Vessel master/rep made sailing short notification for the position of AB from Antwerp, BE to Bremerhaven, DE.

Whenever a vessel is deprived of the service of a member of its complement, and the master or person in charge is unable to find appropriate credentialed personnel to man the vessel IAW the COI minimum manning, a report of sailing short must be filed with the OCMI having cognizance for inspection in the area in which the vessel is operating, or the OCMI within whose jurisdiction the voyage is completed. Certificate of Inspection amended for 02 additional AB's (or OS's w/ STCW RFPNW) and requiring 04 persons on board with fast rescue boat endorsements. IMO crew list received. Vessel is now sailing short 01 AB and 02 OS's w/ STCW RFPNW endorsements. Bring vessel to full manning compliment within 60 days.

Cargo ships shall carry at least one rescue boat complying with the LSA code. Properly reattach automatic bilge pump sensor to within bilge.

Tools necessary for fire hose fittings shall be kept near each hydrant. Replace inadequate spanner wrenches at accommodation space fire stations. SOLAS 74am ('09ConsEd) II-2/10.2.3.1.1

Ships shall be designed, constructed and maintained to class rules. Two cracks on welds identified on forward end of house superstructure at web flanges P&S sides. Make appropriate repairs/monitoring to LR Surveyor satisfaction.

Information shall be posted in crew berthing areas and crew washing places for sexual assault and sexual harassment prevention policies and resources. Master key control logs shall be utilized for all master keys and duplicate cabin keys to single occupancy cabins. Update company SMS, logs and posted information. The subject vessel currently only has 2 oilers onboard, less than the number of oilers (3) required by the COI. Return to COI required crew compliment in no later than 30 days' time. UPDATE 11/20/23: On today's date, the 835 due date has been extended until the vessel's return to Baltimore on/about 12/2/23. The vessel may not depart from Baltimore until it is fully manned in accordance with the COI requirements.

Vessel is sailing short 1x Bosun (AB).

Vessel reports that there is currently only one (1) oiler onboard; three (3) are required by the COI minimum manning. UPDATE 27 OCTOBER 2022: One additional oiler has on-signed in Davisville, RI. There are currently two (2) oilers onboard, of the three (3) required by the COI minimum safe manning. Return to full crew compliment no later than the listed due date.

Vessel has been sailing short 1 AB required by the COI since 16 August 2022, and recently also sailed short 1 AB from 19 July - 06 August 2022. Return to COI required crew compliment no later than first U.S. port call after returning from the European port rotation, reported to be Baltimore, MD, on or about 20 September 2022.

Vessel reports sailing short one AB; return to COI required crew compliment no later than departure from Antwerp port call, reported to be on/about 01 September 2022.

Main machinery essential for the propulsion of the ship shall be provided with effective means for operation and control. Vessel self-reported that their remote control for engine speed was limiting RPMs for main engine just above slow speed. Make permanent repairs to pilot house remote engine controls.

This position must be filled prior to the vessel's completion of its U.S. port rotation, which is reported to be in Davisville, RI on/about 14 August 2022.

One of three required oiler positions, first vacant on 07 May 2022, remains unfilled at this time; return to full crew complement as required under the COI. *Based on formal reconsideration by the Sector Maryland-NCR OCMI on 03 June 2022, the new deficiency due date is upon the vessel's return to the U.S. from its European port rotation. The first U.S. port upon return from Europe is reported to be Newport News, VA on or about 11 July 2022.

Vessel required to meet 46CFR61.10-5(g) requirement to Hydro test bulk storage tank containing Low Psi CO2 for use aboard a vessel as a fire-extinguishing agent shall be subjected to a hydro test of 1 1/2 times the MAWP in the tenth year of installation and at ten-year intervals thereafter. Vessel required to conduct alternative inspection of Low psi CO2 tanks at next port Bremerhaven and coordinate with Activities Europe for attendance and inspection IAW approved alternative inspection plan. 705- due in Bremerhaven

All vessels shall be provided with a bilge pumping plant capable of pumping from any watertight compartment. Found aft/port bilge well isolation valve inoperable. Item was issued a Condition of Class from Lloyds surveyor. Lloyds activity #2048704

Vessel to obtain EIAPP certs from EPA for all diesel engines. Vessel has submitted through EPA request and are awaiting new EIAPP certs. Certs are to be on-board no later than 09 Dec 2019.

Each fire drill shall include checking of fireman's outfit and other personal rescue equipment. During fire drill, observed fireman in fireman's outfit without axe and lifeline per regulation. 74SOLAS14 III 19.3.4.2.3

01199 - THE MASTER, OWNER, OPERATOR OR PERSON IN CHARGE OF A VESSEL SUBJECT TO THIS SUBPART MUST SUBMIT A BALLAST WATER REPORT TO THE NATIONAL BALLAST INFORMATION CLEARINGHOUSE (NBIC) NO LATER THAN 6 HOURS AFTER ARRIVAL AT THE PORT OR PLACE OF DESTINATION. VESSEL HAS NOT SUBMITTED THEIR BALLAST WATER REPORTS TO THE NBIC.
