No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers, unless the equipment is appropriately certified for safe usage in the dusts, vapors or gases likely to be encountered. Bonding wires were found to be deteriorated or missing from piping in cargo pump room, on inter gas piping and cargo piping on deck. 74SOLAS(04)II-1/45.10.4

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and operational nature of the deficiencies noted provide objective evidence of a lack of effectiveness of the implementation of the ISM code. 0610- Lifeboats- A lifeboat which has a water spray fire-protection system shall comply with the following: the system shall be arranged for flushing with fresh water and allowing complete drainage. Crew was unable to provide means to flush with fresh water. SOLAS 74/78 III/34, LSA Code 4.9.2.3

1220- Freeboard Marks- The ring lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. PSCO observed freeboard lines on the port side were not appropriatly painted. ICLL 66 as modified Annex I/8 Rectify prior to next U.S. Port after sailing foreign.

2099- Other SOlas Operational Def.- Crew members with enclosed space entry or rescue responsibilities shall participate in an enclosed space entry and rescue drill to be held on board the ship at least every two months. The crew has not completed an enclosed space rescue drill withinn two months (April 25, 2016) SOLAS 74/78 III/19.3.3

1899- Other (tankers)- Each cargo tank of a tank ship must be equipped with an intrinsically safe high level alarm and overfill alarm. PSCO observed audible alarm for overfill not sounding on deck. Crew cleaned air line and tested alarms satisfactorily. 46 CFR 39.2007
