Where davit arms are recovered by power, safety devices shall be fitted which will automatically cut off the power before the davit arms reach the stops in order to prevent over stressing the falls or davits, unless the motor is designed to prevent such over stressing. The limit switch did not function properly.

The tank overfill alarm must activate a visual and audilble alarm so that ot can be seen and heard on the vessel where cargo transfer is controlled and in the cargo deck area, the vessel's overfill alarm vudual indicator on deck is inoperable.
