Means shall be provided to ensure safe, convenient and unobstructed passage for any person embarking on, or disembarking from, the ship between the head of the pilot ladder, or of any accommodation ladder or other appliance, and the ship's desk. Vessel reported and PSCO observed starboard side accommodation ladder heavily damaged and inoperable.

Resolution: Received class report issuing condition of class until 23OCT24.
The condition of the ship and its equipment shall be maintained to conform with the provisions of the present convention to ensure that the ship in all respects will remain fit to proceed to sea without presenting an unreasonable threat of harm to the marine environment. The OWS is required to be tested weekly per the manufacturers operating manual for the oil content meter. A bottle brush is required to clean the glass eye weekly per the manufacturers operating manual. the vessel did not have one. MARPOL (2011) ANNEX I Reg. 6.4.1
