Machinery driving...fuel oil unit and service pumps must be fitted with remote controls...so that fans or pumps may be stopped in case of fire in the compartment. Emergency fuel shutoff remote pull found inoperable and unable to shut down engine.

Wastage on diesel fuel vents, ballast vents, flame screens and closures on several installations was identified. COnduct full audit of tank vent installations and make proper repairs.
