The American Enterprise offloaded sludge from the bilge oil tank to a tank ship at sea. The owner/operator shall provide oil record book training to the chief engineer.

The chief engineer admitted to using the maximum throughput value for the amount processed through the oily water separator on all previous entries in the oil record book prior to July 1, 2016. Furthermore, the crew routinely transfers fuel over the top of the fish well without using a fixed connection in accordance with the fuel transfer regulations in 33 CFR 155 and 156. Provide a fixed connection to transfer fuel to the fish wells and provide transfer procedures that reflect operations.
