Resolution: Crew of vessel removed and replaced the ME Unit #5 cracked cylinder head. Main engine operation test was carried out with no issues. Class attest to normal operations.

In tankers, electrical equipment, cables and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the organization. PSCO discovered 12 cracked explosion proof lights in the pump room, with the majority being on the lowest level. Cargo transfer is prohibited until lights are replaced, taken out of service with risk mitigation in place. 74SOLAS(04)II-I/45.11
