PSCO observed approximately 40% of required quantity of water expired. Resolution: Reviewed and accepted class report from LR. The report stated there was a defect relay contact for port feedback operation signal on pump #1. Tech replaced parts, and class witnessed satisfactory test.
Ships shall comply with an intact stability standard acceptable to the Administration. PCO observed penetration to hull following marine casualty. ICLL (88) 1.3

Resolution: Reviewed and accepted class report from LR. The report stated a repair company was hired to completed necessary repairs to FWD Ballast Peak Tank. LR witnessed pressure test with 1.5Bar and NDE on the bow shell plate in 100% of the rewelded area.

A barrel of cylinder oil was discovered soft piped into the main engine lubricating system. (74) SOLAS Chapter I, Reg 11(b) -Rectify to satisfaction of CG and Class.

Oil filtering equipment shall be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when oil content of effluent exceeds 15ppm. Port State Control Officer observed, while testing oily water separator, ship's crew could not get a sample of outboard effluent through oil content meter. MARPOL Annex I Reg 14.7.-Rectify to satisfaction of CG and Class.
