The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the authorized ballast water management methods listed in 33 CFR 151.2025(a). Vessel’s ballast water management system is inoperable. Vessel must comply with one of the methods listed in 33 CFR 151.2025(a) until system is repaired. 33 CFR 151.2025(a)

Each cable installation must meet Sections 25, except 25.11, of IEEE 45-2002. Metal braid on several cargo deck cable installations is brittle and wasted. Replace corroded metal braid as needed by next dry dock. 46 CFR 111.60-5(a)

Each cable entrance into explosionproof or flameproof equipment must be made with approved seal fittings, termination fittings, or glands that meet the requirements of 111.105-9. Cable terminations into closed-gauging equipment were sealed with silicon. Reseal cable terminations using an approved method for hazardous locations to the satisfaction of the Coast Guard. 46 CFR 111.105-17(c)

Manned tank barges and tankships authorized to carry Grade A, B, C, or D liquids at any temperature, or Grade E liquids at elevated temperatures shall be provided with a combustible gas indicator suitable for determining the presence of explosive concetrations of the cargo carried. Vessel's fixed gas sampling system just received, and installed, a new sensor causing the system to give a constant alarm and not function as designed. Vessel's fixed gas sampling system must be tested to the satisfaction of the USCG and operate as designed. Vessel shall not carry flammable cargoes. 46 CFR 35.30-15(b)

Non-self propelled ships must meet the requirements of this part (46 CFR 153) when the Officer In-Charge, Marine Inspection, issues a United States ship an IMO certificate endorsed to allow the carriage of hazardous material or NLS cargo in Table 1 of this part (46 CFR part 150, Table 1) in accordance with MSC. 460(101) updates to IBC all cargoes listed on the vessel's Cargo Authority Attachment and International Certificate of Fitness for Carraige of Dangerous Chemicals in Bulk may only be carried proved the vessel meets the requirements of MSC. 460(101), MEPC 318(74), and MEPC 315(74). Vessel's owner is required to contact Marine Safety Center to update Cargo Authority Attachment and request issuance of a new International Certificate of Fitness for the Carriage of Dangerous Chemicals in Bulk. 46 CFR 153.12(d)

Vessel must comply with hazardous location requirements. The Halogen light and housing located on top of the nitrogen generator house is not explosion proof or intrinsically safe.

The explosion proof light fixture located on top of the machinery room on the starboard side by the slop tank P/V was found with a cracked housing and is no longer explosion proof. Repair or Replace.

Anchor was found to have fractures on the inside of the links. Barges equipped with anchors are to comply with standards issued by a recognized class society. Vessel to provide class inspection report.

The semiportable fire extinguisher was found to have the bottle neck highly corroded. Portable and semiportable extinguishers must be maintained in accordance with NFPA 10.

Hazardous location drawing had incorrect distances and must be updated to reflect the actual hazardous location.

ABS Surveyor discovered ODME system is not operable. Vessel must hold slops and discharge to shore until ABS clears the deficiency.

Seal holes at top of distribution panel (cargo control room, lower level).

Repair fire block for cableway near cargo control room door.

Show compliance with section 7 of CG-CVC Policy Letter 16-04.

The fixed gas detection system was inop. System could not be calibrated.

Provide corrective action plan attesting to the proper training and record keeping of all required gas detection meters located on the barge. The crew did not have procedures for the calibration of the oxygen CO2 alarm for the NGG space.

Vessel has SOVC issued by ABS. Vessel needs valid IAPP cert. by 08Jan12 to operate on international routes. Restricted vessel to domestic voyages only.

Complete ISE prior to operating.

Vessel is required a hydrostatic twice in any five year period, not to exceed 3 years, due to the inability of a satisfactory internal examination.

Repair the port running light and the forward anchor light; insure the aft anchor light is working properly. A temporary port running light shall be rigged for night time operations until repairs have been affected.

Repair the E-stop switch box for the #1 residue pump on deck; there are two broken screws on the cover plate.

IAW 46 CFR 153.236(b) replace any brass fittings from the VCS piping to sat of attending MI.

Update Cargo Authority Attachment (CAA) & amend COI to reflect latest approved CAA & to meet same reqmnts as the sister ship PETROCHEM SUPPLIER (ON 1211793). Contact cognizant OCMI to obtain amended COI, CAA & Cert. of Fitness and attachment NLT 30SEP09.

P/V valve for cargo pump #2P is to be moved a min distance of 15 meters from any opening or air intake to accommodation & service spaces. Repair plan to be submitted w/ completion of approved repairs to be NLT 30SEP09. Certain cargoes (see scanned 835) are not to be carried in cargo tank #2P w/o prior approval from cognizant OCMI & use of add. monitoring during cargo ops.
