(01) distress signals found expired. Provide (01) distress signal. 46 CFR 180.68(a)(1)(2)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 06 Marlin Type 1 life jackets installed on vessel (Model 320RT for Adults and Model 321RT for Child size) have a malfunctioning spring-loaded tab on the snap hook of the safety strap. The spring-loaded tabs fail to spring back,(return to closed position) after depressing to secure snap hook in place. Renewal required for all affected Lot #'s. Vessel has 17 serviceable adult life jackets, therefore passenger count shall be reduced from 21 to 17 adult passengers until affected life jackets are replaced with serviceable Type 1 life jackets. 46 CFR 176.808(b)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Alcohol swabs on board were expired. Requires replacement. 46 CFR 4.06-15

The owner or managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service. The inspection may include an inspection of the following items: Certificates and operating manuals, including certificates issued by the FCC. FCC Safety Radio Certificate not valid by FCC licensed technician for radio installation. Provide inspection and valid FCC Safety Radio Certificate. 46 CFR 176.402

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the unmanned spaces. Forward and engine room bilge high level alarm visual indicator at helm is not operational. Provide visual indicators for forward and engine room bilge high levels. 46 CFR 182.530(a)

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Fractured welds on aft support hand holds on swim step. Repair cracked welds. 46 CFR 177.900 (b)

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The shutoff valve is degraded and unable to be operated. Replace valve. 46 CFR 182.455(b)(4)

A vessel on a limited coastwise route must carry six hand red flare distress signals. Vessel only had four had red flares. Place six total hand red flare distress signals on the vessel. 46CFR180.68(a)(1)

All wire must be installed in such a manner as to avoid chafing and other damage. Wiring for battery to starter was hanging in the bilge of the vessel. Remove wire from from the bilge. 46CFR183.340(b)(4)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Required emergency instructions not mounted at the operating station. Mount instructions. 46 CFR 185.510(a)

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Double hose clamping missing on forward and aft bilge suction and discharge hoses. Place double hose clamps on forward and aft bilge suction and discharge hoses. 46 CFR 182.410(d)(1)

Life floats and buoyant apparatus must not be stowed in tiers more than 1,220 millimeters (4 feet) high. When stowed in tiers, the separate units must be kept apart by spacers. The life floats are not separated by spacers. Insert spacers in between Life floats. 46 CFR 180.137(f)

The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float. The life floats are not marked with number of persons capacity. Mark life floats. 46CFR185.604(e)

Each ship transporting more that six passengers for hire equipped with a radiotelephone station must carry a radio operator who: holds a marine radio operator permit or higher class license. Neither of the two masters onboard had a valid radio operator permit. Provide proof of a valid radio operator permit for the radio operator. 47 CFR 80.158 (e)

The employer shall provide an Employee Assistance Program for all crewmembers that includes both education and training on the effects and consequences of drug and alcohol use. Supervisory personnel must receive at least 60 minutes of training. Vessel's O/O has not completed the mandated training for crewmembers. Provide satisfactory evidence of Employee Assistance Program training. 46CFR16.401

Connections must be made to battery terminals with permanent type connectors. Wing nuts on battery terminals. Remove wing nuts and install permanent type connectors. 46 CFR 183.350(d)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Forward starboard mounts for both port and starboard engines have failed. Install satisfactory mounts. 46 CFR 182.100

Through hull fittings and shutoff valves must be metallic. Replace nonmetallic through hull valve with approve type and provide proof of watertight integrity to Marine Inspector. 46 CFR 182.720(d)(3)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters in length carrying not more than 49 passengers, other than a ferry, provided that the pump is listed by an independant laboratory as meeting the requirements of UL 1113. Electric bilge pumps onboard should be model 10-6UL or 14A-6UL if made by Rule. 46 CFR 182.520(e)

Vessel does not currently meet the requirements of Work Instruction 31. Mandated crew posted on COI may not enter the water or act as lifeguard until policy and procedure provided. Provide written procedure. Work Instruction 31
