Auxiliary machinery vital to the main propulsion system must be provided in duplicate unless the system served is provided in independent duplicate, or otherwise provides continued or restored propulsion capability in the event of a failure or malfunction of any single auxiliary component. Vessel reported one of two auxiliary diesel generators inoperable. Provide causal analysis and make proper repairs prior to ATB cargo operations. One tug escort required while moving inbound and around San Francisco Bay OCMI/COTP Zone. 46 CFR 58.01-35

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that: any non-conformity is reported, with its possible cause, if known; appropriate corrective action is taken; and records of these activities are maintained. Deficiencies 2, 3, 4, 5, and 6 were identified by attending inspectors and associated internal non-conformities were not available. Company shall conduct an internal vessel audit focused on these findings within 3 months. 74 SOLAS (20) IX/3.1 ISM CODE 10.2

Every person designated to take charge of medical care onboard vessels subject to the STCW Convention must hold an MMC endorsed as Person in Charge of Medical Care. Vessel sailed without PIC medical care designated or qualified crew member during the previous voyage. 46CFR15.404(i)

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and life raft embarkation deck.02 emergency escape hatches (01 engine room and 01 forward stores) found not easily operable to allow a swift means of escape. Forward stores hatch noted as having an outstanding work order for repair from July of 2024. 74SOLAS(20)II-2/13

Measures shall be taken to the satisfaction of the Administration to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. Vessel found utilizing Periodic Safety Test Procedures which had been modified multiple times after approval without resubmittal. Records were not available to indicate procedures had been completed during the previous calendar year. 74SOLAS(20)II-1/46.2

It shall be the duty of the master or other person in charge of the vessel to see that all of the provisions of the certificate of inspection are strictly adhered to. Vessel found utilizing outdated conditional occupancy policy. Vessel records indicate canceled CG-CVC Policy 16-04 in use in lieu of requirements found in NVIC02-81 CH2. 46CFR97.50-1(a)

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. Two dead end live green wires under port bridge console and approximately 4 non-energized wires under starboard bridge console were identified. 74SOLAS(20)II-1/40.1.3

Maintenance, testing and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. Line throwing rockets all found expired. 74SOLAS(20)III/20.3.1

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. Panel boards throughout vessel found with multiple circuit directories missing information or breakers not matching installed breakers.74SOLAS(20) II-1/45.6.2

In order to achieve the fire safety objectives set out in paragraph 1, the following functional requirements are embodied in the regulations of this chapter as appropriate: ready availability of fire-extinguishing appliances. Fire and safety lockers found disorganized hindering ready access to critical safety equipment. 74 SOLAS (20) II-2/2.1.7

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Lagging at emergency fuel oil shutoff air supply line in AC flat to upper purifier requires repair. 74SOLAS(20)I-11

(a) On a merchant vessel of the United States the construction of which began after March 4th, 1915..(5) each crew berthing area shall be equipped with information regarding-(A) vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage; and (B) procedures and resources to report crimes, including sexual assault and sexual harassment, including information-(i) on the telephone number, website address, and email address for reporting allegations of sexual assault and sexual harassment to the Coast Guard;(ii) on vessel owner or company procedures to report violations of company policy and access resources; (iii) on resources provided by outside organizations such as sexual assault hotlines and counseling;(iv) on the retention period for surveillance video recording after an incident of sexual harassment or sexual assault is reported; and (v) additional items specified in regulations issued by, and at the discretion of, the Secretary of the department in which the Coast Guard is operating. (d) In each washing space in a visible location there shall be information regarding procedures and resources to report crimes upon the vessel, including sexual assault and sexual harassment, and vessel owner or company policies prohibiting sexual assault and and sexual harassment, retaliation, and drug and alcohol usage. (a) In General.-The owner of a vessel subject to inspection under section 3301 shall-(1) ensure that such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to the individuals described in paragraph (2); (2) establish a list of all crew, identified by position, allowed to access and use the master key and maintain such list upon the vessel, within owner records and included in the vessel safety management system; (3) record in a log book information on all access and use of the vessel's master key, including- (A) dates and times of access; (B) the room or location accessed; and (C) the name and rank of the crew member that used the master key; and (4) make the list under paragraph (2) and the log book under paragraph (3) available upon request to any agent of the Federal Bureau of Investigation, any member of the Coast Guard, and any law enforcement officer performing official duties in the course and scope of an investigation. vessel has been discharging food waste into the sea in the Gulf of Mexico no grinder installed.The Gulf of Mexico is a no discharge area unless food waste is processed threw a grinder.

Safety management system contains outdated garbage management plan.

Vessel is to remain at achorage 9 and complete repairs on reported port engine governor control. Provide technician report. 46CFR91.45-5

Ensure proper operation & fitting for watertight door as i.d. on vessel drawings

Replace relief valve as indicated to ensure valve does not reliefe above MAWP/250 as i.d. on ship service air.

Repair Port Engine prior to vessel movement. If vessel needs to move, vessel is required to contact local Marine Inspections office. All items to be completed to the satisfaction of the cognizant OCMI

Rudder angle indicators are not consistent between the bridge and engine room. Align indicators to match.

Include reverse power relay testing procedure in Periodic Safety Test Procedures IAW; 46CFR62.01-5 (b)(1), Applicability, 46CFR62.20-1(a)(7), Plans for approval and 46CFR61.40-10(a), Test procedure details .

46 CFR 58.25-25(b): Rudder angle indictor at local control station in engine room reading off by 10 degrees in right rudder direction. Properly calibrate rudder indicators.

46 CFR 111.12-11(f): Port generator reverse power relay not functioning. Crew to not operate in parallel except to switch generators until repaired. Repair port generator reverse power relay.

46 CFR 111.60-17: Dead end electrical cable coiled above engine room alarm and monitoring panels not properly terminated. Properly terminate dead end wiring.

46 CFR 111.30-25(d): Main switchboard equipped with synchroscope but no synchronizing lamp. Provide synchronizing lamp.

A 6'' fracture has developed in the starboard #2 fuel oil day tank. The bottom of the tank forms the bottom hull plating. The fracture is located on the bottom plating above the starboard shaft strut where the outboard bulkhead joins the bottom plating. The fracture has completely breeched the hull.

The starboard propeller shaft seal has spun inside its collar rendering the starboard shaft out of commission.

On 23JULY2013 vessel reported that the #1 steering gear pump was not working. Vessel switched to #2 pump and sailed into Port Angeles for repairs.

Upon completion of repairs to the steering gear follow-up controller and prior to getting underway, conduct steering gear test in the presence of the attending Coast Guard Marine Inspector. Test to be conducted in accordance with 33 CFR § 164.25

Vessel may not operate until proper operation of the main engine and related systems are witnessed by the attending Coast Guard Marine Inspector.

Renew/repair port engine camshaft speed sensor with satisfactory test.

Make effective repairs to to vessel's steering system. Perform post-repair operation of the steering system in the presence of the attending Marine Inspector. Conduct U/W operational test of the steering system under normal U/W loads on the rudders. Upon successful completion of test notify attending Marine Inspector.

Prove that rescue boat engine is SOLAS compliant prior to sailing on an international voyage.
