An embarkation ladder complying with the requirements of paragraph 6.1.6 of the Code extending, in a single length, from the deck to the waterline in the lightest seagoing condition under all conditions of trim of up to 10 & a list of up to 20 either way shall be provided at each embarkation station or at every 2 adjacent embarkation stations for survival craft launched down the side of the ship. The vessel was found to have only 1 embarkation ladder,whereas the vessel's Fire Plan & SOLAS regs require 2 (one on each embarkation station). Ensure the provision of a second embarkation ladder. 74 SOLAS (20) III/11.7

A fixed gas fire-extinguishing system complying with the provisions of the Fire Safety Systems Code. At each location there shall be clear instructions relating to the operation of the system having regard to the safety of personnel. Vessel’s SAPPHIRE fixed firefighting system had incorrect placards for operating instructions. Replace placards with approved placards from manufacturer’s manual.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Fire main piping had small pin hole leak along seam of piping. Leak is on the piping below the overhead of the engine room leading after the main fire pump. Repair fire main piping.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Main fire pump’s pressure gauge read 25 psi at the pump. Fire hose testing performed at satisfactory pressure with incorrect gauge reading. Renew pressure gauge.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. The forward metal plate on the starboard main engine for the turbo washing gear is snapped from the mount. The oil line into the turbo snapped and is capped. Repair broken plate and oil line.

Two means of escape shall be provided from each machinery space of category A. In particular, one of the following provisions shall be complied with: .1 two sets of steel ladders, as widely separated as possible, leading to doors in the upper part of the space, similarly separated and from which access is provided to the open deck. One of these ladders shall be located within a protected enclosure that satisfies regulation 9.2.3.3, category (4), from the lower part of the space it serves to a safe position outside the space. Self-closing fire doors of the same fire integrity standards shall be fitted in the enclosure; Emergency escape door at the bottom of the engine room is a watertight door that is not self-closing. Outfit door with self-closing arrangement.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Vessel reported a reduction in propulsion due to turbo malfunctioning while transiting to [name withheld] Terminal near Cross Ledge Light. Vessel is completing remaining transit to [name withheld] Terminal for cargo offload and then will transit to Paulsboro Marine Terminal for repairs, with tug assist. Complete repairs to the satisfaction of Class prior to departing Paulsboro Marine Terminal.

Vessels approved NOV22 PSTP's did not match what the crew was actually doing to test the Main Engines and Generators. When the C/E was asked for their copy of the PSTP's, the crew only had pages 1-40 out of the 140 page approved PSTP. The shut down and alarm testing was in the pages the crew did not have. The C/E stated he has never tested over speed, hight temp and low lube pressure shut downs/alarms the way the procedures states, they usually call in a third party to test by using a single generator and lowering set point/ parameters. Crew also stated they recently installed a new Kongsberg system. It looks to be power management for the gens but could be automation in general. Which seems like they system was never commissioned/tested by ABS or CG after installation. Vessel is required to resubmit PSTP's to be approved and tested to the satisfaction for ABS Vessel is also required to have the new Kongsberg system approved and tested to the satisfaction of ABS.

The main steering gear shall be capable of putting the rudder over from 35 on one side to 35 on the other side in not more than 28 s. Starboard rudder is turning slow while under non followup control from the center bridge console. This is an intermittent issue that seems to correct itself after several minutes. 74 SOLAS (2020Cons) II-1/29.3.2

SOLAS (20) II-2 14.2 At all times while the ship is in service, the requirements of paragraph 1.1 shall be complied with. Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. A smoke detector in the aux machinery space was observed to be covered with plastic wrap.

SOLAS (20) II-2 4.1 The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids. The starboard day tank manhole was leaking fuel into the aux space, saturating tank insulation.

During the annual inspection, the CG requested the vessel to preform certain PSTP tests as written within the procedures. Found some PSTP items were unable to be tested due to the vessel's system configuration. A review of the vessel's PSTP program shall take place specify noting items that are unable to be tested per the approved PSTPs. A list of the PSTPs that are unable to be preformed per the approved procedures shall be submitted to the CG for review. Additionally, the vessel shall submit records showing the remaining PSTPs for CY19 are completed or are noted with a description of why the test could not be preformed. 46 CFR 61.40-6

Vessel reported an issue with their engine cooling system. Both main diesel engines are operating at 30% load. Diagnose the issue and make appropriate repairs. Prove full load testing of the engines to the satisfaction of a CG marine inspector or ABS surveyor.

Make proper repairs to the #2 generator and prove proper operation to Class.

Vessel lost power while transiting in the Delaware River and the vessel is currently anchored at [name withheld] Anchorage. Provide tech report identifying the cause of the loss of power and prove proper operation of generating plant to the satisfaction of the vessel's authorized Class Society Surveyor and Coast Guard Marine Inspector prior to movement from [name withheld] Anchorage.

Prove proper operation of the watertight door switch door switch for both the ER escape, tank level and gallery level door per vessel PSTP 5.1. While on-board, the ships crew were not able to demonstrate the PSTP and the indication system appeared to have a fault.

Correct engine exhaust leak found on starboard main engine.

Prove the sliding type 3 watertight door meets the time requirement of between 20-40sec for closure starting from when the door begins to move. While on-board, CG inspectors observed a closing time of approx 12-15sec

Repair visual indicator for engine room watertight door.

Repair emergency steering station rudder angle indicator.

Address low pressure reading on Aux Machinery Space Sapphire bottle gauge.

Repair jacket water leak on port main engine. Monitor leak during operation of engine when at normal operating temperature.

The vessel's port main engine tripped while underway from an overspeed condition. Make main engine available for inspection by the attending USCG marine inspector.

The vessel's spare generator is not operating correctly. Vessel has permission to shift berths to Northeast Ship berth 6 with tug assist. Vessel has installed a temporary generator to the #3 generator engine. Vessel must demonstrate proper installation and operation to the satisfaction of the attending USCG MI.

A 1.5" - 2" hole was discovered on the overboard piping of the sea water cooling system outboard of the skin valve. Complete temporary repairs prior to movment. Submit statement from Master attesting to the seaworthiness of the vessel prior to movement. Submit plans for permanent repairs

NW01 Make satisfactory permanent repiars to the control power supply for the #2 steering gear control unit. Provide causal analsysi and repair method.

Overheating of electrical box resulted in electrical conduit melting.

Make the vessel available for quarterly underwater inspections of the temporary repair made to the rudder/nozzle system to the satisfaction of the attending MI.

Vessel has a fuel oil leak in the #1 void stbd side. Submit plan proposal to repair and identify use of piping.

Prove proper operation of start air (air receiver) relief valves.

Prove proper operation of fuel shut offs/quick closing valves.

Prove proper operation of port aft louver.

Provide 03- 2.5" fire hoses as required by 46 CFR 95.10-10

Vessel was found to have cracks in rudder system as noted in activity narritive.

Make satisfactory repairs to port engine and provide technical report. Vessel required to mainatain assist tug on scene.

Vessel has no copy of the design verification test procedure onboard and is unable to locate a copy. Vessel is required to provide a copy of the approved dvt procedure within 30 days. (46 CFR 61.40-1(c))

Vessel is required to have permanent piping from the storage tank to the hydraulic oil resevoirs. Currently, a hose is clamped via camlocks and is used as needed to refill the resevoirs. The vessel has 30 days to correct. (46 CFR 58.25-20(c)(2))

Vessel was unable to test the reverse power protection using the approved test procedure found in the revised periodic safety test procedures (PSTP). Using an alternate method, the reverse power protection was verified. Vessel is required to determine problem with the approved test procedure and notify OCMI of findings.

Prove starting air receivers meet the construction standards of 46 CFR 54.01-2

Provide starting air receivers with relief valves IAW 46 CFR 54.15

Stbd MDE shutdown on oil demister sensor. VSL not to operate stbd MDE until SAT test run witnessed by marine inspector and tech report received that SAT repairs have been made to stbd mde.
