The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. A valve shall be fitted to serve each fire hydrant so that any fire hose may be removed while the fire pumps are in operation. Fire main valve outside hose station 5 is leaking. 74 SOLAS (04 Cons) II-2/10.2.1.4.2 50a

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must:(1) Be maintained to ensure proper operation. Port main diesel engine secured due to low HT water pressure. Make repairs to port main diesel engine to the satisfaction of the attending USCG marine inspector.

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling or daily service tank having a capacity of 500l and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Remote operation of HFO service system port fuel oil shut off valve was inoperable. SOLAS II-2/4.2.2.3.4

Vessel still does not have USCG reviewed and fully approved PSTP's. ABS COC 414: PSTP Review, will be extended for 3 months (09AUG2022) to incorporate and submit comments to USCG for review.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. While reviewing rescue boat maintenance records, it was noted that the vessel has not completed required weekly maintenance on the rescue boat required by 74SOLAS(14)ChpIII/20.6. It was also noted that this requirement is not incorporated in the SMS. Company shall create a non conformity and corrective action plan as per the SMS. 74SOLAS(14)ChpIX ISM Code Chp 10.1

While reviewing fire fighting maintenance records, it was noted that the annual maintenance on the galley steel rolling fire door has not been completed as per manufactures recommendation. The vessels crew was unable to complete PSTP test number 3.7, SSDG FO Pressure Low on the #1,2 or 3 ssdg because of broken fuel pressure gauges. The gauges shall be repaired or renewed so the PSTPs can be satisfactorily completed.

During the annual inspection, the CG reviewed the PSTP log noting several items were marked unsat. Found some PSTP items were unable to be tested due to the vessel's system configuration and others items required repair or maintenance prior to testing. If a PSTP cannot be completed because of inoperable sensors or equipment, a work order should be created in NS5 per the company procedures. An internal ISM audit shall be completed of the vessel's PSTP program specify noting items that are unable to be tested per the approved PSTPs and additional items that require work orders to be created and addressed for repairs. The results of the audit shall be submitted to the CG for review.

Vessel lost the starboard engine while transiting in the Delaware River and the vessel is currently anchored outside of Wilmington, DE. Provide tech report identifying the cause of the loss of propulsion and prove proper operation to the satisfaction of the vessel's authorized Class Society Surveyor and Coast Guard Marine Inspector prior to movement.

OSG Vision had a controlled shut down of the port main engine due to high exhaust temp. Vessel must provide cause and analysis with repair plan to CG Sec DELBAY inspections division. Vessel is authorized to move from Bombay Hk to Del City via assist tug M/V Linsey to conduct6 cargo operations at the dock. Vessel will not be able to leave Del City until permanent repairs are complete to the CGMI

In main engine room emergency escape, replace light bulbs and mark with "E"

Prove proper operation of lower start air receiver relief valve prior to use of receiver.

Make the vessel available for quarterly underwater inspections of the temporary repair made to the rudder/nozzle system to the satisfaction of the attending MI.

Vessel was unable to demonstrate proper operation of reverse power protection for ship service generators. Vessel is required to develop proper test procedure and prove proper operation of reverse power protection.

Prove proper operation of OWS. Vessel shall maintain all waste oil and oily bilge water onboard while underway until proper operation of OWS is demonstrated.

Vessel has not completed Periodic Safety Test Procedure as required by 46 CFR 61.40-6. Vessel shall complete Periodic Safety Test Procedure within 60 days.

Prove proper operation of rudder angle indicator while using pump #1.

Prior to getting u/w have a steering control panel systems technical representative attend vessel and provide a report attesting to the cause of failure and a report of corrective actions and tests.

Prior to getting underway provide tech report detailing cause of failures to driver cards and corrective actions.
