Upon arrival, MI's observed the the emergency generator was not in the automatic start mode prohibiting the emergency generator from automatically starting. Ensure procedures are in place prior to getting underway to verify e-gen is in automatic mode and that the master is notified of this status. Provide USCG with proof of this procedure.

It shall be the duty of the owner, master, or persons in charge to see that the vessels firefighting equipment is at all times ready for use. MI's requested op test of the emergency fire pump activated from the pilothouse. The pump did not start due to open breakers within the electrical system rendering the pump inoperable. Chief engineer identified breakers and corrected the issue and conducted sat test of the fire pump. Ensure procedures are in place prior to getting underway to verify emergency fire pump is operational and that the master is notified of this status. Provide USCG with proof of this procedure.

Electrical installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Breaker #20 is tripping when shifting from shore power to ship power, and does not provide clear visually indication of a trip fault as it only jumps approximately 1/8 of an inch. This has resulted in the vessel running on battery power to the point where the vessel goes dead ship while conducting ferry operations across the Houston Ship Channel. Breaker #20 is to be replaced with a new breaker to the satisfaction of the [name withheld] and corrective actions to prevent recurrence shall be developed, carried out and provided to the satisfaction of the MI.

MI's observed missing and sheared engine mount bolts. Renew and replace missing/sheered engine mount bolts to the satisfaction of MI.

Vessel crew was unable to demonstrate proper operation of the safety relief valve on the #1 air receiver when asked by the attending USCG Marine Inspector.
