Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems must: (1) be maintained to ensure proper operation. Both shafts are leaking steadily beyond the recommended drips per minute interval when in operation. Repair shaft seals to the satisfaction of the attending USCG Marine Inspector.46 CFR 143.205(c)

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid. The port and starboard side auxiliary generators are leaking oil onto the generator windings. Replace seal or gaskets on both auxiliary generators to the satisfaction of the attending USCG Marine Inspector.46 CFR 143.220(a)

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crew-members are capable of performing the duties expected of them during emergencies. This includes abandoning the vessel, recovering persons from the water, responding to onboard fires and flooding, or responding to other threats to life, property, or the environment. During man overboard and fire drills, crew did not communicate effectively and demonstrated a lack of familiarity with drill procedures.Conduct drills as prescribed and document in Official Log to the satisfaction of the Coast Guard. 46 CFR 140.420 (a)

The following systems must be equipped with gauges at the machinery location; auxiliary generator engine cooling water temperature. The starboard side auxiliary generator temperature gauge was broken and inoperable. Replace gauge to the satisfaction of the attending USCG Marine Inspector.46 CFR 143.230(c)(4)

All lifesaving equipment must be maintained in accordance with 46 CFR 199.190. 46 CFR 199.190(e)(1) requires monthly inspections of all lifesaving appliances. No evidence of the completion of monthly checklist inspections was provided. Provide evidence of required monthly inspections to attending Marine Inspector.46 CFR 141.235 (a)

Sufficient lighting suitable for the marine environment must be provided within the crew working and living areas. In the crew accommodation on the second deck, the primary light between the bunk beds is missing the protective cover, exposing the florescent light bulbs, and causing a safety and electrical hazard. Repair or replace the light fixture to the satisfaction of the attending USCG Marine Inspector. 46 CFR 143.410(a)

The frequency of the tests and inspections required by this section must be: (3) For vessels, annually or as part of the biennial and mid-period inspections. Fuel transfer hose has not been tested within the last year. Test hose and provide certification to the Coast Guard. 33 CFR 156.170 (f)(3)

Piping for flammable or combustible liquid, seawater cooling, or firefighting systems must be fitted with isolation valves that are clearly marked by labeling or color coding that enables the crew to identify its function. Fuel supply pipe fittings are not color coded from tank to engines. Color code fuel supply valves in accordance with the posted color scheme instructions onboard (yellow). 46 CFR 143.250(c)

Each lifebuoy on a towing vessel must: (1) Be approved under approval series 160.050 or 160.150. Retroflective material was badly deteriorated and replace with unapproved red retro tape. Renew all lifebuoy retro tape with Coast Guard approved retro material. 46 CFR 141.360 (b)(1)

hen worn at night, the work vest must be equipped with a light that meets the requirements of 46 CFR 141.340(g)(1). Work vests may not be substituted for the life jackets required by 46 CFR part 141. (b) Each storage container containing a work vest must be marked “WORK VEST”. Work vest were stowed along with survival life jackets and not properly marked. Work vests did not have lights attached. Attached lights to all Work Vests and store separately from other life jackets.46 CFR 140.430 (a)&(b)

The records of inspections and tests of portable fire extinguishers and semi-portable fire-extinguishing systems may be recorded in accordance with paragraph (c)(1) of this section, or on a tag attached to each unit by a qualified servicing organization. There are no records of firefighting equipment maintenance after certification in January by an NFPA certified 3rd party. Provide evidence of monthly maintenance checks on firefighting equipment. 46 CFR 142.240 (c)(2)

The results of the inspections required by 33 CFR 164.76 must be documented in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel. A record of the type, size, and service of each towline, face wire, and spring line,used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. No records provided of inspections, maintenance, and replacement of facing wires and wenches. Provide records of towing wire inspections, replacement, and certification and log in TVR or Official Logbook as required by 140.915. 46 CFR 140.820

46 CFR 143.230(a)(5) Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided: (5) High bilge levels. Bilge alarm was duct taped to a metal plate laying in the bilge. Permanently mount bilge alarm.

46 CFR 142.220 Each towing vessel must be maintained and operated so as to minimize fire hazards. All 4 racor fuel oil filters did not have a plug in the bottom to prevent fuel spillage if drain valve was to vibrate open. Install pipe plugs in racor fuel filter drain piping.

46 CFR 144.800 Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. In areas where space limitations make deck rails impractical, such as at narrow catwalks in way of deckhouse sides, hand grabs may be substituted. Vessel does not have rails or equivalent surrounding the lower deckhouse. Provide grab rails around lower deckhouse on port and starboard sides.

46 CFR 140.405(b) Any towing vessel with alternating watches (shift work) or overnight accommodations must identify the duties and duty stations of each person onboard during an emergency. Duties and duty stations of each person onboard during an emergency are not identified. Identify the duties and duty stations of each person onboard during an emergency and post the emergency duties and duty stations at each operating station and in a conspicuous location in a space commonly visited by crewmembers.

46 CFR 140.635(a) The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Navigation assessments are not been conducted prior to getting underway. Conduct navigation assessments prior to getting underway and record in the Towing Vessel Record.

46 CFR 140.645 Prior to assuming duties related to the safe operation of a towing vessel, each crewmember must receive navigation safety training. Crewmembers have not received navigation safety training. Conduct navigation safety training in accordance with 46 CFR 140.645.

46 CFR 143.245(b) Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Vessel had no test records for Communications, General Alarm, Emergency Lighting, Charge of storage batteries, Pressure relief valves as per Table 143.245(b). Provide test results of the above tests and properly record test results in appropriate vessel log book.
