An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. This is not meant to restrict access to towing equipment such as winches, drums, towing gear or steering compartment equipment necessary for the operation of the vessel.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Vessel Port MDE did not start. Fix issue and notify local OCMI.

There must be an installed or portable bilge pump for emergency dewatering. Bilge system was not operating properly and light indicator for the engine room was inoperable. Fix issues and notify local OCMI.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. 05 open breaker slots on the breaker panel on the bridge. Properly enclose open breakers and notify local OCMI.

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crew members are capable of performing the duties expected of them during emergencies. This includes abandoning the vessel, recovering persons from the water, responding to onboard fires and flooding, or responding to other threats to life, property, or the environment. Vessel was unable to get underway to conduct drills. Once vessel is ready for drills notify local OCMI.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. MI found dead-end wires in the wheel house, under the helm. Update electrical plan and notify local OCMI once corrected.

Vessels operated beyond the 20 nautical mile limitation specified in paragraph (a)(1) of this section, but not more than 100 nautical miles from the nearest land, must be equipped with a MF-DSC frequency transmitter meeting the requirements of § 80.1101(c)(3) and capable of transmitting J3E emission and a receiver capable of reception of J3E emission within the band 1710 to 2850 kHz, in addition to the VHF-DSC radiotelephone installation required by paragraph (a)(1) of this section. Vessel is missing the required MF-DSC frequency transmitter. Vessel is restricted to operating less than 20 nautical miles until MF-DSC frequency transmitter is installed in vessel. Notify local OCMI once corrected. 47 CFR 80.905

Vessel's pre engineered fire extiguishing system shall be serviced annually.

Operating Station installed VHF-FM RDO not able to produce DSC functions during operational testing. Whenever radiotelephone capability is required by this Act, a vessel's radiotelephone equipment shall be maintained in effective operating condition.

Forward storage room bulkhead penetration not properly sealed. Remove or cap penetration.
