At each inspection for certification and periodic inspection, the inspectors shall conduct the following tests and inspections of hull equipment: Every acceptable cargo gear certificate and/or register shall be properly executed by a person authorized to do so and shall:(1) Certify as to the tests and examinations conducted;(2) Show the dates on which the tests and examinations were conducted; and(3) Indicate that the cargo gear described in the certificate or register complies with the standards of the organization or association authorized to issue the certificate or register. Vessel reported discharge of oil into the water from cargo boom at CSX docks. Vessel will need to have ABS survey cargo equipment prior to cargo operations; to the Coast Guard's satisfaction.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel has report a bulkhead failure between the #4 & #5 cargo holds.

On or before 31MAR25 the ballast stripping system must have plans submitted and then approved by MSC for equivalency of the as built design using unapproved PVC piping materials. Or you may submit new drawings to MSC to replace the piping to ensure the system fully meets all applicable portions of 46 CFR Sub-chapter F and then complete the installation. Any and all modifications must be overseen and inspected by a qualified CG Marine Inspector. Either option must be fully completed NLT 31MAR25 and no extensions will be granted.

In the cargo unloading tunnel there are numerous electrical issues. There are cut cables hanging in cable trays at numerous locations in both the port and stbd tunnel. A survey by crew or third party is to be completed and ALL dead ended cables are to be removed or properly terminated in approved junction boxes.

There are several cable penetrations passing through class A-0 boundaries where the multi cable transit systems approved for structural fire protection are missing. Survey ALL cable penetrations and reinstall approved structural fire protection cable penetrations.

Unloading tunnel ventilation system installation is incomplete. The new partial installation does not have required plans approved by the Marine Safety Center. Also, the system is not yet fully installed. Plans are to be approved by MSC and the system installation complete. Plan approvals must cover, haz area drawing, one line electrical drawing, structural modifications to the fwd and aft sections of the vessel, and ventilation system arrangement details. A CG Marine Inspector must review approved drawings, verify materials used, ensure proper hazard class for equipment installed, and witness satisfactory testing of the system. The system may not be energized or used for any reason until cleared by an attending CG Marine Inspector.

During engine testing on the unloading boom #2 diesel driven hydraulic pump the pump suffered a catastrophic failure. The pump is to be replaced or repaired and demonstrated to function properly. To the satisfaction of a CG Inspector.

The navigation lights on the bow both red and green have broken housings and must be replaced.

Temp Cement Box patch installed in the #2 Stbd ballast tank, at bottom shell in way of Frame 23. Cement Box patch shall be cropped out with permanent steel repairs made at vessels next dry-docking as required by 46 CFR 91.45-5

Both aft engine rooms fixed CO2 system time delays failed to activate within the mfg. specs. Both activated at 45 seconds, vice 26 seconds. Replacement Time Delays unavailable. Both Time Delays may remain but shall be renewed or the fixed CO2 system serving each engine room must be redesigned, approved, installed and re-tested as required by 46 CFR 95.05-10(e)(3)(ii) on or before 31MAR2024.

Suitable hand covers, guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc. The cover to the aft winch on the barge is damaged and internal components are exposed. Effect adequate repair to the satisfaction of the attending marine inspector prior to use and prior to the end of 2023 winter layup.

Inspect and verify proper operation of the #2 hydraulic pump and its engine/prime mover prior to placing them online to move cargo.

Test all electrical cables that were exposed to the heat from the fire for insulation resistance using a megohmeter and provide our office with a report of the readings. Insulation tests should test the insulation of each conductor in a cable, testing insulation resistance between conducter in the cable and insulation resistance between each conductor and the ships hull. Replace or repair any electrical cables which the insulation resistance is found to be less than what is specified in section 34.2.1 of IEEE 45-2002. Any cables replaced should be replaced-in-kind with what was approved/installed prior to the fire. If replacement cables will not be replaced-in-kind, datasheets for the new cables showing that the cables are constructed to meet the requirements of 46 CFR 111.60-1 must be submitted to the cognizant OCMI for review and approval.

Door leading into the ladderwell to the cargo tunnel had missing gasket material preventing it from making a tight seal. Members of the crew had replacement gasket material on hand and issue was corrected before the inspection was completed.

The ballast water system did not have an installed guard over the exposed/rotatating coupling.

Noted several leaks in the port fire main on the main deck, in the vicinity of #7 and #8 cargo holds. Also noticed a section of flexible hose instead on fixed piping near cargo hold # 3. CG-835 issued to "Make permanent repairs port fire main as shown on the approved plan".

Bulkhead between forepeak and #1 Port ballast tank had a fractured weld aprox. 15" wide at the bottom hull plating due to hull bottom deformation. Shipboard welder ground and welded fractured weld from the forepeak side, unable to weld the #1 side due to deformation. Owner to submit permanent repair proposal and make permanent repairs under the discretion of the cognizant OCMI.

Provide copy of ABS Class Report for temporary repair to damage in bottom of #6 starboard ballast tank by May 15, 2010, to Coast Guard MSU Cleveland.
