Automation Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner.
Ship could not produce objective evidence of dates when individual periodic safety test procedures were completed IAW approved procedures 3.0 final copy 06-04-15. Some of the items were accomplished in the PMS system but not to the detail of the approved PSTP. During the spot check we also could not locate some items that are required in the approved PSTP. With out evidence of such by 22Nov the ship is required to fully man the engine room to the satisfaction of the [name withheld] shall be manned with an addition of 3 qmeds to maintain a 3 watch section. The oilers may be omitted with the addition of the 3 QMED.

When cables are fixed by means of non-metallic clips or straps, and are not laid on top of horizontal cable trays or cable supports, suitable metal clips or saddles shall be added at regular distances not grater than 1 meter in order to prevent the release of cables during a fire.
Throughout the engineroom on all levels, cables were installed without metal clips on horizontal and vertical runs.
