An IBA must be serviced at a facility speciffically approved by the Commandant and in accordance with servicing procedures... Vessel's IBA is currently being serviced so vessel does not have IBA or valid servicing report onboard. Vessel restricted to operations not to exceed 1 NM from shore. 46 CFR 185.730 (a)

A vessel must carry one floating waterlight. Ring buoy waterlight inoperable. Limited to daylight operations. 46 CFR 180.70

Portable fire extinguishers must be located so that they are clearly visible and readily accessible. Engine room and forward galley fire extinguishers not properly mounted. 46 CFR 181.520

The part of the exhaust between the point of cooling water injection and engine manifold must be insulated. Port and stbd main diesel engines missing exhaust insulation before water injection point. 46 CFR 182.425(b)(3)

A vessel must be provided with bilge pumps IAW Table 182.520(a), fixed power bilge pump. Pump mount is broken and unable to operate. Temporary repair completed, and authorized until 20 May 2023. 46CFR 182.520(a)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart. Cable and wire in the engine room is supported by zipties and shall be changed to metal supports. 46CFR183.340(b)(4)

Each vessel must be arranged so that the electrical loads can be energized from two sources of electricity. Vessel's battery charger is currently in the process of being rebuilt. Install battery charger to the satisfaction of the Marine Inspector. 46CFR183.310(a)(1)

The name of the vessel must be marked on some clearly visible exterior part of the port and stbd bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel did not have any of the markings.

A hatch cover must have securing devices and other devices to prevent its loss. Hatch on focsle is missing dogs and gasket.

Individual bilge suction lines must be led to a central control point or manifold and provided with a stop valve at manifold and provided with a check valve at some accessible point in the bilge line. Bilge suction line in E/R is missing a check valve.

Each ladder used as a means of escape must be mounted atleast 7 inches from the nearest permanent object in the back of the ladder. Ladder in lower aft accomodation space is not permanently mounted or a sufficient ladder.

Vessel must comply with the type II subdivision and damage stability requirements of 171.070. Vessel has several holes in the chain locker bulkhead that need filled.

The inspection and maintenance of the vessel’s survival craft shall be logged or otherwise documented for review by the Coast Guard upon request. The vessel did not have a lifesaving maintenance log available for review. The log was created and the inspection was conducted during the annual examination. 46 CFR 185.702 (d)

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The crew training log was not available for review. The log was created and the training was conducted during the annual examination. 46 CFR 185.420 (c)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. The wire connections for the electric submersible bilge pump in the Engine Room were not of the approved type and were enclosed with residential duct tape. Replace the wire connectors with an approved type IAW 46 CFR 183.340.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The hydrostatic release unit for the vessel’s IBA was expired. The owner replaced the hydrostatic release unit on the spot in the presence of the Marine Inspector. 46 CFR 185.740 (b)

46 CFR 177.01-5 - The scantlings, material, fastenings, workmanship, and arrangement of each vessel are such as to provide for safe operation in accordance with the terms of its certificate of inspection. Multiple rails and stanchions on the main deck port side and 2nd deck port and stbd sides by the operating station have broken loose and can no longer provide support to passengers. Passengers must be prohibited from access to port side main deck until further notice. Vessel must submit repair proposal to OCMI for approval.

46CFR 183.05-50 - Multiple wiring installations throughout the engine room and lazarette are not in accordance with 46 CFR 183.340.Remove all dead end wires, unused wire runs, and unapproved connections.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 2 inches high: "Emergency Exit, Keep Clear". Forward and aft emergency exit were missing label.

On a vessel on whcih the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessels stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Vessels placard was incorrect in the number of passengers allowed on the upper deck and posted in the wrong location. 46 CFR 185.602(g)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Ring buoy on the upper deck was missing retroreflective tape.

REPAIRS OR ALTERATIONS TO MACHINERY OR EQUIPMENT THAT AFFECT THE SAFETY OF THE VESSEL MUST NOT BE MADE WITHOUT THE APPROVAL OF THE COGNIZANT [name withheld] OF INCORRECT WIRING INSTALLATIONS OF ELECTRICAL WIRING SYSTEMS FOUND THROUGHOUT ENGINE ROOM INCLUDING WIRES RUNNING THROUGH BILGES, IMPROPER LABELING OF CIRCUIT BREAKERS, MANY INSTANCES OF WIRES JOINED WITH SCREW TYPE CONNECTORS EXPOSED TO THE ELEMENTS. FOUND INSTANCE OF LIVE WIRE IN BILGE, INSTALLATION OF CIRCUIT FOR EMACERATOR IMPROPERLY RAN AND NO OVERCURRENT PROTECTION. PROVIDE DOCUMETATION OF APPROVED ELECTRICAL INSTALLATIONS. ALL UNAPPROVED ELECTRICAL INSTALLATIONS MUST BE REMOVED OR YOU MUST SUBMIT PLANS SHOWING COMPLIANCE FOR APPROVAL. ENSURE ORIGINAL INSTALLATIONS ARE IAW 46 CFR 183 OLD T REGULATIONS. APPROVED INSTALLATIONS INSTALLED AFTER MARCH 11, 1996 MUST BE INSTALLED IAW 46 CFR 183 NEW T REGULATIONS. PROVIDE DOCUMENTATION FROM MARINE ELECTRICIAN THAT ELECTRICAL SYSTEMS ARE SAFE AND INSTALLED IAW ELECTRICAL STANDARDS LISTED IN THE CFR OR OTHER EQUIVILANT APPROVED STANDARD. 46 CFR 176.700

A vessel on a lakes, bays and sounds route must carry three red and three orange flares or have authorized substitutions. Flares onboard are expired. 46 CFR 180.70

A vessel must have one or more life ring buoys. Life Ring Buoy has reached end of service life. 46 CFR 180.68

Fire extinguishers must be installed IAW industry standards. Fire extinguisher on flybridge is not installed with approved bracket. 46 CFR 181.120

The construction and arrangement of a vessel must allow for safe operation. Main Deck forward slowly leaking below deck. Wood decking is deteriorating. Electrical lighting cover contained water. No passengers are allowed in area of concern. Forward electrical tagged out. 46 CFR 177.100

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Hand held bilge pump has reached the end of its service life. 46 CFR 182.500

Emergency Fuel Shutoff rod is connected with plastic wire tie. Plastic Wire Tie broke during test of operation. 46 CFR 182.15-40 (b) (3)

Pressure Relief valve on air compressor is not labled with MAWP. 46 CFR 182.01-5

Battery terminal connections are not connected with permanent connectors. 46 CFR 183.05-20 (c)

Multiple wiring installations throughout Engineroom and Lazarette are not made within junction boxes and installed IAW 46 CFR 183.340. 46 CFR 183.05-50.

Forward hatch does not seal and has a broken dog.

Vessel has plastic trash containers throughout the vessel in longe areas below deck. 46 CFR 177.30-7

Local control box for Engine is broken off mount and secured with plastic wire ties. Electronic Gear Interface Module grounding wire is not connected. 46 CFR 182.01-5
