Pitting was found in the fore peak tank on the third level behind against the watertight bulkhead to tank 1 on the port and starboard side aft of the anchor chain lockers. Gauge in the presence of a marine inspector.

A pitted weld in the 2 starboard ballast tank in the double bottom 3rd bay from the aft watertight bulkhead two bays in on the cargo tank bottom. Gauge in the presence of a marine inspector.

The aft peak tank hatches had evidence of leaking. Reseal the hatch and test for watertight integrity in the presence of a marine inspector.

Two closed hatches were found in the aft peak tank. Open the hatch and ensure space is gas free for inspection.

On all tankships where the plans and arrangements will possibly permit, all passageways leading to living quarters, or places where anyone may be regularly employed, shall be provided with not less than two avenues of escape so located that if one of such avenues is not available another may be. The locality and arrangement of such additional means of escape shall be determined by the inspectors as will in their judgment best carry out the purpose for which this provision was made. MI observed lack of maintenance on both generator room escape hatches resulting in significant difficulty in opening hatches as intended in addition to wastage of one hatch cover dogging bar recess on the stbd generator room escape hatch. Operator to conduct maintenance as per manufacturers instructions and affect repairs to the wasted recess to the satisfaction of attending MI by way of photo documentation. 46CFR32.02-1 16c.

Operational tests and checks of all safety and limit controls, combustion controls, programming controls, and safety relief valves must be conducted by the owner, chief engineer, or person in charge at the inspection for certification, periodic inspection, and when directed by the Officer in Charge, Marine Inspection, to determine that the control components and safety devices are functioning properly and are in satisfactory operating condition. These tests and checks must be conducted in the presence of a marine inspector and must include the following: proper prepurge, burner ignition sequence checks, operation of the combustion controls, limit controls, fluid flow controls, fluid level controls, high temperature control, proper postpurge control, and verification of the flame safeguard. MI observed significant difficulty of thermal fluid heater operator in demonstrating the above operational checks. Operator is required to request inspection of thermal fluid heater by servicing OCMI and demonstrate proper operation of tests to the satisfaction of the attending MI prior to conducting cargo operations requiring the thermal fluid heater. 46CFR61.30-20 705c

Vessel is overdue for the next Internal Structure Exam, as per date on COI. After discharging cargo in Delaware City, DE, vessel is scheduled to return to Sector NY COTP zone. At that time, OOA 07Sep2022, operator must make the vessel available to CG Marine Inspectors for an Internal Structure Exam.

Vessels that operate in salt water must be inspected in accordance with the intervals set forth in table 31.10-21(a). Where the table 31.10-21(a) indicates a 2.5 year examination interval, it means a vessel must undergo two examinations within any five year period. No more than three years may elapse between any two examinations. Vessel has gone past its Internal Structural Examination date. Provide documentation validating different ISE date or conduct ISE with CG MI's. 46 CFR 31.10-21(a)(1)

46 CFR 151.2035 (a): To discharge ballast water into waters of the United States, master, owner, operator, agent, or person in charge of a vessel subject to 151.2025 of this subpart must either ensure that the ballast water meets the ballast water discharge standard as defined in 151.2030 (a). The MV ACHIEVEMENT a US Flag Petroleum tug and barge loaded with petroleum experienced equipment failure in ballast system at BR Exon dock #1, position 30’ 28.6N 91’11.6 W. Sent an e-mail notifying them that discharge is prohibited. Must discharge outside of 12NM or at the facility, no EC authorized. Vessel must clean the ballast tanks to remove sediments and properly operate their system prior to discharge to the satisfaction of CG Inspector or TPO.

Cargo piping test letter does not include the discharge piping which goes through the vacuum tank and sends product ashore. Vapor is emiting from the piping in the vicinity of the vacuum pump. Test the piping in the vicinity of the vacuum tank and produce documentation satisfactory to USCG prior to discharging product through the vacuum tank.

Each vapor connection flange must have a permanent stud located at the top of the flange. Replace flange end to ensure stud is secured properly. No stud was visible through flange end on forward starboard line at manifold.

Make repairs. Replace Starboard SSDG Breaker and prove proper operation of the starboard generator to parallel and operate reverse power relay.
