The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions.Emergency procedures were not present during inspection. Provide emergency instructions and post at the operating station. 46 CFR 185.510

No marine employer may engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. 01 Crew member was not listed in current drug testing program. Provide evidence of crew members drug testing 46 CFR 16.210

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] label on escape hatch, Provide label. 46 CFR 185.606

Each life jacket kept in a stowage container must be readily available. Several life jackets were stowed within individual plastic wrapping. Remove plastic wrapping from PFD's to ensure readily available.

A bilge pipe in a vessel of more than 19.8 meters (65 feet) in length must be not less than 40 millimeters (1.5 inches) nominal pipe size. A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. PVC strainer fwd of crash bulkhead was made of PVC and fell off during bilge pump testing. Replace strainer with suitable metal type.

The equipment required for life floats and buoyant apparatus must meet the following specifications: (1) Lifeline and pendants. The lifeline and pendants must be as furnished by the manufacturer with the approved life float or buoyant apparatus. Replacement lifelines and pendants must meet the requirements in Subpart 160.010 of this chapter. (2) Paddle. Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant.....(4) Light. The light must be a floating water light approved under approval series 161.010 or other standard specified by the Commandant. The floating water light must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. Life float pendants were found to be coiled and zip tied, life float paddles were not lashed to the life float to which they belong, and multiple water light lanyards were approximately 12 feet or less. Configure life floats in accordance with 46 CFR 180.175 to the satisfaction of attending marine inspector. 46 CFR 180.175(f)

The name of the vessel must be marked or painted in clearly legible letters on each life float paddle. None of the life float paddles were marked with the vessel's name. Mark life float paddle's with vessel's name to the satisfaction of attending marine inspector. 46 CFR 185.604(g)

Except for a storage battery used in a rescue boat, each battery without an expiration date indicated on it or for which the owner or managing operator does not have a record of the expiration date, used in an item of lifesaving equipment, must be replaced during the annual inspection. The water light batteries for the life floats and ring buoys did not have expiration dates marked on the batteries. Renew water light batteries to the satisfaction of the attending marine inspector. 46 CFR 185.726(e)

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Owner was unable to provide servicing records of fire extinguishing systems. Provide annual servicing records to the attending marine inspector. 46 CFR 176.810(b)(2)

A penetration in a watertight bulkhead must be... made watertight. Marine inspectors found penetrations in the 2nd and 3rd watertight bulkheads that were not watertight. Make bulkhead penetrations watertight to the satisfaction of the attending marine inspector. 46 CFR 179.320(c)

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crew member assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Owner/Operator did not have information posted on board vessel. Must provide information to crew members. 46 CFR 16.401(a)

Each buoyant apparatus must have Type II retro-reflective material meeting subpart 164.018 of this chapter on each side and end. All buoyant apparatuses have worn retro-reflective tape that needs to be replaced. 46 CFR 160.010-4(p)

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Maintenance was being conducted but not documented in a log. This corresponds with Monthly maintenance under 46 CFR 185.722. 46 CFR 185.702(d)

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Maintenance was being conducted but not documented in a log. This corresponds with Monthly maintenance under NFPA 10. 46 CFR 176.810(b)(2)(iv)

Spanner wrenches must be provided where a 40 millimeter (1.5 inch) diameter firehose is required by § 181.320(b). No spanner wrench was on board. 46 CFR 181.310(d)

Inspectors must also consider the results of the past year’s visual and analytical monitoring when planning and conducting inspections. Owner/Operator did not know they needed a VGP, thus they did not have one. Must obtain VGP documents. VGP Parts 3, 4.2 para 3 CG-543 Policy Letter 11-01

The pump outlet must be fitted with a pressure gauge. Gauge found inoperable.

The Rules concerning lights (§§ 83.20 through 83.31) shall be complied with from sunset to sunrise and in restricted visibility and may be exhibited in all other circumstances when it is deemed necessary.

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity: (i)The vital systems listed in 182.710 of this chapter; (ii) Interior lighting except for decorative lights; (iii) Communication systems including a public address system required under 184.610 of this chapter; and (iv) Navigation equipment and lights. Marine Inspector observed not all required equipment above was operational on back up power. Rectify prior to carrying passengers.

A station bill must be posted by the master on a vessel of more than 65 feet in length having a Certificate of Inspection requiring more than four crew members at any one time, including the master. The vessel did not have a station bill.

Vsl is broadcasting Invalid Call Sign and Vsl is broadcasting greater than expected vessel length

Pitting found in LAZ forward bulkhead IWO the keel. Once ground out, clad welding has been approved of 02 pits which have been identified.

Holes found in sewage piping throughout eng room. Proposed repair of cutting sections of piping and replacing with PVX with metallic penetrations through W/T bulkheads determined to be SAT. Complete repair to satisfaction of attending MI.

Bilge strainer in lazarette wasted.

Fill cable penetration in aft engine room bulkhead.

Replace bilge alarm in forepeak.

There were no servicing records for portable fire extinguishers. Provide.

03 paddles were not serviceable. Replace.
