A vessel equipped with fixed pitch propellers must have on the navigating bridge and at the engine room control station a propeller speed and direction indicator for each shaft. Starboard side port engine control LCD screen is inoperable. Replace screen unit.

Where the general emergency alarm signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon, in addition to the general emergency alarm signal that has sufficient intensity above the background lighting that would alert personnel in the space and is activated whenever the general emergency alarm signal in the space are activated. General alarm light located in the engine room was found inoperable. Repair or replace light.

Each battery charger enclosure must meet 46 CFR 111.01-9 Each charger must be suitable for the size and type of battery installation that it serves. Battery charger for the starboard main diesel engine was found inoperable allowing the starboard engine to secure itself upon loss of power. Repair or replace battery charger.

An audible and a visible alarm must activate in the pilothouse upon failure of the electric power to the control system of any steering gear; failure of that power to the power unit of any steering gear; or occurrence of a low oil level in any oil reservoir of a hydraulic, power-operated steering-gear system. Vessels #2 steering gear alarms where found to going off intermittently for unknown reasons. Trouble shoot and repair alarm circuit.

Each navigation light system must meet the following: Dual light sources. Each self-propelled vessel must have duplicate light sources for the side, masthead, and stern lights. Port side light and anchor light secondary light sources are inoperative. Renew secondary lights for port and all-around white anchor lighting.

Each vessel must carry lifebuoys approved under 46 CFR 160.050. The grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. The life buoy grab line on the starboard side is deteriorated. Replace grab line.

A card of instructions giving a clear summary of the radiotelephone distress procedure must be securely securely mounted and displayed in full view of the principal operating position. Emergency broadcast placard missing. Replace emergency distress broadcast placard. 47 CFR 80.868

All valves and closing appliances, or other mechanisms which may be required to be operated for damage control purposes in case of an emergency shall be conspicuously marked with letters at least 1 inch high. The placards for fire dampers on main deck were faded and not legible. Replace fire damper placards.

Each battery clearly marked with an expiration date and used in an item of survival equipment must be replaced during the annual inspection if the expiration date has passed. The self-igniting lights on the ring buoys were expired. Replace batteries.

Approved fire extinguishing systems must be installed in internal combustion engine machinery spaces. Timer delay bottle on system was approved/rated to actuate supply valve at 30 seconds (in accordance with onboard procedures). The timer delay actuated at 25 seconds. Replace timer bottle so that actuation occurs at rated time stamped on the bottle (with an allowance of +20%/-0% of stamped time). With a requirement of a 30 second delay, the bottle should actuate between 30 and 36 seconds.

All alarm panels must be continuously powered. Battery back-up for engineering alarms was not operating.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. The wheel house utilizes an extension cord and power strip to power 5 different appliances creating a potential fire hazard. Provide permanently installed outlets to power appliances that are permanently utilized (i.e. microwave, refrigerator, fan, radio/battery chargers).

Illuminated exit signs are required and must be installed in accordance with subchapter J (Electrical Engineering Regulations) of this chapter. The "EXIT" sign in the passenger lounge (aft) illumination is inoperable.

Each lighting fixture must meet UL 1598A or IEC 60092-306 standards. Light in starboard engine room was missing cover. Repair cover.

Emergency lighting must be provided to allow crew to find their way to survival craft and embarkation stations. Emergency lights on the aft vehicle deck were not operating. Repair emergency lights.

The minimum system monitoring requirement for reduction gears requires a high lube oil temperature alarm. During testing, high lube oil temp alarm for port reduction gear failed. Repair alarm.

The minimum system monitoring requirement for propulsion control air is to have an alarm for low control air pressure. Alarm for low control air pressure only activated at 50 PSI, when it should activate at 90-100 PSI. Repair control air alarm.

Each lighting fixture must meet UL 1598A or IEC 60092-306 standards. Light in starboard engine room was missing cover. Repair cover.

Suitable covers, guards, or rails shall be installed in way of all exposed hazards, such as rotating machinery. Covers were missing over shaft couplers to main engines. Replace covers in way of main engine shaft couplings for both main engines.

Each life jacket aboard vessel must be approved under approval series 160.155, 160.176, or 160.177. Additionally, each life jacket container must be marked with the proper quantity of life jackets within. 2 adult size life jackets were found unserviceable. Unserviceable life jackets were in excess of required carriage amount. Replace unserviceable life jackets or change label on container to match actual number within (if unserviceable life jackets will not be replaced).

The material, design, workmanship, and arrangement must be equivalent to ABS standards for engineering systems. The support bearing on the port shaft has a loose top cap. Repair bearing.

Each lifebuoy self igniting light must be of the approved type and operational. Repair life buoy light on starboard side.

Each vessel must carry a life jacket that meets required approvals for each person on board. Replace 03 unserviceable adult life jackets.

All portable fire fighting equipment shall be maintained in accordance with NFPA 10. Exterior fire extinguishers were issing service tags. Ensure all extinguishers are properly serviced.

A vessel required to have Type I or II subdivision must be watertight below the bulkhead deck. Additionally, bilges must be kept to a minimum content at all times. Starboard rudder tube/through hull fitting is leaking approximately 0.25 gallons per minute when vessel is underway. Also, pump excess water out of steering space.

The material, design, workmanship, and arrangement must be equivalent to ABS standards for engineering systems. The water pump on the starboard main engine leaks from the seal. Repair water pump.

Emergency lighting must be provided to allow crew to find their way to survival craft and embarkation stations. Emergency lights on the aft vehicle deck were not operating. Repair emergency lights.

The operator of each self-propelled vessel 12 meters or more in length shall carry on board a copy of Inland Navigation Rules. Ensure a current copy of Inland Navigation Rules is placed in the wheelhouse.

Approved portable and semi-portable extinguishers must be installed in accordance with table 76.50-10(a). Service or replace two fire extinguishers in the engine room and the one extinguisher in the passenger space.

Each machinery space must be ventilated to ensure that, when machinery is running at full power, an adequate supply of air is maintained for operation of machinery and comfort of crew. Motor is inoperable for main supply ventilation.

Gears, couplings, and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Shaft of the potable water pump must be fully guarded from accidental contact with personnel.

Each switchboard must meet the requirements of IEEE code 45-2002. Switchboard rear access is not locked. Provide lock for access.

The material, design, workmanship, and arrangement must be equivalent to ABS standards for engineering systems. The dump valve on the port main engine starter leaks excessive air when engine is started. Repair dump valve.

Fire pumps shall be fitted with a pressure gauge on the discharge side of the pumps. The installed pressure gauge was not working. Repair/replace gauge.

Emergency lighting must be provided to allow crew to find their way to survival craft and embarkation stations. Emergency lights on forward starboard vehicle deck and inside the CO2 room were not operating. Repair emergency lights.

Electric installations on vessels must ensure maintenance of services necessary for safety under all conditions. Water was discovered filling the battery box aft of the wheelhouse. Remove water and ensure safe installation of electrical equipment.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. While transiting ICW vessel experienced vibrations. Upon third party diver inspection, damage to both propellers was identified. Conduct appropriate permanent repairs and operational testing to the satisfaction of the attending Marine Inspector.

46 CFR 16.401 The employer shall provide an Employee Assistance Program (EAP) for all crew members [name withheld] must include an education program. The EAP education program is not displayed and distributed in accordance with 46 CFR 16.401(a). Implement an EAP education program in accordance with 46 CFR 16.401(a).

46 CFR 78.37-5 The master or person in charge of a vessel that is not required by 46 U.S.C 11301 to have an official logbook, shall maintain, on board, an unofficial logbook or record in any form desired for the purposes of making entries therein as required by law or regulation in this subchapter. Watertight door operation, loudspeaker system, steering gear, whistle, and means of communication, emergency lighting and power systems are not being logged in accordance with 78.37-5. The Department of Transportation for the State of Louisiana must develop a corrective action plan for maintaining required log entries.

46 CFR 112.15-5(a) The emergency lighting power loads listed in paragraphs (b)-(l) shall be arranged so that they can be energized from the final source. All emergency lighting in the steering compartment, shaft alley, and all lights in the engine room except for one, did not illuminate when the power from the generator was secured. Repair all engine room emergency lighting to the satisfaction of the Marine Inspector.

46 CFR 111.15-25(a) An overload protective device shall be placed in each battery conductor, except that engine cranking batteries need not be protected against overload. Both 12 volt battery banks located port and starboard aft corners of the engine room have several conductors attached with no overload protection. Provide fuses for all wires that are not for engine cranking to the satisfaction of the Marine Inspector.

46 CFR 111.15-25(a)An overload protective device shall be placed in each battery conductor, except that engine cranking batteries need not be protected against overload. Both generator starting battery banks have auxiliary wires attached other than cranking leads that do not have overload protection. Provide fuses for all wires that are not for engine cranking to the satisfaction of the Marine Inspector.

46 CFR 111.30-159b)(7)Adequate means for ground detection shall be provided on the ship service generator and distribution switchboard for ship’s main power system. The center bulb for 240 volt ground detection is burned out. Replace bulb in kind.

46 CFR 78.47-30(a)Each hand portable fire extinguisher shall be marked with a number and the location where stowed shall be marked with a corresponding number at least 1 /2 inch high. Four extinguishers outside of the superstructure and all of the engine room fire extinguishers were not numbered to their corresponding location. Mark portable extinguishers to their corresponding location to the satisfaction of the attending CG Marine Inspector.

46 CFR 112.15-5(a)46 CFR 112.15-1(h)Emergency lighting power loads shall be arranged so that they can be energized by the final source. The starboard aft and aft door “Exit” signs on the main deck superstructure leading to the open deck did not illuminate during the emergency power test. Repair or replace “Exit” signs to illuminate on emergency power to the satisfaction of the attending CG Marine Inspector.

46 CFR 112.15-5(a) 46 CFR 112.15-1(h)Emergency lighting power loads shall be arranged so that they can be energized by the final source. The port forward and starboard aft exterior emergency lighting did not energize during the emergency power test. Repair or replace the emergency lighting to the satisfaction of the attending CG Marine Inspector.

46 CFR 112.05-10(d)Emergency lights shall be marked with a letter “E” of at least a ½ inch in height. The port forward exterior emergency light was not marked with an “E” and lighting in the E/R is marked with an "E" that is not emergency lighting or is not near a light fixture. Ensure all emergency lights are marked with a letter “E” and remove unnecessary "E" designations to the satisfaction of the attending CG Marine Inspector.

In addition to the emergency power sources required under part 112 of this chapter, each self-propelled vessel and each mobile offshore drilling unit must have at least two electric generating sources. Port generator is inoperable. Repair generator to the satisfaction of the Marine Inspector prior to carrying passengers.

Each lifejacket must be marked with Type I retro-reflective material approved under approval series 164.018. The arrangement of the retro-reflective material must meet IMO Resolution A.658(16). 29 of the 156 lifejackets on board retro-reflective tape was found worn and lifejackets need to be replaced. Vessel will reduce the amount of passengers allowed on board to 125 until lifejackets are replaced to the satisfaction of the attending marine inspector.

All general alarm bells shall be identified by red lettering at least 1/2 inch high: “GENERAL ALARM—WHEN BELL RINGS GO TO YOUR STATION.”. The general alarm located forward of the bridge near the main deck is not marked with the required red lettering.

Each hand portable fire extinguisher shall be marked with a number and the location where stowed shall be marked with a corresponding number at least 1/2 inch high. portable fire extinguishers in the E/R were found mislabeled. Vessel will correctly label the fire extinguishers IAW their fire control plan to the satisfaction of the attending marine inspector.

The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in a satisfactory condition. Vessels visual and audible alarm for the steering gear space emergency escape hatch was found inoperable. Vessel will fix to the satisfaction of the attending marine inspector.

An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs or alterations are undertaken. The second ship service generator starter has failed rendering the generator inoperable. Upon completion of repairs (installation of new starter) an inspection and testing of the generator shall be done to the satisfaction of a Coast Guard Marine Inspector.

Each pump shall be capable of delivering water simultaneously from the two highest outlets at a Pitot tube pressure of approximately 50 p. s. i. Both fire pumps are only producing 20 PSI. Repair pumps to produce 50 PSI.

Distribution panels of the switchboard type shall be completely enclosed or otherwise protected against accidental contact and unauthorized operation. The 120V distribution panel in the aft of pilothouse has broken enclosure locking mechanism and the front of the panel is missing all but 2 securing screws. Repair distribution panel cover and locking door mechanism.

Each item of lifesaving equipment carried on board the vessel in addition to those required by this part must be accepted by the cognizant OCMI for use on the vessel. All three blades of the rescue boat propeller are damaged with chunks of leading edge missing. Replace rescue boat propeller.

Each general emergency alarm signal must be marked ‘‘GENERAL ALARM—WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATION’’ in red letters at least 1/2 inch high. The general alarm bells on the main deck exterior were not marked. Mark as appropriate.

Each lifejacket must be marked in block capital letters with the name of the vessel. Two lifejackets were found to be marked with a different vessel name. Properly mark the lifejackets with the appropriate vessel name.

Each lifebuoy stowage position must be marked with either the words ‘‘LIFEBUOY’’ or ‘‘LIFE BUOY’’, or with the appropriate symbol from IMO Resolution A.760(18). The stowage location for lifebuoys stowed on the main deck were not legible. Mark lifebuoy stowage locations appropriately.

46 CFR 199.70(a) requires that each vessel carry lifebuoys approved under approval series 160.150. Amount of retro-reflective material on each lifebuoy was not of sufficient quantity or sufficient condition. Replace retro-reflective material to ensure a sufficient width (approx. 5cm) is applied around or on both sides of the body at four evenly spaced points.

Liferaft Hydrostatic Release: Each liferaft or group of liferafts must be arranged for float-free launching. The arrangement must ensure that the liferaft or liferafts, when released and inflated, are not dragged under by the sinking vessel. A hydrostatic release unit (HRU) used in a float-free arrangement must be approved under approval series 160.162. The manufacturer's instruction on the installed disposable HRU for the vessel's survival craft requires an expiration date of two years after installation date. The expiration date required to be marked on the HRU was not marked and the vessel rep had no objective evidence of when it was installed. Install a new HRU and mark the expiration date in accordance with the manufacturer's instructions.

An emergency means of communication required by this subpart must— (1) Be comprised of either fixed or portable equipment; and (2) Provide common talking means of two-way voice communication and calling among the navigating bridge, emergency control stations, muster stations, embarkation stations, and other strategic positions listed in § 113.30–5. The crew does not carry a portable means of two way communication and the call button on the comms boxes located on main deck (exterior) do not operate to call the bridge. Prover proper operation of all call boxes onboard.

The vessel is restricted to Bollinger shipyard and is not allowed to get underway until a complete root cause analysis is conducted o nthe entire steering system. This analysis is to include all mechanical, Hyd. And electrical components of the entire system. With all parameters tested, measured, observed, in the aforementioned sections of the steering system. A detailed report of the condition of the system and its serviceability for the future.

When alteration or repair of machinery becomes necessary, the work shall be done under the cognizance of the Officer in Charge Marine Inspection. The port shaft cooper bearing failed rendering the port main engine inoperable. Repair to the satisfaction of attending USCG Marine Inspector prior to carrying passengers.

Onboard training, musters, and drills must be in accordance with Subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Drills were not witnessed during COI due to availability of personnel during COVID-19. Drills to be conducted at permanent operating location prior to carriage of Passengers to satisfaction of CG Marine Inspector. 46CFR78.17-50 701 c
