Cable runs from Engine Room to Lazarette did not have adequate Fire Stop. Ensure all electrical wires between spaces have adequate fire protection installed prior to credit Dry Dock.

46 CFR 177.600(a) 705C - 1 week An enclosed or partially enclosed space within a vessel must be adequately ventilated in a manner suitable for the purpose of the space. MI identified sunlight peering through the bottom on port and stbd sides in E/R. Ensure ventilation closures meet the intention w/i 7 days.

46 CFR 177.100 50c The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull...MI identified several areas of pitting in void 2. Address the pitting in void 2 and revisit for further evaluation at next DryDock.

Owner/Operator must provide satisfactory evidence of the required servicing of fire protection systems to the Marine Inspector. Vessel's pre-engineered fixed fire extinguishing system (Fireboy) and portable fire extinguishers did not have current annual servicing tags or evidence of certification. 46 CFR 176.810(b)

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: An audible alarm that sounds upon discharge at the operating station. Vessel did not have an audible alarm indicating deployment of the pre-engineered fixed fire extinguishing system. 46 CFR 181.420(b)(2)

A vessel on a Limited Coastwise route must carry six hand red and six hand orange smoke distress signals approved in accordance with Subchapter Q of this chapter. Nine of the vessel's required flares were expired. 46 CFR 180.68(a)

Each vessel that operates beyond 3 miles from the coastline must have on board an EPIRB installed to automatically float free and activate. Hydrostatic release unit was not marked with an expiration date at the time of install and there was no evidence present to determine how long it had been in service. Replace hydrostatic release unit or prove date of install within past two years prior to conducting operations outside 3 miles from shore. 46 CFR 180.64

An adult life jacket must be provided for each person carried on board a vessel. MIs found 30 life jackets to be deteriorated rendering them unserviceable. Maximum passenger count is reduced to 75. 46CFR180.71(a)

Connections must be made to battery terminals with permanent type connectors. MIs observed multiple battery terminal connections to the three battery banks with out permanent type connections installed. 46CFR183.350(d)

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 C (150 F), must be suitably insulated where necessary to prevent injuries. MIs found the insulation missing around each of the turbos on the main engines. 46CFR177.970

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. When testing the closing of the starboard side supply duct, MIs observed one louver to not fully close. 46CFR182.465(h)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. MIs observed during the man overboard drill it was extremely difficult for the deck hand to bring the simulated person in the water back on to the vessel by use of the rescue platform alone. Deckhand required the use of a "passenger" to assist in lifting the individual from the water and onto the vessel, putting the passenger at risk of falling overboard or injury. Vessel must revise their man overboard emergency procedures to be able to safely retrieve a person from the water without additional help from passengers or having the master leave the operating station. 46CFR176.808(g)

A vessel must comply with the applicable requirements for any Electronic Position Indicating Radiobeacon (EPIRB) installations, as set forth in 47 CFR part 80. To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. Vessel's EPIRB is not registered. Vessel route is restricted to lakes, bays, and sounds until rectified. 46 CFR 184.502 47 CFR 80.1061(f)

A vessel must be fitted with a Federal Communications Commission (FCC) type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station, if the vessel is self-propelled, has a limited coastwise route, and carries more than 49 passengers. Vessel radar is not operational. Vessel route is restricted to lakes, bays, and sounds until rectified. 46 CFR 184.404(a)

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Except as noted in paragraph (c) of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel's current FCC Station License is expired. 46 CFR 184.502 47 CFR 80.13(a)

The Fuel shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Valves did not close completely from the remote location. 46 CFR 182.455(b)(4)

At least one ring life buoy must be fitted with a lifeline. each ring buoy must have synthetic or of a type certified to be resistant to deterioration from ultraviolet light. The ring buoy grab line was broken.

The boundaries that separate machinery spaces from control spaces must be lined with noncombustible panels or insulation approved in accordance with 164.009 in subchapter Q of this chapter. MI noticed 1 foot by 10 inches opening on port and starboard overhead deck from engine room to helm station support arms.

Cables and wiring systems must be installed with metal supports spaced not more then 24 inches apart and in such manner as to avoid chafing and other damage. MI noticed bunched wires leading up to control station from engine room without proper support.

Vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel did not complete annual inspection.

Examination of all through hull fittings including sea values must be opened for examination. The vessels sea valves were seized and not able to be examined. Replacement of valves are on order and will be installed to perform as designed.

During each annual inspection, the vessel must have ready for inspection all lifesaving equipment to include the vessel's life floats. The vessel is currently not operating, and did not have the required lifeloats stored onboard. MI must verify installation and presence of lifefloats before vessel can carry passengers for hire.

Vsl experienced LOP. Demonstrate proper operation of both main engines prior to carrying passengers.

The fuel shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 12 inches into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high. The remote fuel shutoff located within the fuel tank space is not shielded and is only accessible through the hatch. The location of the controls also needs to be labeled.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct operational tests of the means provided for pumping bilges. Potential oil in the bilge well and bilge piping in the engine room and void #3 posed a pollution risk, preventing operational test of these spaces during inspection.

Each life float must be fitted with a light. The most-inboard light for the life floats does not activate. Vessel may not operate at night or when visibility is less than 1 NM, until the light is fixed.

An adult life jacket must be carried for each personon board a vessel. 03 life jackets were missing.

On a vessel at least 26 feet in length, a visual and audible high level alarm must be provided at the operating staiton to indicate a high water level in a machinery space bilge. Te alarm for the engine room bilge level was found non-functional.

Vsl used and could not recover anchor. Provide sufficient equipment for anchoring prior to carrying passengers.

Starboard engine exhaust, flexible joint that connects two pipes in the exhaust line was missing an upper clamp and the lower clamp was found ineffective due to signs of soot and salt build up.

Lifefloats were stacked four high without adequate spacers in between.

Strainer for forward bilge line was missing.
