FMT 3206 has an inset approximately 2 feet wide and 7 inches deep on the headlog due to bridge allision. Vessel is permitted to transit to Mobile, AL. Vessel must have a USCG inspector attend for a damage survey prior to movement from Mobile, AL facility or fleeting area.

Resolution: Conducted damage survey and noted that vessel was fit to continue as intended under conditions provided. Issued Permit to Proceed to discharage cargo and transit to FMT Shipyard at [name withheld] in New Orleans for permanent repairs.

While loaded with crude oil, FMT 3206 allided with a bridge and suffered damage to the port bow headlog. Loaded barge must proceed directly to Mobile, AL and make the affected area available for a damage survey. During the transit, soundings of the voids and cargo tanks shall be taken to ensure no cargo migration or water ingress. If migration is detected, Sector Mobile Command Center must be notified.

Resolution: Pittings in all cargo tanks bottom were repaired by pad welding. Hydrostatically tested all cargo tanks. Found no leaks and structural integrity was satisfactory.
