Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel's anniversary date is July 6th. Annual exam window expired October 6th. 46 CFR 176.500(b)(1)

The master of the vessel shall ensure that the EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. Vessel’s EPIRB battery is expired based upon manufacturer’s markings. Vessel limited to Lakes, Bays, and Sounds, warm water only. 46 CFR 185.728(b)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Hydrostatic release for EPIRB was not marked with expiration date. Vessel limited to Lakes, Bays, and Sounds, warm water only. 46 CFR 185.740(b)

Stations in the maritime service must be licensed by the FCC. COI does not match FCC certificates. 47 CFR 80.13

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) current tables. Vessel did not have all required up-to-date publications as appropriate for the intended voyage. 46 CFR 184.420

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including checking of each cylinder containing compressed gas to ensure it has been tested and marked in accordance with 46CFR147.60. Vessel could not prove that the pre-engineered system on board had been serviced in last year. Control action: within 7 days, carry NMT 6 pax. 46CFR176.810(a)(5)

Each life jacket must be marked with type I retroreflective material approved in 46CFR164.018 or other standard specified by the Commandant. Observed 05 adult life jackets that needed retroreflective material replaced. 46CFR185.604(h)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Observed excessive amount of oil in bilge W/I E/R compartment. Oil leaking from oil filter. 46CFR176.830(b)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. MI observed expired first aid kit. 46CFR710

Except as noted in paragraph C of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. MI observed an expired FCC station license. 47CFR80.13

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position, or perform the duties and functions of a position, required by the vessels COI. Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on uninspected vessels who: are required by law or regulation to hold a license issued by the Coast Guard in order to perform their duties on the vessel, perform duties and functions directly related to the safe operation of the vessel. Vessel is authorized to operate as a UPV - 6 pax. Captain is not covered under the employer's EAP consortium. 46CFR16.230(a)(b)

The person in Command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effet for that vessel. Vesse's COD was expired. 46CFR67.313

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abondoning ship or man overboard and fire fighting. Vessel has no documentation of recent drills or training. 46CFR185.520

Every vessel must have a detailed inspection of the radio installation by a FCC licensed technician once every five years. Radio safety certificate was expired. 47CFR903

Vessel is required to have an audible alarm that sounds upon dischard. Did not hear alarm when testing fixed firefighting system. Repair audible alarm.

Vessel's ring buoy throw-grab line was not serviceable. Outfit vessel ring buoy to satisfaction of the Coast Guard.

Vessel had two, 2-gallon fire buckets. Outfit vessel with three 2.5-gallon buckets.

Vessel is required to have life jacket donning placard, but did not have one. Demonstrate donning of lifejackets to passengers until replaced.

First aid kit missing several items, incomplete

Vessel did not complete hull inspecation at regulated interval. Must complete sastifactory hull exam prior to taking passenegers.

Vessel has not conducted annual inspection within 3 months of anniversary date. Vessel is restricted from carrying passengers until annual inspection is complete.

Vessel's emergency instructions not tailored to specific ship's operations. Provide proof of emergency procedures to satisfaction of marine inspector.

Operator did not have a copy of his Radio Operator's Permit. Operator is restricted to operation 1,000 feet from shore.

High Flyer One forepeak hatch broke off during operation. After examination High Flyer One, Two and Three hatches are required to be replaced prior to operation IAW 46 CFR 179.360 (b) (1) and 46 CFR 179.360 (b) (2).

Repair/replace RPM Gauge and provide proof of proper operation.

Provide lifering floatl light, line & clip. Prior to carrying passengers at night.

Provide EPIRB w/NOAA registration. Vessel required Cat I 406 EPIRB for route. Prior to carrying passengers on Limited C/W W/I 3NM warm water.

Provide emergency tiller.

Provide portable bilge pump.

Provide FCC SSL, Radio Safety, & operators permit. Prior to carrying passengers beyond 1000' from shore.

Conduct fuel tank post installation test. (IAW 182.440(c)(2)Tank tested to 5 psi prior to installation. Tank was empty for vessel shipment at time of inspection.

Provide charts & navigation publications.

Conduct underway portion of inspection. To include machinery inspection (including fixed system shut down) & drills.

Conduct new to zone inspection, prior to carrying passengers in new zone.
