Except as otherwise provided in this section, on a documented vessel, each unlicensed seafarer (seaman) must be: a citizen of the US, an alien lawfully admitted to the US for permanent residence; or a foreign national who is enrolled in the USMMA. Marine Inspector was notified of 01 non-US resident employed on vessel. 17-Vessel is may complete operations and return to Port Fourchon to offload unauthorized non-US citizen crew member. Vessel must comply with 46 USC 8103 prior to movement.

On vessels required by § 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source:Lighting, ... for passageways, stairways, and escape trunks in ... public spaces, [and] machinery spaces, ... sufficient to allow passengers and crew to find their way to open decks and to survival craft, muster stations, and embarkation stations with all watertight doors and fire doors closed. Emergency lighting in the rudder room, bow thruster room, and galley provided insufficient illumination.

There shall be at least two means of escape from all general areas accessible to the passengers, if carried, or where the crew may be quartered or normally employed. The emergency escape door from the engine room needs to be adjusted to open and close properly.

Oil piping drains, strainers, and other equipment subject to normal oil leakage must be fitted with drip pans or other means to prevent oil draining into the bilge. The fuel filters on the main engines, generator prime movers, and bow thruster engine were missing drip trays.

Amendments to a Vessel Security Plan that are approved by the Marine Safety Center (MSC) may be initiated by: The Coast Guard upon a determination that an amendment is needed to maintain the vessel's security. The company owner or operator will have at least 60 days to submit its proposed amendments. The VSP on-board the vessel was not amended for the last two changes in CSO. Ensure the new CSO has appropriate and documented training/experience for the position. Submit an amendment to MSC and ensure appropriate training of the CSO in 60 days.

When alteration or repair of boilers, pressure vessels, machinery, safety valves or piping systems becomes necessary, the work must be done under the cognizance of the Officer in Charge Marine Inspection. The Calcium Chloride piping was temporarily soft patch repaired, complete final weld repair to the satisfaction of the [name withheld] repair prior to carriage of Calcium Chloride or other cargo using its transfer system.

Sector NOLA was notified of an allision with platform VK 817A which damaged pilot house, bow and stbd side shell. Vessel to conduct repairs of affected areas to the satisfaction of the attending MI. 46CFR91.30-1(a)

Each rescue boat must be stripped, cleaned, and thoroughly inspected and repaired as needed at least once a year. This procedure includes emptying and cleaning each fuel tank and refilling it with fresh fuel. Vessel could not provide documentation of annual rescue boat servicing. 46 CFR 131.575(a) (L service) 46CFR90.27-1 46CFR199.190(f)(2) (I service)

Each OSV must carry a EPIRB meeting the requirements of 47 CFR part 80. Vessel's EPIRB battery was expired. 46CFR133.60(a)

Each refrigerated space that is accessible to the vessel's personnel and that can be locked from the outside so that it cannot be opened from the inside, must have an audible alarm system that can be operated from within the refrigerated space. Vessel has lockable refrigerated spaces without audible alarms. 46 CFR 113.45(a)

Owners and operators of applicable vessels must establish a master key control system and related provisions for use of master keys, and record keeping. Vessel does not currently have a master key control system. 46 USC 3106(a)(1) CVC Policy Letter 23 46 USC 3106

Each davit, winch, and fall must be thoroughly inspected at least once a year and repaired as needed. Vessel could not provide documentation of annual davit servicing. 46 CFR 131.575(b) (L service) 46CFR90.27-1 46CFR199.190(f)(2) (I service)

Each vessel must comply with 47 CFR part 80 as applicable. Vessel did not have valid FCC Bridge to Bridge or Radio-safety telephone certs. Third party attended and issued both during the inspection. 46 CFR 130.210

Proper servicing of hydrostatic release unit shall be conducted. Hydrostatic release on epirb was expired. Replace.

Life buoys and associated equipment shall be of proper conditions. Float light batteries expired. Replace.

Watertight doors must be watertight. FWD engine room door not adjusted properly.

Each fire station must have a fire hose. Fire hose was leaking. Replace.

Rotating machinery must have a guard. STBD generator alternator cover had gaps in cover.

Visible and audible alarms must be installed in the pilot house for high bilge level. Rudder room bilge alarm was showing "spare" when activated.

Vessel discharged diesel during fueling operation due to faulty valve. Valve must be replaced prior to transfering fuel or transfer procedures must be updated.

Lifesaving appliances and arrangements are subject to tests and inspections described in paragraph (b) of this section during OSV boarding to ensure that the appliances and arrangements comply with applicable requirements, are in satisfactory condition, and remain fit for service. During the emergency test of the rescue boat davit the system was found to be overfilled with hydraulic fluid and releasing hydraulic fluid out on deck through the vent. 46 CFR 133.45 (C)

Each OSV must carry at least one rescue boat. Each rescue boat must be approved under approval series 160.156 and equipped as specified in table 133.175 of this part. The rescue boat is not equipped with the self-righting lines. 46 CFR 133.135 (a)

Each OSV must have posters or signs displayed in the vicinity of each survival craft and the survival craft's launching controls. Emergency instructions for the rescue boat davit were not posted. 46 CFR 133.90

At least two lifebuoys, each with attached self-activating smoke signals, must be stowed where they can be quickly released from the navigating bridge and should, when released, fall directly into the water without striking any part of the vessel. The life buoys installed onboard the bridge wings were not rated for “quick release”. 46 CFR 199.70(v)

Each lifebuoy must be capable of being rapidly cast loose. Life buoys on the 01 deck and main deck were not ready for service. 46 CFR 199.70(a)(1)

Each lifejacket must have a lifejacket light approved under approval series 161.112 or 161.012 securely attached to the front shoulder area of the lifejacket. 02 lifejackets in the staterooms had inoperable lights. 46 CFR 133.70(b)(4)

The marine employer must have a sufficient number of urine-specimen collection and shipping kits meeting the requirements of 49 CFR part 40 that are readily accessible for use following SMIs. The specimen collection and shipping kits need not be carried on board each vessel if obtaining the kits and collecting the specimen can be completed within 32 hours from the time of the occurrence of the SMI. The vessel is certified for oceans routes and is not supplied with specimen collection kits or collector training. 46 CFR 4.06-15

The installation of all systems of an electrical engineering or interior communication nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. The means of communication and calling must be a reliable means of voice communication and must be independent of the vessel's electrical system. MI observed that the ship’s internal means of communication via installed telephones was not connected to emergency power. 46 CFR 96.05-1(a) 46 CFR 113.30-3(b)

The installation of all systems of an electrical engineering or interior communication nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. Each generator prime mover must have an overspeed device that is independent of the normal operating governor and adjusted so that the speed cannot exceed the maximum rated speed by more than 15 percent. MI observed that the vessel did not have records onboard of the generator prime mover overspeed device having been tested. Provide technician report attesting to proper functionality. 46 CFR 96.05-1(a) 46 CFR 111.12-1(b)

Each EPIRB unit must be programmed with a unique identification code and registered with NOAA; vsl EPIRB registration had wrong call sigh unique to the vessel DRY TOURTUGAS; Prove correct call sign with NOAA epirb registration. 46CFR133.60 47CFR80.1061(e)

Each temporary source of emergency power must contain battery storage sufficient capacity to supply emergency loads; back up battery for general alarm and radio was not operational; replace battery that meets requirements in 46CFR112.37-1

Search light battery on the rescue boat shall be maintained in accordance with table 133.175, operational. Search light battery was not functional resulting in an inoperable search light. Replace battery to provide required power.

Fluid conditioner fittings (fuel filters) shall be made in accordance with the adopted standards of ASTM F 1201 in 46 CFR 56.60-1(b). Fuel filters had a glass bowl which does not meet the ASTM fire rating. Deficiency to be cleared by next drydock. 56.15-5((b), 46CFR56.60-1(b), ASTM F 1201

Each vsl must provide an Employee Assistance Program/ Drug and Alcohol Policy (EAP/DAPI) and display and distribution of information materials; VSL did not display contact info for employees. Display info in public area.

Each lifebuoys must be marked in block capital letters with the name of the OSV and name of the port of registry; the lifebuoy located on the forecastle was faded and not legible; provide markings meeting stated requirements

Vessel must carry the personnel required by the Certificate of Inspection (COI). During the inspection, vessel did not provide minimum required manning per vessel's COI; for inspection to be completed, a complete crew per the COI must be available and onboard.

The three life-buoys with self-igniting lights had expired batteries. The Starboard Bridge Buoy needs to be re-stenciled with the vessel name and port. The retro-reflective tape needed to be replaced on both the Port/Starboard Bridge buoys.

The watertight door leading to the forward storage compartment had a tightening tensioner (dog) that was loose.

The USCG and/or ABS was not made aware of major alterations affecting the safety of the vessel. Specifically, the aft bulwarks were cropped out and removed from the vessel to accommodate a job. An inspection of the hull repairs are to be observed by either a USCG Marine Inspector and/or ABS Surveyor.
