Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500 l and above shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. In the vessel’s fire control station, PSCO observed the air bottle used to close the engine room’s fuel oil tank was empty and would not have operated as designed if used during an emergency.

0936- the angular position of the rudder shall be recognizable in the steering gear compartment. PSCO observed the rudder angle indicator bolt sheared on the port side of the rudder angle indicating device causing it to list when the rudder is swung from side to side. When the rudder was swung from the bridge the rudder angle indicator on the bridge shows a 3 degree difference than the local steering gear indicator. When the rudder was swung to port, the PSCO observed loud noises and the ram does not move smoothly. 74 SOLAS (04)II-I/29.11.2

PSCO observed the oil water separators oil content meter is giving error code (E3)when tested. The crew was not able to demonstrate proper operation of the equipment. MARPOL I/6.4.1
