The owner, master, or person in charge must ensure that portable extinguishers are inspected and maintained IAW NFPA 10. The CO2 fire extinguisher in way of the paint locker is missing proper marking of the required annual conductivity test. Conduct conductivity test of the hose IAW NFPA 10 or provide written statement from maintenace company attesting to completion.

Electric installations on a vessel must ensure protection of passengers, crew, and other persons from electrical hazards. The temperature guage on the #4 port cargo tank is broken w/exposed wires. Repair or replace the gauge.

Operator of a vessel with a capacity of 250 or more barrels of oil shall provide transfer procedures that meet the requirements of this part and part 156 of this chapter for transferring. Barge transfer procedures were not up to date. Procedures need to be updated to current operations prior to cargo carriage. CITE 33 CFR 155.720

Shipboard oil pollution emergency plan (SOPEP) must be available on board. SOPEP Plan was not on board. Place on board and send copy for review. CITE 33 CFR 151.26(a)

The name and 24-hour contact information for the following key individuals and organizations must be included. Vessel Response Plan had incorrect numbers for NRC and Sector Detroit. Update numbers. CITE 33 CFR 155.1040(e)

The equipment and supplies must include: Sorbents; Non-sparking hand scoops, shovels, and buckets; Containers suitable for holding recovered waste; Emulsifiers for deck cleaning; Protective clothing; A minimum of one non-sparking portable pump with hoses; and Scupper plugs. Spill kit did not have list of items in drum. Received copy of list, which did not include some items required by the regulations. Obtain and install missing items. CITE 33 CFR 155.210(b)

Each cargo tank of a tank barge must have a high-level indicating device. The high-level indicating device must - Indicate visually the level of liquid in the cargo tank when the liquid level is within a range of 1 meter of the top of the tank. Cargo Tank #2 Port meter stick missing grommet to hold stick in place. Install new. CITE 33 CFR 155.480(b)(2)(iii) 46 CFR 39.2003(b)

Batteries must be constructed to take into account the environmental conditions of a marine installation. Found wing nuts on batteries terminals in cargo pump room. Remove wing nuts and replace with permanent type connectors. CITE 46 CFR 31.35-1 46 CFR 111.15-2(d)

Thermal fluid heater door needs replaced due to wasted steel and refractory before the vessel can conduct cargo operations that require the thermal fluid heated being used.

All oil tankers shall be fitted with a stability instrument, capable of verifying compliance with intact & stability requirements approved by the administration having regard to the performance standards recommended by the organization. Barge is currently in the process of being deferred from this regulation by the flag state.

Cargo pump relief valves shall be tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open. Relief valve. Test dates are past due.

The cargo discharge piping of all tank vessels shall be tested at least once each year for tightness, at the maximum working pressure. Test dates are past due. Each transfer hose for vessels must be tested annually or as part of the biennial and mid-period inspections. Test dates are past due.

The owner, master, or person-in-charge of a tank vessel must ensure that portable and semi-portable extinguishers are inspected and maintained in accordance with NFPA 10. Portable fire extinguishers have not been serviced in over a year.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. No E-lights were operational in accommodation areas.

All electrical equipment and circuits must be clearly marked and identified. Cooking range breaker shut off not labeled.

Each hydrostatic release unit, other than a disposable unit, must be serviced: (1) Within 12 months of its manufacture and within 12 months of each subsequent servicing, except when servicing is delayed until the next scheduled inspection of the vessel, provided that the delay does not exceed 5 months. Hydrostatic release was out of service.

Each rescue boat and ring life buoy must be marked with Type II retro reflective material approved in accordance with 46 CFR 164.018. All life rings needed new reflective tape.

All safety and relief valves for use on pressure vessels or piping systems shall be designed to meet the protection and service requirements for which they are intended and shall be set to relieve at a pressure which does not exceed the “maximum allowable working pressure” of the pressure vessel or piping system. Relief valve on air reciever is past due on its servicing.

Each tank vessel must undergo internal structural examinations every 2.5 years for vessels in salt water. Currently your vessel is past due on it's ISE. No operations are permitted until the ISE has been conducted.

The inspection for certification shall include an inspection of the structure, boilers, and other pressure vessels, machinery and equipment. The inspection shall be such as to insure that the vessel, as regards the structure, boilers, and other pressure vessels and their appurtenances, piping, main and auxiliary machinery, electrical installations, life-saving appliances, fire-detecting and extinguishing equipment, pilot boarding equipment, and other equipment is in satisfactory condition and fit for the service for which it is intended, and that it complies with the applicable regulations for such vessels, and determine that the vessel is in possession of a valid certificate issued by the Federal Communications Commission, if required. The lights, means of making sound signals, and distress signals carried by the vessel shall also be subject to the above-mentioned inspection for the purpose of ensuring that they comply with the requirements of the applicable statutes and regulations. Vessel's aux machinery operations, and emergency shutdowns were not tested due to lack of 3rd party operator. Also, no electrical components were available for test as well as nav lights, or radios.

Pressure test records for Hoses, Valves, and Piping have not been maintained.

Each vessel must carry lifebuoys approved under approval series 160.150. Lifebuoys required new plugs and new reflective tape. 46 CFR 199.190

Radio lifesaving appliance installations and arrangements must meet the requirements of 47 CFR part 80. EPIRB hydrostatic release date was expired. 46CFR199.60(a)

If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. Air reciever's safety relief valve was past its test date. 46CFR61.10.5(i)(2)

Internal Structural Exam has not been completed with in the required timeframe. Vessel currently bieng put in laid-up status and will not have ISE conducted until it is put back in service.

If the voltage of a distribution system is less than 1,000 volts, a tank vessel must not have a grounded distribution system. Ground detection system onboard indicated a ground in deck lighting. Vessel may not conduct Cargo Operations until deficiency is rectified.

#3 port side ballast tank side shell repair

A vessel 79 feet in length and more, and 150 gross tons or over, the keel of which is laid or which has reached a similar stage of construction after April 14, 1973, must meet the requirements of part 45 (GREAT LAKES LOAD LINES). Vessel has a breach in the watertight envelope in way of 1P void/wing tank. Provide either of the following to facilitate clearing of this CG-835 item: 1.Provide for review and approval, documentation attesting to the positive stability and safe navigation of vessel while transiting to Erie, PA for repairs taking into account unknown location/extent of hull breach and possibility of propagation into adjacent spaces. Documentation shall be verified by Class prior to review by Coast Guard Marine Inspector. Documentation shall take into account unknown extent of breach in hull. Calculations should take into affect all aspects of the dynamic conditions the vessel may encounter while transiting to shipyard in Erie, PA. 2. Provide access to hull breach where breach can be fully examined for temporary repair and extensive assessment.

If, during an internal structural examination damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. Vessel has a breach in the watertight envelope in way of 1P void/wing tank. Once item AM1 above has been cleared, vessel is allowed to transit to Erie, PA under favorable weather conditions to carry out repairs to the satisfaction of Class and attending Coast Guard Marine Inspector. Repair proposals shall be reviewed and approved by Class and Coast Guard prior to work being done. Attending Coast Guard Marine Inspector shall witness extent of damage, fit-up and final NDT procedure.

Starboard side cargo pump gasket has failed, allowing cargo to spill on deck. Cargo ops suspended, until repaired.

Clean and repaint loadline marking.

Prove safety shutdowns on house generator prime mover prior to operation.

Provide drug and alcohol program compliance documentation.

Provide vessel with line throwing appliance and four charges.

Provide cargo pump engine with metal bowls on Racor filters.

Prove proper operation of ground detection system.

The vessel was not in compliance with ABS issued IAPP certificate. The vessel did not have any bunker delivery notes on board, bunker samples on board, or have an approved drip sampler.

The barge must meet all the requirements of ANNEX VI. Therefore the Route has been restricted the vessel route to "Not on an International Voyage" until the vessel can demonstrate compliance with MARPOL ANNEX VI.

COD does not have Regisrty endorsement. Vessels route is restricted to "Not on international voyages" until proof of proper endorsement.

Area in #2 stbd void, 2nd bay forward of aft bulkhead, side shell punctured approx. 4' x 3', transverse frame fractured, and inset caused weld to pop at the deck/side shell seam approx. 1' in length. Vessel to submit repair proposal before start of any repairs.

Prior to conducting cargo operations or departure from the port of Hampton Roads, must provide a copy of a dive report (damage Survey) assessing damage caused from pier allision.

Exhaust leak on bellows on #2 Generator. Gasket deteriorated and small hole in pipe.

Location of life raft does not allow easy side to side transfer on the same deck as required.

Replace hydrostatic release for EPIRB.

Thermal fluid heater label for high level temp greater than 400 deg F. Label was marked as 550 deg F. Can not operate greater than 400 deg F or it will be considered boiler and regs for boiler are significantly different.

Prior to lading cargo, vessel shall make repairs to the forward rake to the satisfaction of the attending CG Marine Inspector.

Tug and barge are authorized a one time transit from Philadelphia to Lyons shipyard for repairs (Norfolk, VA). Vessel is to make permanent repairs to the forward rake prior to loading cargo to the satisfaction of the cognizant OCMI.
