The scantlings, material, and workmanship, the subdivision of cargo spaces, the arrangement of cofferdams, the testing of tanks and cofferdams, shall be at least equivalent to the requirements of a recognized classification society for the particular service specified in the application for the certificate of inspection and permit for the transportation of liquid flammable cargoes in bulk as of the date when the tank vessel was built or as of the date when the vessel Vessel self reported 8x1/4 fracture in the side shell of the No.5 port wing tank, aft hatch, 12 in above the waterline. Vessel plans to apply temp repairs at J&S Shipyard before transiting to conduct permanent repairs at Texas Boat and Barge in Freeport,TX. Vessel authorized one time transit to Texas Boat and Barge facility for permanent repairs to satisfaction of USCG.

Scantlings, material, and workmanship shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. 3 inch by 1/16 fracture found located at the bow, starboard side transition knuckle, 8 feet for the bulkhead and 4 feet from the bottom. Make permanent repairs IAW NVIC 07-68 prior to taking on cargo.

Openings for fill and vent pipes must be on the topmost surface of a tank. There must be no openings in the bottom, sides, or ends of a tank except an opening fitted with threaded plug or cap may be used on the bottom of the tank for tank cleaning purposes. Clean-out on bottom of tank is equipped with a flange fitting without a spray shielding vs threaded fittings. Install flange spray shielding in accordance with CVC Policy letter attached.

The following deficiencies were identified: 1) #4 Starboard wing void improper repair of two hull plate stiffeners. Apply proper fit-up and full penetration welds on both sides of repair in accordance with NVIC 07-69. 2) #3 Port wing void improper repair of hull plate stiffener. Apply full penetration welds on both sides of repair in accordance with NVIC 07-69. 3) Repair the following pick-ups: * 4 pick-ups in #2 port wing void * 3 pick-ups in #3 port wing void * 3 pick-ups in #1 port wing void.
