Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Starboard MDE No. 11 fuel injector had a fault that the crew was trying to fix during the annual inspection. A certified technician's report must be submitted to the MSU Toledo OCMI to clear the starboard MDE fault. The report shall attest that no active faults are present in the ECM and the engine is fully operational. Vessel is authorized a one time only transit to Torco Docks until repairs are made. 46 CFR 143.205(a)

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. Both main propeller shafts need their guards extended to cover hazardous rotating areas. 46 CFR 144.820

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Vessel self reported a small fracture in the STBD #2 BWT bottom shell plating. Vessel will have a temporary repair by installing a concrete box over the affected area. This temporary repair is subject to an annual inspection by a United States Coast Guard (USCG) Marine Inspector. A permanent repair, to the satisfaction of the local Officer in Charge, Marine Inspection (OCMI), must be completed at the vessel's next dry-docking. 46 CFR 140.605(b)

While transiting the St. Mary's river, vessel experienced a failure of the STBD MDE outboard turbo. Vessel is to transit to Toledo, OH to effect repairs to the satisfaction of attending MI prior to departure. A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment; while transiting from Duluth Harbor to Silver Bay, MN, vessel suffered reduction in power of STBD MDE. Vessel authorized to transit from Silver Bay, MN to Marquette, [name withheld] of transit is only permitted to begin granted the weather conditions remain favorable; winds are not to exceed 10kts. Repairs are to be conducted in Marquette to the satisfaction of attending MI from Sector NGL prior to departure from Marquette. 46 CFR 140.205

Vessel made notification to MSU Toledo of crew shortage due to unavailable replacement asst. engineer. Master shall ensure work/rest requirements are met, and all maintenance performed in accordance with SMS. Master is authorized to continue operation while short one asst. engineer for 30 days and required to find a replacement with the same or greater credential. The Chief Engineer shall conduct a risk assessment prior to each cargo operation, underway transit, and special evolution. If no replacement is found, the master/operator must provide a new request to the cognizant OCMI with: 1) work/rest logs for previous period of crew shortage; 2) documentation that a substantial effort was made to find a replacement; and 3) statement from the vessel Master and Chief Engineer attesting that the vessel is sufficiently manned. 46 CFR 15.515(a)

Each liferaft must be arranged for float-free launching. Life raft was missing hydrostatic release.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists such as high bilge levels. Bilge alarm was inoperable.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists such as low hydraulic steering fluid. Low Hydraulic steering fluid alarm was inoperable.

Each life jacket must have a life jacket with light securely attached to the front shoulder area. Found 07 light jacket lights broken. Renew with appropriate lights.

Alarms must be visible and audible at each operating station. Vessel's primary operating stating does not have means to indicate high bilge levels.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light. STBD aft deck ring buoy was missing light.

Vessel was not ready for annual COI inspection. Make vessel ready and available for inspection prior to departure.

Each towing vessel operating north of lat 32* N. or south of lat 32* S. must carry at least one appropriate-size immersion suit for each person onboard. Found 05 immersion suits out of serviceable condition and condemned. Renew with appropriately size suits.

Fire detection panel is exhibiting trouble alarm. Crew states that system is due for update. Have system serviced by authorized technician to remove fault indicator.

Vessel reported starboard rudder was stuck at 15 degrees left rudder. Upon investigation they found a loose solenoid coil that is responsible for receiving the signal from the pilot house. Solenoid was replaced, steering was tested from all locations and in all modes and reported to be functioning normally. Vessel is to maintain an anchor watch while transiting the remainder of the Detroit River. Once into Lake Erie, they are to sufficiently test the steering and report to the USCG Marine Inspector, MSU Toledo. Propulsion machinery, including main engines, must be maintained to ensure proper operation. Vessel experienced a failure of the #7 cylinder on the port main engine. Vessel may continue transiting towards their destination port of Toledo, OH under reduced propulsion and shall provide a report attesting that permanent repairs have been completed, to the satisfaction of Coast Guard marine inspector prior to departure.

Port rudders moving to starboard without operator input. Provide proof of proper operation at the satisfaction of the local OMCI.

The Starboard MDE has been placed out of commission due to bearing and crank damage. Port MDE and bow thruster remain fully operational. Vessel is allowed to operate on single engine in order to effect proper repairs. The bow thruster shall be fully operational while transiting on sinigle engine. Repairs are to be completed to the satisfaction of the cognizant OCMI, by 08MAY2019. While Transiting COTP Zone Sault St. Marie maintain contact VTS St. Marys River.

Starboard main diesel engine is inoperable due to an overheated crankshaft bearing. COTP Sault Sainte Marie revised the CG835 to allow the vessel to transit with the stipulation that the engine is repaired prior to entering back into the COTP Sault Sainte Marie zone. COTP Detroit and CO of MSD Toledo briefed. VTS is imposing a directive on the vessel prohibiting it from transiting the St. Marys River while winds are more than 30 kts.

Vessel is displaying an innaccurate IMO number and ANT Distance A and B information on the AIS.
