Parties responsible for fir extinguishing shall be organized. These parties shall have the capacity to complete their duties at all times while the ship is in service (15.2.1.3). Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire fighting systems and appliances that they may be called upon to use (15.2.2.1).
PSCO observed crewman improperly donning SCBA mask, allowing air to escape due to improper seal of mask. Crewman then approached fire engaged in fire fighting efforts with no air pressure left in bottle. Additionally, crew attempted to fight simulated oil fire on cargo deck with fire hose connected to fire hydrant instead of foam applicator to foam line.
