Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station. Could not test #1 shut down due to inability to start prime mover. Demonstrate operation and remote shutdown prior to cargo operations. 46CFR 32.50-35(a).

Each cargo tank of a tank ship must be equipped with an intrinsically safe high-level alarm and a tank overfill alarm. The high-level alarm must activate an alarm once the cargo level reaches 95 percent of the tank capacity or higher, but before the tank overfill alarm. High level alarm on #1 Port Cargo tank was inoperative. Demonstrate operation prior to cargo operations in #1 port cargo tank. 46CFR 39.2007(a),(c)

Where pressure-indicating gages are used, the upper range of the gage shall not be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. Gauge range was outside of parameters, replace with appropriate gauge. 46CFR 54.15-5(f)

The spring in a relief valve in service for pressures up to and including 250 pounds per square inch must not be reset for any pressure more than 10 percent above or 10 percent below that for which the relief valve is marked. Relief valve did not reset within the required parameters. Demonstrate proper reset during operation of pressure relief valve. 46CFR 54.15-10(g).

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions, protection of passengers, crew, other persons, and the vessel from electrical hazards, and maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. Generator missing interlock protection between barge and shore power. Install prior to operation of generator. 46CFR 111.01(a)

Piping systems conveying vapors, gases, or mixtures whose temperature exceeds 150 °F must be suitably insulated as necessary to preclude injury to personnel. Lagging on #1 and #2 Cargo pump prime movers missing. Replace prior to operations of #1 and #2 cargo pumps. 46CFR 56.50-1(k)

Each tank vessel must meet the applicable requirements in sub-chapter S of this chapter as applicable to vessels with watertight doors in bulkheads that have been made watertight to comply with flooding or damage stability regulations. Watertight doors in starboard side accommodation were not properly sealing. Adjust doors to ensure proper closure/seal. 46CFR 31.10-30, 46CFR 170.248(a)

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. To maintain a valid Certificate of Inspection, you must complete your annual inspection within the period specified in § 31.10-17 (a) and (b) and your Certificate of Inspection must be endorsed. Vessel did not complete its annual inspection within the required time frame. Complete annual inspection prior to vessel movement.

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. They shall also be permanently marked on the sides of the vessels to the satisfaction of the assigning authority. The marks shall be plainly visible and, if necessary, special arrangements shall be made for this purpose. Attending Marine Inspector (MI) found load lines not properly painted with contrasting color. Enact permanent repairs to the satisfaction of the attending MI.

The vessel's name or number shall be so displayed at the highest part of the vessel's hull or permanent structure that the name or number can be seen from either side. MI found the vessels name and official number missing from port side of vessel. Enact permanent repairs to the satisfaction of the attending MI.

Each hose must be marked with one of the following: For oil products, the words "OIL SERVICE". MI found fuel and cargo hoses onboard not marked with the words "OIL SERVICE". Enact permanent repairs to the satisfaction of the attending MI.

Each hose also must be marked with the following, except that the information required by paragraphs (f)(2) and (3) of this section need not be marked on the hose if it is recorded in the hose records of the vessel or facility, and the hose is marked to identify it with that information: (1) Maximum allowable working pressure; (2) Date of manufacture; and (3) Date of the latest test required by 33 CFR 156.170. MI found the fuel and cargo transfer hoses onboard do not have the proper markings as required. Enact permanent repairs to the satisfaction of the attending MI.

The vessels cargo temperature gauging system is broken/missing probes/has exposed wires. This system shall not be used until it is fixed or replaced.

No repairs or alterations affecting the safety of the vessel or its machinery shall be made unless applicable requirements in this chapter are met. All required documentation required to be submitted to the cognizant OCMI as stated in CG MSC Letter serial#C1-2202232 dated July 7, 2022 must be submitted for review. Barge is not permitted to conduct any work affecting the safety of the vessel and its load line until all the comments addressed in the mentioned letter above, a repair plan to include proof diver operation is conducted in accordance with 46 CFR Part 197, and information on the cofferdam installation is submitted to cognizant OCMI and written approval is received.

Replace all battery cable installations labeled "for welding use" with the proper battery cable.

The terms, endorsements and conditions set forth on a certificate of inspection shall have the same force and effect as the regulations contained in this subchapter. Prior to loading cargo, contact the nearest CG unit to obtain an amended COI with the updated operator and associated operator controlled documents and procedures.

The following locations in the weather area are class 1 division 1: within 5 meters (16ft) of cargo pressure/vacuum valves with an unlimited height. Barge has multiple deck lights rated as class 1 division 2 within 16 feet of pressure/vacuum valves. 46 CFR 111.105-31(l)(3). 16c

Each cargo tank of a tankship must be equipped with an intrinsically safe high-level alarm and a tank overfill alarm. HL/OF alarms are currently seized. Provide proof of functionality upon first loading of cargo for all alarms

Vessel must be built to a strength standard (ABS) or other recognized classification society as per the regulation and maintained to ensure safe service. Vessel was built to ABS standard during a NVIC 10-82 project. Deck plating comprises a highly stress portion of the hull girder and is critical to the longitudinal strength of the vessel. During the recent DRY-DOCK inspection massive pitting was found through the entire deck and subsequent UT testing report shows extensive wastage over allowable limits throughout the entire deck. Must submit a repair/replacement plan to Sector New York inspections and Classification Society for review/approval prior to work commencing.

All installations shall be of marine type engines suitable for the intended service. Marine Inspectors witnessed #2SSDG with fuel leaks from #5 fuel injector. Repair to satisfaction of attending Marine Inspector.

All installations shall be of marine type engines suitable for the intended service. Marine Inspectors witnessed excessive soot build up on #1SSDG turbo charger and lagging causing potential fire hazard. Vessel to rectify deficiency to satisfaction of attending Marine Inspector.

Exhaust system shall meet the requirements of 46CFR58.10-5(d) and ABYC P-1 ; shall be designed and installed to prevent the ingress of water from entering the exhaust system under normal operating conditions. Marine Inspectors observed three individual exhaust mufflers, each with approximate 6-inch diameter holes. Repair to original design prior to utilizing installed cargo pumps.

Each junction box must be suitable for the environment in which it is installed. Wire gland on the starboard running light was loose and did not provide a watertight connection. Cable entrance to the running light must be made watertight. 46 CFR 111.81-1(f)

The person in charge of a vessel shall be responsible for monitoring the security and integrity of the tank barge and for ensuring adherence to proper safety precautions. These responsibilities include, but are not limited to - (i) Ensuring that any tank barge added to the tow has all tank openings properly secured; has its freeing-port and scuppers, if any, unobstructed; meets any loadline or freeboard requirements; and neither leaks cargo into the tanks, voids, or cofferdams. Port forward void hatch gasket was leaking. The lip of the hatch appeared to be in questionable condition. Replace watertight gasket in forward port cargo hatch. 46 CFR 35.05-15(a)(i)

All electrical installations in hazardous locations must comply must comply with the general requirements of section 33 of IEEE 45-1998. Numerous Tank Level Indicators on the cargo deck has significant oxidized and cracking fittings at the cable entrances. Replace wasted wiring runs to electronic tank level monitoring devices throughout barge as original. 46 CFR 111.105-3

Each vessel vapor connection flange face must have a permanent stud projecting outward that is at least 1/2 inch diameter and is at least 1 inch long. Stud on the starboard side was approximately 40% wasted and flange face showed signification oxidation potentially prevented a liquid tight connection. Prior to transferring any 46 CFR Subchapter D flammable and/or combustible cargoes in accordance with vessel's Cargo Authority Attachment that collect vapors, ensure vapor collection systems meets requirements to the satisfaction of the attending U.S. Coast Guard marine inspector. 46 CFR 39.2001

All tank vessels, the construction or conversion of which is started on or after July 1, 1951, shall conform to the requirements in this subpart in construction of hulls. The hull and deckhouses shall be of steel or iron construction except that the pilothouse and decks over quarters may be constructed of wood. Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Numerous areas on the forward cargo deck appear to have excessive pitting approximately 3 inch in diameter. These areas must be gaged to determine extent of wastage and report provided to Sector NOLA. These ares shall be considered for replacement during next yard period. 46 CFR 32.60-1

At each inspection for certification the marine inspector shall conduct such test and inspections of the auxiliary machinery and of its associated equipment as he feels necessary to check safe operation. Prior to the inspection, the marine inspector notified the company that high jacket water temperature, and over-speed trips be tested on each auxiliary engine among other test, however these test were not preformed during the inspection. Provide a technician's report certifying proper operation of high jacket water temperature and over-speed trips on each auxiliary engine. 46CFR31.10-17(a)(2) 46CFR61.20-3(a)

In general, connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable. The battery terminal ends of each starting battery cables for all 4 generators and two cargo pumps were corroded and secured with wing nuts, not allowing for a secure connection. Replace all terminal ends to each generator and cargo pumps with suitable terminal connections and captive securing devices that will not back out with vibrations. 46CFR31.35-1 46CFR111.30-17

All installations shall be of marine type engines suitable for the intended service. The #2 diesel generator was leaking coolant during the inspection. The leak was corrected prior to departure of the Marine Inspector. 46CFR31.30-1 46CFR58.10-10(a) 46CFR58.10-5(a)

Company failed to notify the Coast Guard of three (03) Service generator diesel prime movers replacements.

International Air Pollution Certificate is invalid. Company failed to have IAPP survey completed on newly installed service generator prime movers (03). Vessel restricted from sailing foreign until new IAPP survey is completed and IAPP Certificate is issued. MARPOL VI/5.1.5

Company failed to notify the Coast Guard of newly installed MSD/Grey Water tank and piping arrangement.

Prior to departure from anchorage. Renew worn hydraulic hoses and leaking valve at forward mooring winch to the satisfaction of attending Marine Inspector.

Prior to departure from New Orleans, have high level overfill devices serviced. High level/overfill device for #2 STBD C/T did not function as manufacturer intended. Troubleshoot and affect repairs to the satisfaction of the attending Marine Inspector.

Prior to departure from New Orleans, finalize troubleshooting procedures and affect repairs to the aft Thermal Fluid Heater. Provide all technician’s reports from attendance in Port of New Orleans AUG2018. Affect repairs to the satisfaction of the attending Marine Inspector.

Prior to departure from New Orleans, troubleshoot and affect repairs to the Electronic tank level indicating device (Bergen System). Affect repairs to the satisfaction of attending Marine Inspector.

Prior to departure from New Orleans, renew leaking fittings, hoses, & piping arrangements on both cranes to the satisfaction of attending Marine Inspector.

Remove "jumpers" on Low Fuel Oil PR for aft Thermal Fluid Heater(TFH). Provide technicians report to attending USCG Marine Inspector attesting to the operational readiness of the TFH, and conduct testing of the TFH in the presence and to the satisfaction of USCG Marine Inspector prior to next use.

Re-install Lockout bar on generator switchboard to ensure generators will not be simultaneously used due to lack of syncroscope or ability to parallel in accordance with 46 CFR 111.30-25(d). To the satisfaction of a USCG Marine Inspector.

Repair two deck lights on outboard side of Crew House. Drain water and prove intrinsically safe to the satisfaction of the attending USCG Marine Inspector.

Remove "Oil" soaked lagging from TFH space and complete floor cleaning to decrease fire hazard and slip, trip, fall safety to the satisfaction of a USCG Marine Inspector.

Repair Deck Eye Wash/Shower to as designed condition to the satisfaction of USCG Marine Inspector.

The bellow covers on all three cargo pumps are not vapor tight.

Flame screen on port vent for the sewage tank has excessive build up of residue.

Found in the Thermal Fluid Htr room, ASH Monitor System shows code "F4" Sensor Leads open or Shorted. Repair or Provide Tech report to testify as to thecriticalness of component to system operation and / or safety of the thermal fluid htr system prior to cargo operations.

Found in Tankermans house, Beacon 200 gas monitoring system was inop. Provide proof of proper operation of gas monitoring system. (Prior to crew sleeping onboard)

See worklist, several ballast tank hatches and combings are wasted, must renew as appropriate prior to credit ddx.

Service fire screen door to crew accommodations to ensure proper self closing.

Replace light guard in crew accommodations and properly mount 02 exterior lights IWO deck fuel tank.

Hardwire L.E.L. sensor in crew accommodations.

Replace relief valve on air receiver IWO generator room.

Hull plate portside bottom damaged by cutting of staging just aft of ballast outtake. C/R with insert.

Provide a copy of a valid ILLC prior to operating.

Demonstrate the low lube oil pressure shut downs on all generators fuction properly. 46 CFR 111.12-1c.
