Significant teel wastage identified in and around access cover bulkheads, frame flatbars and covers themselves for the access cover plates to the main deck exhaust/blower box stacks. 36 Remove the access cover plates (the 4th plate on the blower box stacks on port and starboard sides) due to corrosion on the exhaust cover plates. 37 Replace 2' wasted vertical flatbar identified in starboard exhaust space. 38 UT 18 x 18 main deckplate insert on the main deck due to indications of corrosion. 39 UT starboard side vehicle deckplating just aft of the superstructure in way of the bulwark gate due to indications of corrosion.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck.(b) The emergency lighting required by paragraph (a) of this section must automatically actuate upon failure of the main lighting system. If a vessel is not equipped with a single source of power for emergency lighting, it must have individual battery powered lights which:(1) Are automatically actuated upon loss of normal power;(2) Are not readily portable;(3) Are connected to an automatic battery charger; and(4) Have sufficient capacity for a minimum of 2 hours of continuous operation. Several emergency lighting of the battery operated type were found to stop working within 1 to 2 minutes during loss of power simulation. Also, several emergency"EXIT" signs were found inoperable. Verify all emergency lighting, and exit signs and prove proper operation to the satisfaction of a attending USCG Marine Inspector. 46 CFR 120.430. 17C.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings. All closure devices must be readily available and mounted in the vicinity of the vent. Both port and stbd ventilation vent manual covers were found with the rubber gasket not sealing adequate, and the securing butterfly nuts/fasteners wasted, not screwing to secure the cover. 46 CFR 119.465. Code 17C.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Port & stbd watertight door hatches were found not closing airtight upon testing, allowing the access of air in a Co2 machinery protected room, and not to be watertight. 46 CFR 115.840/ 46 CFR 118.410/ 46 CFR 171.124/ 17C.

Each dead cover must be designed so that it can be secured watertight. Port light window dead cover plate, located in portside, closer to the exit was found missing all 04 securing fasteners, nuts. 46 CFR 171.117. Code 17C.

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. One of the portside fire ventilation dampers failed to close upon automatic actuation of the CO2 system during testing, remaining stuck in open position. 46 CFR 118.410 (10)/ Code 17 Corrected on the spot Code 10C.

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Handles and securing devices must be permanently installed and not capable of being easily removed. Stbd side main deck aft door, leading to the cargo deck as found not opening properly, with opening handle getting stuck in close position. 46 CFR 116.500/ Code 17/ Corrected on the spot code 10C.

Each generator must have a nameplate attached to it containing the information required by Article 445 of the National Electrical Code (NEC) (National Fire Protection Association (NFPA) 70), and for a generator derated in accordance with paragraph (b)(2) of this section, the derated capacity. Port and Stbd generators does not have the nameplate attached. 6 CFR 120.320.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space, including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. An area of hull plating deterioration due to corrosion was found in both starboardside sideshell plating passed the doubler rings at the exhaust pipes outside of the vessel. This area shall be monitored until next dry dock (AUG2025) on which an NDT shall be conducted to determine the percentage of wasted. If determined to be wasted, then a crop and renew of the sideshell shall be conducted for next dry dock credit. 46 CFR 115.600(d). 705C For next dry dock credit.

Requirements #02 through #09 are objective evidence that a serious substandard conditions have not been proactively managed by the vessel's owners/operator, leading to a lack of safety culture on board the vessel.46 CFR 115.830

Each vessel must have an amplifier—type announcing system that will supplement the general emergency alarm. This system must provide for the transmission of orders end information throughout the vessel by means of microphones end oudspeakers connected through an amplifier. Public Announcement System was not functioning. 46 CFR 121.610(b) 46 CER 113.50—5

At each initial and subsequent inspecticn for certification the owner or managing operator shall be prepared to test the steering systems of the vessel to the extent necessary to determine that they are in suitable condition and fit for the service intended. Emergency steering pump 1 control and the emergency controls were found inoperable.46 CFR 115.814/COde30C.

A power ventilation system must be capable of being shut down from the pilot house. HVAC power ventilation shutdown control at the operating station was not working upon actuation. 46 CFR 116.600. Code30C.

An audible and a visible alarm must activate in the pilothouse upon (l)failure of the electric power to the control system of any steering gear; (2) Failure of that power to the power unit of any steering gear; or (3) occurrence of a low oil level in any oil reservoir of a hydraulic, power—operated steering—gear system. The following alarms were found inoperable: a. pump 1 audio alarm at operating station, b. audio alarm at the emergency steering controls in the steering room, c. LED indication light alarm for motor overload’.46 CFR 58.25—25 (d). Code30C.

The fire detection and alarm system must be so arranged and installed that the presence of a fire in any of the protected spaces will be automatically registered visibly and audibly in the pilothouse or fire control station. The visible notice must indicate the zone in which the alarm originated. Fire detection system was found not programed to identify the zones and locations upon activation of the smoke, heat, and manual call points; and does not differentiate between fire system and high bilge water alarms.46 CFR 76.27—5/Code30C.

A hatch exposed to the weather must be watertight. The following have been found not to be watertight due to faulty rubber gaskets seals: 1. Port and Starboard engine room access hatches, 2. port and starboard muffler rooms cargo deck access hatches. 3. Port and starboard deck access hatches. 46 CFR 116.1160(a)/Code 30C

Indication of the rudder angle must be provided both at the main steering station in the pilothouse and in the steering-gear compartment. The rudder-angle indicator must be independent of control systems for steering gear. Upon testing rudder angle indicators at the operating station, port and starboard bridge wings and emergency steering controls all had different readings with 5 to 7 degrees in difference. 46 CFR 58.25-25/Code 30C.

Connections and terminations to all conductors must retain the original electrical, and where necessary, fire-resisting properties of the cable. If twist-on type of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Multiple electrical cables were found with twist-on type connectors and dead-ending and not connected to any electrical equipment, some exposing the wires in the following locations: a. underneath the main center operating station. b. underneath port and starboard bridge wings c. Rudder angle gauge light stbd bridge wing hanging. 46 CFR 111.60-17/Code 17C

Each door used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. A door stbdside from passenger accommodation area 2nd deck was found with a broken door handle. 46 CFR 116.500(i)/Code16.

Each battery box must have a watertight lining to a height of at least 76 mm (3 inches) that for lead-acid batteries, the lining must be 1.6 mm (1/16 inch) thick lead or other material that is corrosion-resistant to the electrolyte of the battery, or for alkaline batteries, the lining must be 0.8 mm ( 1/32 inch) thick steel or other material that is corrosion-resistant to the electrolyte of the battery. All batteries underneath the operating station were found installed inside of a unapproved fabricated PVC tray box without any lining inside. 46 CFR 111.15-5/16C.

The master of the vessel shall ensure that: (a) Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; (b) The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR Part 80, whichever comes sooner; and (c) The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Last EPIRB logged testing was dated 28 November 2022, and EPIRB battery will be due for servicing in July 2024. 46CFR122.728/Code16C.

At each inspection for certification and at every other vessel inspection all hazardous situations must be corrected. Water intrusion was observed through the bridge center operating station, port and starboard bridge wings, leaking into and underneath the operating station and bridge wings electronics and electrical wiring. Evidence of evaporated water and salt found. 46 CFR 115.830/ Code16C.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. 06 portable fire extinguishers support brackets were found corroded and deteriorated, making it difficult to open them to release the extinguisher. 46 CFR 118.520/Code 16C.

The marine employer shall ensure that all individuals engaged or employed on board a vessel are fully indoctrinated in the requirements of this subpart, and that appropriate vessel personnel are trained as necessary in the practical applications of these requirements. Master failed to demonstrate testing required times to conduct testing for alcohol and drugs upon a marine casualty. Training shall be provided to all crews as to be an reoccurring situation, and provide evidence to USCG Marine Inspector. 46CFR4.06-01(e)/ Code 16C.

At each inspection managing operator shall be prepared to test the steering system of vessel and make them available for inspection to determine that they are in suitable condition and fit for the service intended. (46 CFR 115.814) The STBD side rudder shaft (post) observed with severe leakage while operating under full load.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Found that the 160.041 first aid kit onboard was missing 100 Aspirin Tables. 46 CFR 121.710

Designated Employee Representative (DER) An employee authorized by the employer to take immediate action(s) to remove employees from safety-sensitive duties, or cause employees to be removed from these covered duties, and to make required decisions in the testing and evaluation processes. The DER also receives test results and other communications for the employer, consistent with the requirements of this part. Service agents cannot act as DERs. The Master and all four deckhands did not know the name of the DER, there was no documentation onboard stating the name of the DER. 49 CFR 40.3

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. There was no EAP hotline number posted on the vessel. Crew did not know where to find the EAP hotline number. 46CFR16.401

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level. The bilge high level alarm for the mid-ballast void space was inoperable. 46 CFR 119.530(a)

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. The bow thruster room bilge dewatering piping was found holed which prevented the dewatering system from operating as intended.46 CFR 119.500(a)

The approved fire control plan must show the location of fire protection and emergency equipment. The current fire control plan shows 5 heat detectors located within the engine compartment. However, 3 of the 5 have been swapped for smoke detectors.46 CFR 116.530

Electrical equipment and installations must be suitable for roll, pitch, and vibration while the vessel is underway. There were 3 junction boxes in the starboard mid-ballast void (under A/C units) that were hanging by its wires and not properly supported.46 CFR 120.220(a)

Placards containing instructions for the donning and use of life jackets onboard the vessel must be posted in conspicuous places that are visible to the crew and passengers. The life jacket donning placards provided and posted throughout the vessel were for life jackets not available on the vessel. 46 CFR 122.516(a)

The owner must submit for approval a structural fire protection plan that addresses its details and components. The approved structural fire protection plan (P-594-13) contains inaccurate states all six exterior fire on decks 1 & 2 are aluminum weather-tight doors instead of Class A fire doors as required by the regulations. Submit to Sector San Juan for approval. 46 CFR 116.202(b)(2)

The approved fire control plan must show the location of fire protection and emergency equipment. The current fire control plan does not show 2 life jacket storage lockers that have been added on the main deck, port and starboard sides. Submit to Sector San Juan for approval.46 CFR 116.530

A framed chart or diagram shall be installed in the wheelhouse or control station adjacent to the detecting cabinet indicating the location of the various detecting zones and giving instructions for the operation, maintenance, and testing of the system. This chart, or a separate card or booklet to be kept near the chart, shall have tabulated spaces for the date and signature of the licensed officer of the vessel who shall witness or conduct the periodic tests. A fire detection system diagram or chart is missing next to the detection system.46 CFR 76.27-80 (6).

Nonmetallic flexible hoses must be in accordance with SAE J1942 (incorporated by reference; as per 46 CFR 56.01-0) and may be installed only in vital, lube oil and fuel systems, and power systems. Nonmetallic flexible hoses must have factory-assembled end fittings in compliance with SAE J1475 (incorporated by reference). The following nonmetallic flexible hoses were found in the engine room are not in compliance with SAE J1942: Aeroquip FC510-12; Continental XR16SC-12; Parker 302-301-16. These have been found installed in all four main diesel engines, two generator sets and all four transmissions. 46 CFR 119.445(a)(iii) and 46 CFR 56.60-25 (b)(1) and (b)(5). Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or sprays. All the battery tray covers had means for securing, but were not being used. The tray were lose, exposing the wiring and battery terminals. 46 CFR 120.350 (b)

A door in a bulkhead required to be A-60, A-30, or A-15 Class must be of hollow steel or equivalent material construction, solidly filled with approved structural insulation, and capable of meeting the requirements of an A-15 Class bulkhead. The following doors have a label affixed stating the doors are Class B instead of Class A as per DeanSteel Manufacturing Inc. Class A certification Letter dated 3-15-2022. Install correct labels. a. Deck 2 port and starboard double doors. b. Main Deck port and starboard doors to cargo deck. 46 CFR 116-435(c)

The spring in a relief valve in service for pressures up to and including 250 pounds per squire inch shall not be reset for any pressure more than 10% above or 10% below that which the relief valve is marked, the current relief valve resets at 40 psi, not with in the 10% of the MAWP which is 135 PSI. 30 Gallon air receiver, engine room.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. On 21 SEP 2019 the vessel captain reported an engine malfunction. 22 SEP 2019 it was reported to be the oil filter and old oil. Repairs to the engine must be approved by attending USCG Marine Inspector prior to carrying passengers. 46 CFR 115.700

On 23 August 2019 the vessel captain reported an engine malfunction. Repairs to the engine must be approved by attending USCG Marine Inspector prior to carrying passengers.

The main steering gear and rudder stock must be of adequate strength for and capable of steering the vessel at maximum ahead service speed, which must be demonstrated to the satisfaction of the cognizant Officer in Charge, Marine Inspection. The port rudder housing experienced structural damage: three of four threaded bolts holding the rudder in place are exposed and the rudder housing has weakened.

Indication of the rudder angle must be provided at the steering gear compartment. While conducting a local control steering gear test it was noted that the local rudder angle indicator was not showing the correct position of the rudder. As an alternative, there were painted markings in an overhead beam that showed rudder angle indicating degrees in 5 degree increments which were marked incorrectly.

At every vessel inspection, all unsafe practices and hazardous situations must be corrected. During a test of the main steering gear it was noted that the starboard side rudder post was leaking water into the steering gear compartment. Prior to operating, the vessel master(s) must ensure that the temporary repairs conducted are satisfactory and water intrusion is eliminated or substantially minimized.

Immediately after addressing resultant safety concerns, the owner, agent, master, or person in charge of a vessel involved in a steering system failure that reduces the vessel's maneuverability shall make a notification to the cognizant Sector office. USCG marine inspectors and marine investigators learned through social media and news outlets (later confirmed through a vessel representative) that the vessel CAYO LARGO suffered a marine casualty related to a malfunction of the steering system's port side rudder post. Vessel operator must prove proper operation of the vessel's main steering gear system to the satisfaction of the attending marine inspector prior to the carriage of passengers or cargo.

Propulsion machinery must be suitable in type and design for propulsion requirements if the hull in which it is installed and capable of operation at a constant marine load under such requirements without exceeding its designed limitations. NO.3 Main Diesel Engine (MDE) unexpectedly shut down and was unable to be restarted during a mooring evolution. Provide causative factors and correction actions for loss of propulsion. Prove proper operation of the NO.3 MDE prior to carrying passengers, to the satisfaction of the attending Marine Inspector.

Repair / Replace missing closure devices on fire ventilation closures in way of fwd bulkhead of Car Cargo Area.

Remove all bilge oily water mixture.

Replace packing on all four shafts within seven days.

Make permanent repairs to port rudder post leakage and ensure the steering room is monitored and water is pumped out. Make vessel available for inspection NLT 28 Dec 2009.

Back up control power for steering system is inoperable.Repair and test for proper operation

Rudder angle indicator on main steering stand not working properly with main steering lever.Troubleshoot,repair and test.

Steering system no.2 switch on bridge not allowing controls at steering joysticks. troubleshoot, repair and test for proper operation.

Engines vibrate at higher rpms.Troubleshoot and repair and test for proper operation.

Lagging on no.3 lagging causing smoke. Replace.

Install a B-V Fire Extinguisher prior to carrying cargo. Update the fire control plan.

Valves marked with non-durable lables. Provide permanent labels for the bilge manifold in the engine room. Provide means of marking and ensuring one of the three bilge suction valves in the steering gear room is always open.

Port side aft cargo deck hatch gasket painted. Remove paint from the gasket.

Guard for the light fixture in the steering gear room is broken. Repair light fixture.

No Senior Deck Hand list posted on the bridge, see NVIC 1-91 for guidance. Post Senior Deck Hand list prior to carrying more than 149 passengers.

Provide current U.S. Coast Pilot [name withheld] List, Tide Tables and Tidle current tables.

Prior to carrying passengers conduct new to zone inspection

Prior to carrying passengers prove enrolments in an approved drug and alcohol program

Prior to carrying passengers conduct underway man overboard and fire drills

Prior to carrying passengers provide appropriate log books, charts, navigation pubs, and waste management plan to the satisfaction of the attending marine inspector

Provide a non tank vessel response plan by August 15, 2008
