Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter... Vessel needs to test alarm set points for main engine & auxiliary engine Twice every 5 years, with no more than 3 years elapsing since last test. Provide a tech report showing that all alarms were tested

Each towing vessel must be maintained and operated so as to minimize fire hazards. Non-approved fire stop was being used in engine room for cable run bulkhead penetration. Replace with approved fire sealant.

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. Semi-portable extinguisher on 01 deck was not permanently fixed.

Immersion suits carried on towing vessels must meet the requirements of § 199.70(c) and (d) of this chapter. Markings.(c)Each immersion suit or anti-exposure suit must be marked in such a way as to identify the person or vessel to which it belongs. Immersion suits did not have the vessel's name.

A Coast Guard Option inspected towing vessel must undergo an annual inspection within three (3) months before or after the COI anniversary date. The vessel was found to have not completed its annual inspection within the required periodicity. Vessel is retricted from commercial operations until completion of COI Inspection.

All lifesaving equipment must be tested and maintained in accordance with the minimum requirements of § 199.190 of this chapter. Each inflatable lifesaving appliance and marine evacuation system must be serviced w/in 12 months of its initial packing; w/in 12 of each subsequent servicing. IBA was overdue for servicing. 46 CFR 141.235 46 CFR 199.190

AIS must be maintained in effective operating conditions, which includes the accurate input and upkeep of all AIS data fields and system updates. Upon re-attendance, present logs showing AIS updates and demonstrate for attending marine inspectors. 33CFR164.46(d)(2)(iv)

To stop the flow of fuel in the event of a fire or break in the fuel line,a remote fuel shutoff valve must befitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Fuel shutoff valves did not close completely, would not stop flow of fuel. Re-calibrate and demonstrate for attending marine inspectors upon re-attendance. 46 CFR 143.260

The Coast Guard, on its own initiative, may examine or inspect or reinspect at any time any vessel subject to inspection under subtitle II, title 46 of the U.S. Code, title 46 and title 33 U.S. Code. If during such examination, inspection, or reinspection, any failure to comply with any applicable requirement of law and/or applicable regulations in this chapter, or any defects or imperfections become apparent tending to render the navigation of the vessel unsafe, or that repairs have become necessary, the Coast Guard will so notify the master and state what is required. Observed multiple items rendering the vessel not ready for inspection. Address list described below and make vessel available for inspection w/in 14 days when fully prepared for inspection. a) Locate annual servicing records for all portable/semi-portable fire extinguishers b) Locate Health and Safety plan, and associated annual training records c) Locate safety orientation records d) Locate crew training and emergency drill records e) Locate towing gear certificates and records f) Locate records of oil residue discharges/disposals g) Locate DAPI enrollment certificate for crew/vessel h) Review TVR requirements logging requirements per 46 CFR 140.915, adjust procedures to ensure compliance i) Prepare to test fixed bilge suction without discharge overboard, to be tested upon re-attendance j) Prepare relevant equipment required to test heat detection in engine room and galley, to be tested upon re-attendance k) Make relevant preparations to test pressure relief valve tests on both air receivers, to be tested upon re-attendance l) Remove oil soaked pads from under both engines m) Tighten PORT shaft seal packing 46 CFR 2.01-10(b)

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. STBD battery utilized clips presenting an electrical hazard.

Each towing vessel must have...a portable pump, and hoses and nozzles, in accordance with paragraphs (e) and (f) of this section. The portable fire pump failed during a test and the pump is no longer operational. Replace pump prior to towing operations.

A towing vessel that is certificated for coastwise service whose contract for construction was executed prior to August 27, 2003,must be equipped with either an approved 160-B semi-portable fire-extinguishing system to protect the engine room or a fixed fire-extinguishing system installed to protect the engine room. Vessel does not have an approved 160-B semi-portable fire extinguisher on board.

No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for the employer. Provide records that crew members underwent chemical testing prior to employment.

Station documents must be readily available to the licensed operator(s) on duty during the hours of service of the station and to authorized Commission employees upon request. Vessel did not have a FCC Bridge to Bridge Radiotelephony certificate onboard. 47 CFR 80.403

Each ship subject to the Bridge-to-Bridge Act must have on board a radio operator who holds a restricted radiotelephone operator permit or higher class license. Vessel did not have a radio operator who holds a marine radio operator permit. 47 CFR 80.163

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel has not been conducting monthly inspections. 46 CFR 142.240(a)(ii)

The regulations in this part provide a means to minimize the use of intoxicants by merchant marine personnel and to promote a drug free and safe work environment. Vessel did not have required DAPI documentation available for inspection. 46 CFR 16

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either:(1) Automatic, battery-operated with a duration of no less than 2 hours; or (2) Non-electric, phosphorescent adhesive lighting strips that are installed along escape routes and sufficiently visible to enable egress with no power. Vessel does not have emergency lighting along escape routes. 46 CFR 143.410

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards.Battery in engine room is not secured. 46 CFR 143.400(a)

n exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. This is not meant to restrict access to towing equipment such as winches, drums, towing gear or steering compartment equipment necessary for the operation of the vessel. Vessels air compressors did not have installed guards. 46 CFR 144.820

The owner, master, or operator of each vessel towing shall ensure that the strength of each towline is adequate for its intended service. The port aft facing wire had signs of being crushed, several broken wires and a fishook protruding from the core. 46 CFR 164.74 46 CFR 164.74

Towing vessels must meet the communications requirements of 33 CFR part 26 and 33 CFR 164.72, as applicable. Vessels MMSI was not programmed into either of the VHF-FM radio. 46 CFR 140.715

The owner, master, or person in charge of each vessel underway shall ensure that: (i) Magnetic variation and deviation and gyrocompass errors are known and correctly applied by the person directing the movement of the vessel. Vessel did not have a compass deviation card. 46 CFR 164.11

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Extracts or copies from the publications listed in paragraph (b) of this section may be carried, so long as they are applicable to the route. 46 CFR 140.705

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of §141.370 for specified areas where the vessel operates. Vessel had only 4 unexpired visual distress signals onboard. 46 CFR 141.375
