Epoxy soft patch/temporary repair found on the # 1 and #2 fire/general service pump flange to the suction pipe; crew stated flanges were holed/leaking and temporally repaired it approximately 2 months ago. No evidence provided of non-conformity report submitted to company. Address to class/flag representative satisfaction.

Resolved 3 April 2009
Resolution: The temporary repaired pipes on suction side of no.1 and no. 2 fire and gs pump were replaced with brand new pipes. Such repaired pipes were tested under normal operating condition with pressure 5MPa and found satisfactory.
