were noted: 1) Port side fiddley light not permanently wired. Permanently wire light.2) Aft head fan closet, exposed fan wiring due to missing junction box cover. Replace junction box cover. 3) E/R bilge alarm light wiring to receptacle plug. Permanently wire equipment running off receptacle. 4) E/R stbd side aft, steering limit junction box, No bulbs or covers. Install bulbs and light covers. 5) Light over stbd engine ran to receptacle with exposed wiring. permanently wire. 6) Light switch receptacle IWO grey water tank. Permanently wire equipment running off receptacle. 7) Stbd side E/R outboard aft, junction box open with no means of closure. Install means of closure. 8)E/R receptacle IWO sink not GFCI or suitable protected. Must be suitably protected from wet environments. 9) Port and Stbd Forward E/R heater fans, electrical connection made with wire nuts not in an enclosure. Install enclosure for heater fan wiring. 10) Dead end wire, E/R inboard of the stbd forward heater fan. Remove dead end wire or place in an enclosure. 11) E/R wire above switchboard spliced with wire nuts. Place connection in an enclosure. 12) E/R upper battery terminals secured with wing nuts. Electrical connections must be suitably installed to prevent them from coming loose through vibration. 13) E/R 12/24 Volt panel has exposed cut wires protruding from panel. Remove cut wiring. 14) E/R junction box IWO port generator expansion tank has exposed cut wires protruding. Remove cut wiring. 15) E/R J/W heater power panel not labeled and missing multiple breakers. Install breakers or blanks. 16) E/R port forward battery charger missing knock out in side of charger. Install plug. 17) Dead end wire in E/R port forward. Remove dead end wire or place in an enclosure. 18) E/R port forward hot water heater junction box missing stuffing tubes and exposed wiring. Install stuffing tubes. 19) E/R port A/C Compressor wiring has exposed conductors due to cable nsulation failure. Replace. 20) E/R tow disconnect switch missing knock out on side. Install plug. 21) E/R center stanchion below ladder has dead end wire. Remove dead end wire or place in an enclosure. 22) E/R center stanchion below ladder junction box has female electrical plug coming out of junction box. Remove the female electrical plug. 23) Above boiler in E/R, junction box has male electrical plug coming out of junction box. Remove the male electrical plug. 24) E/R Boiler junction box cover loose exposing wiring. Repair junction box. 25) Forepeak heater wiring exposed. Enclose wiring in the bottom of the heater. 26) Forepeak stbd light, no light in light fixture, being used as a junction box and splice made with wire nuts/loose wiring. Wiring not properly supported. Repair light and wiring. Deficiency #15 is continuation.

Continuation from Deficiency #14. 27) Forepeak dead end wires IWO detector, remove or place in an enclosure. 28) Port and stbd forward main deck berthing, All lights have no covers. Install protective covers on lighting. 29) Port forward main deck berthing, Light at entrance above door has exposed wiring. Repair. 30) Mast wiring not properly supported. Properly support mast wiring. 31) 02 deck stbd side wiring entering the pilot house is not weather tight. Make wiring weather tight. 32) Wiring in fiddley ran through ventilation system. Remove wiring from ventilation system and run wires through bulkhead stuffing tube.

An exposed hazard such as gears and rotating machinery, must be protected by a cover. Port generator crankshaft is exposed rotating machinery. Install guard for rotating machinery.

Machinery systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Port fuel return valve missing handle. Install handle.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Aft fan closet, fan missing all upper hardware. Install hardware or prove that the fan is mounted correctly.

Windows and other openings at the operating station must be of sufficient size and properly located to provide a clear field of vision for safe operation in any condition. Pilot house port aft window made with plexi-glass. Prove strength of window or replace. Stbd forward window cracked. Replace. Aft stbd window cracked. Replace. Prior to Initial Certification.

The OCMI may require review of a vessel's watertight and weathertight integrity. The following were noted: 1) E/R port forward bulkhead has a 2 to 3” inset from previous damage as well as some side shell clad welds. Inspect and repair if necessary at next drydock. 2) E/R room port and stbd, aft above compressed air receivers, four deck inserts that appear to be leaking. De-scale and hose test and repair if necessary at next drydock. 3) Port fiddley door, broken window. Replace at next drydock. Prior to Initial Certification.

Master had expired license with no MISB number 08-20 change 3 on hand additionally license mate could not produce credential to boarding team.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
