A fixed fire pump must be capable of: Delivering water simultaneously from the two highest hydrants, or from both branches of the fitting if the highest hydrant has a Siamese fitting, at a pitot-tube pressure of at least 344 kilopascals (kPa) (50 pounds per square inch (psi)), and a flow rate of at least 300 liters per minute (lpm) (80 gallons per minute (gpm). Inspector discovered that the fire pump was only outputting 40 psi pressure. 46 CFR 142.325(a)(1)

A towing vessel must carry lifebuoys as follows: be capable of being rapidly cast loose; not be permanently secured to the vessel in any way; marked in block capital letters with name of vessel, lifebuoys must have the following attachments. Floating electric water light approved under 161.010 or 161.110. Vessels Stbd side light did not have a lanyard with a corrosion resistant clip and was tied to lifebuoy. Wrong battery was inside the light. Vessels Port side life buoy was tied to the vessel and light was not secured to the ring by any means. The Port side life buoy setup also made it impossible to cast free lifebuoy based on the set up. Vessels name was faded on both ring buoys. Repair all items listed above prior to movement to the satisfaction of the Marine Inspector.

Each escape hatch and emergency exit used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. Marine Inspector observed forward engine room emergency escape hatch was labeled "Confined Space", properly label prior to operation.

Found no on-board procedures for testing fire detectors and crew was not familiar with test methods. Provide on-board fire detector test procedures and ensure crew is properly trained on test methods to the satisfaction of the attending USCG MI.
