The following alarms must be provided: (1) Main engine low lubricating oil pressure;(2) Main engine high cooling water temperature; (3) Auxiliary generator engine low lubricating oil pressure; (4) Auxiliary generator engine high cooling water temper. Alarms must be visible and audible at each operating station. Master could not identify any alarms (including summary alarm) on the bridge. Provide alarms as required.

Properly dispose of oily waste in bilge. b. The dryer vent is venting lint into the engine room space posing a potential fire hazard. Redirect dryer vent to an acceptable location outside the engine room that does not pose a fire hazard. c. Combustible material (cardboard boxes) are being stowed up against ship service generator exhaust piping. Properly stow combustible material to minimize fire hazards. d. Ships main engines and ship service generators have excessive oil leaks. Make repairs to minimize oil leaks and fire hazards.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217 (incorporated by reference, see §?136.112 of this subchapter). Detection systems or individual detectors must be kept operational at all times when the crew is onboard the towing vessel. The individual battery operated detectors were missing batteries and no replacement batteries were onboard. Prove proper operation of smoke detector in galley and berthing space.

All electrical connections leading into and out of engine room bulkheads is not sealed properly. Engine room overhead wiring leads into pilothouse with moisture and oil coming through it. Properly seal all wire runs into engine room.

Renew closure hardware to ensure all electrical connections are protected. c. Washer/dryer outlet box behind washer has broken housing exposing internal wiring. Renew outlet box. d. Electrical box under generator missing cover and exposed wiring not properly connected is protruding from housing. Make proper electrical connection within housing and renew housing cover.

The following conditions exist: a. there are several pieces of installed equipment that are plugged into extension cords and splitters (daisy chained) to include: gang plug in engine room overhead for installed automatic overboard bilge pump, sewage aerator, and sewage discharge pump; refrigerator; 6 plug gang boxes in both galley 110v outlets; washer/dryer; freezer. Remove extension cords and properly wire all installed equipment in junction boxes. b. The general alarm box on the bridge is wired without properly securing connections nor is the connection in a junction box (wires are exposed). Properly wire general alarm box.

Each towing vessel must be operated in compliance with: Parts 151, 155, and 156, of 33 CFR, as applicable. Vessel has a secondary bilge pump piped directly overboard through the hull in automatic mode with a float switch. Pump is not under manual control. Due to the amount of gray water entering the engine room bilge, it is reasonable to suspect that the pump is pumping (automatically) engine room bilge water that may contain oil or oil residue over the side into the navigable waterway, not under emergency conditions. Remove or secure pump to prevent the unintentional discharge of bilge oily water over the side (except under emergency conditions).
