Vent outlets for cargo loading, discharging and ballasting required by regulation 11.6.1.2 shall permit the free flow of vapor mixtures. PSCO observed all flame screens on P/V valves 1 through 6 on port and starboard tanks, as well as P/V breaker painted over restricting flow. 74SOLAS(04)II-2/4.5.3.4.1.1.3

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers, unless the equipment is appropriately certified for safe usage in the dusts, vapors or gases likely to be encountered. Bonding wires were found to be deteriorated or missing from piping in cargo pump room, on inter gas piping and cargo piping on deck. 74SOLAS(04)II-1/45.10.4

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and operational nature of the deficiencies noted provide objective evidence of a lack of effectiveness of the implementation of the ISM code. An internal audit is required within 03 months. 74SOLAS(20)IX/3.1 ISM PART A/10.1

A ship which is designed, constructed and maintained in compliance with the appropriate requirements of an organization including a classification society, which is recognized by the Administration or with applicable national standards of the Administration in accordance with the provisions of regulation 2-1, may be considered to provide an acceptable level of strength. The above provisions shall apply to all structures, equipment, and fittings covered by this annex for which standards for strength and construction are not expressly provided. On 04 APR 2019 at approximately 0558 while mooring at Buckeye Dock 3, the vessel allided with the dolphin pier tie off, forward starboard side way of number one ballast tank. Puncture approximately 1 foot by 3 inch approximate 8 feet above the water line. Vessel restricted in movement to inner harbor until classification survey is complete. ICLL (88) Annex B/Reg 1.2

0610- Lifeboats- A lifeboat which has a water spray fire-protection system shall comply with the following: the system shall be arranged for flushing with fresh water and allowing complete drainage. Crew was unable to provide means to flush with fresh water. SOLAS 74/78 III/34, LSA Code 4.9.2.3

1220- Freeboard Marks- The ring lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. PSCO observed freeboard lines on the port side were not appropriatly painted. ICLL 66 as modified Annex I/8 Rectify prior to next U.S. Port after sailing foreign.

2099- Other SOlas Operational Def.- Crew members with enclosed space entry or rescue responsibilities shall participate in an enclosed space entry and rescue drill to be held on board the ship at least every two months. The crew has not completed an enclosed space rescue drill withinn two months (April 25, 2016) SOLAS 74/78 III/19.3.3

1899- Other (tankers)- Each cargo tank of a tank ship must be equipped with an intrinsically safe high level alarm and overfill alarm. PSCO observed audible alarm for overfill not sounding on deck. Crew cleaned air line and tested alarms satisfactorily. 46 CFR 39.2007

US Coast Pilot not onboard vessel. Receipt for purchase of US Coast Pilot must be faxed to USCG prior to departure. 33 CFR 164.33

Oxygen levels in slop starboard and slop port tanks carrying cargo (unleaded gasoline) are at 9.2% and 10.8%. Bring oxygen content in all cargo tanks to no more than 8% by volume. Rectify prior to movement. FSS Code Ch. 15 2.1.3.2

Remove wooden decks in way of gangways to center catwalk from both port and stbd sides. The wooden decks are deteriorating. Have this completed prior to next US port after sailing foreign to the satisfaction of Class.
