The company should ensure that each ship is manned with qualified, certificated and medically-fit seafarers in accordance with national and international requirements. Upon inspection, the master was found to be sailing with a recently expired medical cerfificate, after arriving from the port of New York. Company is to conduct an investigation to determine the root cause, develop and implement any corrective action into the SMS to prevent future occurances within the next 90 days. The effectiveness of the implemented corrective action to be verified at the next schedule external audit.

In addition to complying with the specific provisions for fire integrity of bulkheads and decks of cargo ships, the miniumum fire integrity of bulkheads and decks shall be as prescribed in tables 9.5 and 9.6. The structural fire protection on the forward bulkhead of the CO2 room was found in need of repair to maintain its integrity. All repairs should be completed at the upcoming dry dock availability in Oman.

With out evidence of such by 22Nov the ship is required to fully man the engine room to the satisfaction of the [name withheld] shall be manned with an addition of 3 qmeds to maintain a 3 watch section. The Administration shall take appropriate steps to ensure uniformity in the implementation and application of the provisions of this part in respect of electrical installations.* *Refer to the recommendations published by the International Electrotechnical Commission and, in particular, Publication IEC 60092 - Electrical Installations in Ships. When cables are fixed by means of non-metallic clips or straps, and are not laid on top of horizontal cable trays or cable supports, suitable metal clips or saddles shall be added at regular distances not grater than 1 meter in order to prevent the release of cables during a fire. Throughout the engineroom on all levels, cables were installed without metal clips on horizontal and vertical runs. 1974 SOLAS (04) II-1 reg 40.2 IEC 60092-352 sec 3.19

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements inspections are held at appropriate intervals, and records of these activities are maintained. Vessel has no record that approved Periodic Safety Test Procedures were ever completed after initial approval. PSTP's should be completed annually and records of completion maintained. 74 SOLAS (04) IX/ 3.1 ISM Code 10.1, 10.2.1, 10.2.4

he company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. These measures should include the regular testing of stand-by arrangements and equipment or technical systems that are not in continuous use. Marine Safety Center approved Periodic Safety Test procedures are not be followed. Reliability of the automation is not being tested per approved PSTP's. 74 SOLAS (04) IX/ 3.1 ISM Code 10.3

The inspections mentioned in 10.2 as well as the measures referred to in 10.3 should be integrated into the ship's operational maintenance routine. Approved Periodic Safety Test Procedures are not integrated into the ship's maintenance routine. Recommend an internal audit of SMS, PSTP completion tracking and recording. 74 SOLAS (04) IX/ 3.1 ISM Code 10.4

Components of a diesel engine fuel system shall be designed considering the maximum peak pressure which will be experienced in service, including any high pressure pulses which are generated and transmitted back into the fuel supply and spill lines by the action of fuel injection pumps. MDE fuel return line valve located inside of the F/O purifier space is leaking. SOLAS (Cons. 09) II-2/4.2.2.5.4
